Description
Responses and/or questions to this RFI must be submitted via e-mail to the points of contact listed. E-mail responses should be no larger than 5 megabytes (MB) in size. If larger attachments are required, additional methods to submit responses include: Sending documents separately due to size restrictions, as long as each e-mail indicates how many total e-mails are being sent; or The following website can be used for file transfer: AMRDEC safe site file transfer: https://safe.amrdec.army.mil/safe/. The vendors’ responses shall include all detailed information requested in the RFI Vendor Response Worksheet and shall also include: (1) Company name and address; (2) Company POC (Name and Phone Number, Fax Number, and E-mail address); (3) Company website information; and (4) the Company’s Commercial and Government Entity (CAGE) Code. Each RFI response will receive a confirmation e-mail. REQUEST FOR INFORMATION (RFI) Disposable Shoe Covers 1.0Introduction 1.1General. The purpose of this request is to obtain information from industry to support the government's intent to establish a Qualified Supplier List (QSL) for the standardization of Disposable Shoe Covers. This RFI is an invitation to submit to the Defense Health Agency (DHA) Medical Logistics (MEDLOG) Division Medical Materiel Enterprise Standardization Office (MMESO) the following: technical information and all related supporting documentation for Disposable Shoe Covers. This request is a market research effort undertaken by the government which will be used for information gathering purposes. The information received may be used for Acquisition Planning purposes and may help define future requirements for the subject item(s). This is not a request for quote or proposal. 1.2RFI Submission Package. Vendors must provide responses to this RFI via the RFI Vendor Response Worksheet and Addendum. Complete answers and all supporting documentation for each product number must be provided with the vendor's response. Responses for each salient characteristic should be in the form of yes, no, or alternate proposal. Although explanatory comments may be provided for all responses, they are required for "no" responses. 2.0Scope Examples of items for this product line could include but are not limited to: Disposable Shoe Covers, boot covers, and or ankle height shoe covers designed to be worn on the shoes of clinicians and hospital workers. 3.0Characteristics/Specification Feedback Requested: Respondents are requested to provide information concerning their Disposable Shoe Covers regarding the following questions: 3.1Are your Disposable Footwear (shoe and boot covers) products manufactured in a TAA compliant country? Please identify the country of origin. 3.2Are your Disposable Footwear (shoe and boot covers) products made with natural rubber latex? 3.3Are your Disposable Footwear (shoe and boot covers) products FDA approved? Please provide documentation to support approval. 3.4Are your Disposable Footwear (shoe and boot covers) products made from Spunbond + Meltblown + Spunbond Nonwoven (SMS) material? Spunbond polypropylene material? Please list types of material used to make Disposable Footwear. 3.5Do you offer knee high boot covers in your Disposable Footwear (shoe and boot covers) products line? 3.6Do you offer ankle height shoe covers products in your Disposable Footwear (shoe and boot covers) products line? 3.7What AAMI level do your Disposable Footwear (shoe and boot covers) products meet? 3.8Do you offer skid-resistant Disposable Footwear (shoe and boot covers) products? 3.9Are your Disposable Footwear (shoe and boot covers) products non-conductive? 3.10Do you provide Disposable Footwear (shoe and boot covers) products in a variety of sizes? Please provide manufacturer product numbers for each Disposable Footwear size. 3.11Please list all of available colors and patterns of your Disposable footwear (shoe and boot covers) products. 3.12Can your Disposable Footwear (shoe and boot covers) products be disposed of in general waste through routine housekeeping channels? If not, please provide documentation on how your Disposable Footwear should be disposed of. 3.13What are the Units of Measure/Units of Issue available for procurement? Please list all forms of packaging available for your Disposable Footwear (shoe and boot covers) products (example: 100/bag, 10bg/case). 4.0Feedback Requested Please complete the attached RFI Vendor Response Worksheet and Addendum with indicated information about your Disposable footwear. 4.1Have your Disposable Footwear been subject to any recall(s)? If so, what corrective actions and/or product modifications have been made in response to the recall(s)? 4.2Do you have any emerging Disposable Footwear technology you would like to share? 4.3Please provide any additional information including relative topics outside of the specific information requested which you consider relevant. Any information provided in response to this RFI will be considered and treated as unrestricted information which can be used by the Government in any future planning or acquisitions. All information, comments, and suggestions may be incorporated into future acquisition documents, without identifying the source or obtaining prior approval from the source. 5.0Liability The Government shall not be responsible or liable for any costs incurred by any party in the preparation and submission of any response to this RFI. 6.0RFI Responses 6.1Vendor responses to this RFI must be received by this office no later than 1700 (CDT) on 25 September 2017. 6.2Supporting documentation is required to be provided by the offeror as attachments to this RFI. The RFI Vendor Response Worksheet must be completely filled out and submitted timely along with all supporting material such as but not limited to product catalogs, sale sheets, Training Literature, FDA approval documentation and information, etc. 6.3Responses and/or questions to this RFI must be submitted via e-mail to the points of contact listed below (section 7.0). E-mail responses should be no larger than 5 megabytes (MB) in size.
