Statement of Work Date 06/13/2017 Client Hampton VAMC Job Name Acquisition of 3D Printers Requested by JOHN HARMSEN Summary 3D printing in medicine offers the opportunity to streamline fabrication of prosthetics and medical models. Processes that previously took days or weeks with many hours of skilled labor performing many steps consuming pounds of extra materials can now be generated with additive fabrication the same day, using little manpower and little more than the exact quantity of materials needed. This streamlined workflow represents a significant reduction in cost to the facility, as well as a paired increase in the quality of patient care, interprofessional training, and education. Project Scope To procure 3D printers to be utilized by the Prosthetics, Radiology & Surgical services. The flexibility that 3D printing provides is ideal to benefit the medical professional, trainee and medical facility. Benefits to the facility include reduced length of stay due to decrease surgical time, increase operating room turnover, and optimal patient outcomes. Prosthetics/Orthotics: The in house 3D production process would occur as follows: Patient s anatomy is scanned by prosthetist. Post processing is completed by 3D lab tech with prosthetist s supervision. 3D printed model is fabricated and reviewed by prosthetist. Patient is seen in the lab to confirm fit If adjustment is required, return to step 1 or 2. Surgical: Decreased OR Time/Increased OR Turnover/Improved Patient Outcomes Surgeons now have the ability to use 3D imaging to scan patients create models to be used for pre-surgical planning, and, in some cases, to determine whether patients require and are able to survive surgery. Cardiologists are now printing accurate models of their patient s vasculature to determine whether their anatomy will allow for a catheterization, with the goal to reduce the amount of iatrogenically caused embolisms.3 Orthopedics now will have the ability to scan the trauma patient and immediately see where displaced or damaged bone needs to be moved to match the uninjured areas, and where screws for the implants can be seated correctly for a successful outcome. Surgeons can now quickly design patient-specific surgical guides and custom implants, as well as set up scenarios for practice sessions with prototypes that can be cut, drilled, and practiced on. THIS PURCHASE COVERS BOTH PRINTERS ONE FOR PROSTHTICS AND ONE FOR IMAGING. Using prosthetic limbs as an example, we can see how materials and labor cost can be significantly decreased with the incorporation of 3D printing. Making a traditional prosthesis for an amputee can take up to 7 steps and several weeks to generate the final product. Patients are brought into the office, where their residual limb is casted with plaster or fiberglass to generate a negative model of the patient s residual limb. This casting is then filled with plaster to create a positive 3D model of the patient s limb. This model is then manually modified to shape it to apply customized pressures based on the patient s anatomy so that the prosthetic socket will provide a comfortable interface between the limb and the prosthesis, while strategically applying pressures to distribute weight in a tolerable way and ensure that the prosthesis will grip the residual limb securely. Clear plastic is then heated and placed over this custom modified model to make a check socket to trial on the patient. The advantages of 3D printing are apparent when you compare the old strategies to the new. In addition to rapid and accurate turnaround, this process would offer the additional benefit of documented data capture of a patient s residual limb morphology changes over time; this documentation in turn could theoretically be used to support the need for replacement prostheses for documentation purposes.
