- The Department of Veterans Affairs, Network Contracting Office 10, 24 Frank Lloyd Wright Drive, Lobby M, Suite 2200, Ann Arbor, MI 48105, intends to solicit offers for contractors who are authorized to perform Preventative Maintenance on government owned medical equipment: Becton Dickinson and Company LSRII S/N H0109, HeNe Laser, Violet Laser 50mW, Laser SAPPHIRE 488-20 Blue, Laser Compass 561-50 Lt Green, High Throughput Sampler S/N U30100170, located at the VA Ann Arbor Healthcare System, 2215 Fuller rd., Ann Arbor, MI 48105. Contractors that can provide this requirement and meets the requirements below are requested to send their 1. Company Name, 2. DUNS number, 3. Business size under NAICS 811219 and 4. If the item(s) are available on a FSS contract or not and the FSS contract number if it is available on contract, 5. Notification if they are a VETBIZ registered VOSB/SDVOSB or not and their company size. This is not a solicitation. This is a Request for Information, Sources Sought. DESCRIPTION/SPECIFICATIONS/WORK STATEMENT SCOPE OF WORK: The Contractor shall provide materials, labor, supervision, test equipment, technical manuals, consumable supplies, operator training and transportation to perform two (2) preventive maintenance inspections on the following Government owned equipment as referenced in item # two (2) below, located at the VA Medical Center, 2215 Fuller Road, Ann Arbor, MI 48105. At this time we are exercising a new contract with the choice of four option year renewals. Base year with four (4) one (1) year option periods. EQUIPMENT: Government owned: is located at the VA Medical Center, 2215 Fuller Road, Ann Arbor, MI 48105. *LSRII S/N H0109, *HeNe Laser, *Violet Laser 50mW, *Laser SAPPHIRE 488-20 Blue, *Laser Compass 561-50 Lt Green, * High Throughput Sampler S/N U30100170, located at VA Medical Center, 2215 Fuller Road, Ann Arbor MI, 48105. SERVICE CALLS: Hours of work: All scheduled work will be performed during normal working hours of 8:00 am until 4:30 pm. Monday through Friday, excluding holidays and any day declared an official Federal holiday. Holidays observed by the VA Medical Center are as follows: New Year s Day Labor Day Martin Luther King Day Columbus Day President s Day Veterans Day Memorial Day Thanksgiving Day Independence Day Christmas Day Emergency Service: Emergency service calls are ones required when equipment, is not in usable condition and immediate response by Contractor is required. Response time of the Contractor for emergency service calls will be four (4) working hours after official notification. The term response will result in the physical presence of a fully qualified technician arriving on station within the specified time. If Contractor requires, additional parts/time to complete required repair/service, Contractor shall contact the COR to coordinate the completion of work within an agreed upon completion time/date, to keep inoperable time at an absolute minimum. All repairs shall be in accordance with accredited standards and codes applicable to equipment covered in this contract. Equipment shall meet or exceed manufacturer s operating specifications upon completion of repairs. PREVENTATIVE MAINTENANCE: Contractor shall perform two (2) scheduled preventative maintenance (PM) procedures during the contract period, during the months of October and April. It is the sole responsibility of the Contractor to schedule and perform all maintenance inspections. The specific date(s) for scheduling the (PM) will be coordinated between the Contractor and the COR of the Research Office. Preventative maintenance inspections will be performed during the hours of 8:00 am until 4:30 pm Monday through Friday, excluding holidays and any day declared a Federal holiday. Holidays observed by the VA Medical Center are as previously mentioned in #4 above. Section (a) Preventative maintenance procedures shall include all items outlined in the original equipment manufacturers (PM) procedures. The Contractor shall submit a copy of the proposed procedure thirty days after award of the contract. Preventative maintenance will include, but not restricted to: Cleaning of the equipment Completing the installation and documentation of all minor operation and reliability engineering notice changes. Running system diagnostics, hardware and software. Calibrating and lubricating the equipment to manufacturer s specifications. Providing all factory required or recommended software updates. The hardware and software updates which ensure performance to current product specifications. This service will be performed during routine preventative maintenance, or at a time specified by the VA and agreeable to the Contractor. The Contractor will be required to install and maintain the engineering updates requested by the factory. The updates will be incorporated in a time frame as specified by the manufacturer or mutually agreed upon by the VA and the Contractor. All will be provided by the Contractor at no additional cost to the VA. Documented form showing the (PM) procedures followed is required from the Contractor. 6. SPECIAL CONDITIONS: The Contractor s service representative shall notify the designated COR of the Research Office, located in Building 22, Room G20C, VA Medical Center, 2215 Fuller Road, Ann Arbor, MI 48105 Area Code (734) 769-7100 ext. 55586 upon arrival at the facility and prior to beginning work and upon completion of work. Clean-Up: Contractor shall leave the work site in as clean a condition as when work began. Debris accumulated during work procedures shall be removed and the equipment or furniture moved during work procedures shall be repositioned to original locations. Removal of Equipment by Contractor: Any equipment to be removed from this facility for repair work shall not be removed without the proper approval from the Acquisition and Material Management Service representative. SERVICE TICKET: Contractor shall submit to the designated COR of the Research Office a routine work and/or emergency work slip. This work slip is to be legibly signed and dated by the VA technician who operated this equipment or works in the department where the work has been completed. Upon completion Contractor will submit the signed service ticket to the COR of the Research Office. Service ticket shall be legible and contain the following information: Date and time of service/inspection Brief description of work performed Equipment type, model and serial number Copy (PM) inspection report, if applicable Signature of designated COR accepting work during normal working hours On Saturdays, Sundays and acknowledged holidays observed by the VA Medical Center and after normal working hours, the Contractor will notify the originator of the call, when the service call is complete. Upon completion service ticket should be dated and signed by personnel on duty at time of work/repair. Service ticket must be forwarded to the COR of the Research Office. Saturday, Sunday, Holiday and after hours service will be provided at an additional charge. SAFETY REQUIREMENTS: In the performance of this contract the Contractor shall take safety precautions as the Contracting Officer or his/her designee may determine is reasonably necessary to protect the lives and health of occupants of the building. The Contracting officer or his/her designee will notify the Contractor of any non-compliance with the foregoing provision and the actions to correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at this site of work, shall be deemed sufficient for the aforesaid. If the Contractor fails and/or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work and hold the contract in default.
