Description
52 215-5 T & C This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number HHSI2462017Q0052 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. This solicitation is 100% set-aside for small business. The associated NAICS code is 511210 and the small business size standard is $38.5M. Specifications: GI Software compatible with the soon to be discontinued EndoWorks. Must be capable of providing procedure documentation, image capture and management and HL7 integration solutions with Electronic Medical Records. The contractor must provide exclusive industry insight into the EndoWorks platform and be granted exclusive rights to the EndoWorks Data Migration Toll offered by Olympus. 1 at $_________. Dates and places of delivery and acceptance and FOB point is Lawton Indian Hospital, 60 days ARO or earlier if available. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition no addenda to the provision is applicable. The provision at 52.212-2, Evaluation-Commercial Items will not be used. All evaluation factors and that will affect the contract award are Technical and Past Performance and Cost or Price. The evaluation factors other than cost or price, when combined are approximately equal to cost or price. The Indian Health Service will not consider your quote for award if you fail to respond to the following evaluation criteria. Technical: Must be capable of providing procedure documentation, image capture and management and HL7 integration solutions with Electronic Medical Records. Contractor must be able to ensure they are capable of providing the services and expertise needed to partner with Olympus EndoWorks users. The contractor must provide exclusive industry insight into the EndoWorks platform and be granted exclusive rights to the EndoWorks Data Migration Toll offered by Olympus. Past Performance: Provide three references that include similar experience in providing software to similar hospitals. Include name of contractor, contractor contact, current phone number, current e-mail address and dates of service. Must be registered in the System for Award Management (SAM) database (reference https://ww.sam.gov Please note that all parties interested in conducting business with the Indian Health Service must be registered in the SAM database. Evaluation Rating Information Five: Outstanding - Quote meets requirements and indicates an exceptional approach and understanding of the requirements. Strengths far outweigh any weaknesses. Significantly exceeds performance or capability standard. Risk of unsuccessful performance is very low. Four: Good - Quote meets requirements and indicates a thorough approach and understanding of the requirements. Exceeded some performance or capability standards. Proposal contains strengths, which outweigh any weaknesses. Risk of unsuccessful performance is low. Three: Acceptable - Quote meets requirements and indicates an adequate approach and understanding of the requirements. Strengths and weaknesses are offsetting or will have little or no impact on contract performance. Risk of unsuccessful performance is no worse than moderate. Two: Marginal - Quote does not clearly meet requirements and has not demonstrated an adequate approach and understanding of the requirements. The quote has one or more weaknesses, which are not offset by the strengths. Risk of unsuccessful performance is high. One: Unacceptable - Quote does not meet requirements and contains one or more deficiencies. Proposal is unawardable. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. Federal Acquisition clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. Additional clauses or provisions are attached. Federal Acquisition clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition, and is attached.
