Description

COMBINED SYNOPSIS/SOLICITATION Provide seven (7), storage carts to store vascular Ent, Orthopedic and Ophthalmology sterile supplies in the operating room. Mass Medical Storage Model numbers are attached. This is a brand name or equal requirement. The order is to be delivered to the Wilmington VA Medical Center, 1601 Kirkwood Highway, Wilmington, DE 19805. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This acquisition is Set-Aside for Service Disable Veteran Owned Small Business Concerns. All Small Businesses submitting an offer must be registered on https://www.sam.gov and https://vip.vetbiz.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). This solicitation is issued as a Request For Quote (RFQ). Submit written quotes on RFQ Number VA244-17-Q-1737. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94 and 2005-95. This procurement is restricted. The associated NAICS code is 339113. The small business size standard is 1000 employees. This combined solicitation/synopsis is for purchase of the following commercial items: Required Items: No Substitutions. No Grey Market Items. SPECIFICATIONS: Must be made of metal and stainless steel Must have premium casters Shelves, trays, baskets and bins must be interchangeable Carts are can be easily reconfigured for new procedures ABS side panels Model Description Quantity Price See the Attached For Items and Model Required delivery October 20, 2017. Place of delivery is Wilmington VAMC, 1601 Kirkwood Highway, Wilmington, DE 19805. Delivery shall be FOB Destination to Wilmington, DE. FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jun, 2008), applies to this acquisition. Evaluation - This acquisition will be conducted in accordance with Federal Acquisition Regulation (FAR) Subpart 15.101-2, and as supplemented by agency procedures using the Lowest Price Technically Acceptable source selection process. Technical ability will be evaluated as either Acceptable/Unacceptable, Pass/Fail, or Go/No-Go. Tradeoffs are not permitted. The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (Jan 2017), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at https://www.sam.gov. If an offeror has not completed the annual representations and certifications electronically at the SAM website, the offeror shall complete only paragraphs (c) through (o) of this provision.
