Description
Siemens Radiology Service Agreement Background: The Coatesville VAHCS has many different departments that offer clinical services. One such department is Radiology. A number of different pieces of equipment are utilized and maintenance on the equipment needs to be completed. Scope: This contract is to provide preventive maintenance and full maintenance and repair services on the Siemens radiology equipment located at the Coatesville VAHCS. Specific Tasks: This service agreement is for full maintenance and repair services on the Siemens equipment listed below. The contractor shall provide unlimited emergency repair and call-back repair services as well as preventative maintenance inspections (PM inspections frequency will be based on the manufacturer s recommendations) to include all parts, labor, travel and all other materials including x-ray tubes and wi-FD detectors for the equipment listed below. This agreement will last through FY17 (10/27/17 10/26/18). Equipment The contractor shall provide maintenance, as described above, on the equipment listed below: EE# Serial # Manufacturer Model 37272 1280 Siemens AXIOM Luminos Agile Flouro 38914 42628 Siemens Magnetom Aera Scheduled Maintenance The contractor will maintain, check and calibrate, according to manufacturer s procedures and recommended schedule for all the included equipment identified above. There will be preventative maintenance (PM) visits during the contract period based on the OEM s recommendations. Scheduling months for the PMs will be determined after contract award. Scheduling Maintenance Procedures - Upon request, Contractor must provide Biomedical Engineering with a written description of the procedures to be performed on each equipment item. The description must be provided in sufficient detail so as to be acceptable to the field inspectors of the Joint Commission. Scheduling Arrangements: Except by special alternative arrangement, scheduled maintenance will be provided only during Contractor s normal working hours (8:00 a.m. to 5:00 p.m., Monday thru Friday, excluding holidays). Upon acceptance of this contract, the Contractor shall immediately contact Radiology Service to schedule mutually agreeable times for the performance of inspections. Contractor will inform Biomedical Engineering of these times. Repair Services: This contract will include unlimited emergency repair and call-back repair services to include all general spare parts, labor, travel and other materials including the x-ray tube and wi-FD detectors. The Contractor will provide a five-day, Monday thru Friday, 8:00 a.m. to 5:00 p.m., emergency repair service for urgently needed equipment. This will exclude government holidays. Contractor will repair on request all equipment identified above. All equipment repaired by Contractor must be restored to proper working order and subjected to appropriate safety checks before being returned as fit for use.
