Page 3 of 3 Solicitation Number: VA119-17-Q-0443 Notice Type: Combined Synopsis/Solicitation Synopsis: COMBINED SYNOPSIS/SOLICITATION This is a combined synopsis/solicitation for brand name commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number VA119-17-Q-0443 is issued for this combined synopsis/solicitation; this solicitation is being issued as a Request for Quote (RFQ). This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-94 and 2005-95, both dated 19 Jan 2017. Requirement Description: The Department of Veterans Affairs (VA), Veterans Health Administration (VHA) is responsible for providing high quality and timely commodities and services for our Veterans and their families. As the complexity of those items and services increases, program and technical acquisition and associated team involvement, also becomes more complicated. In addition, as our budgets become more constrained, enterprise acquisition solutions provide the capability to enhance and streamline our processes for efficiency, timeliness, and an overall cost savings in leverage quantity buys across the VA while maintaining or improving quality. The Department of Veterans Affairs (VA), VISN 11, The John D. Dingell Veterans Affairs Medical Center (VAMC) is contracting to provide all necessary labor, management, supervision, technical skills, quality control, materials, software, and equipment necessary to complete the upgrade and expansion site services of our current Spacelabs Healthcare LLC Patient Monitoring System to upgrade the current Government owned system. All requested installation site services, additional server installation, clinical configuration, software upgrades, and additional hardware installation shall operate and be compatible with/ under the existing Spacelabs Healthcare LLC upgraded patient monitoring system will allow clinicians and staff new functionality which will assists with accurately and precise monitoring patient information using bedside monitors, a central station, remote alarm management, telemetry and other associate accessories throughout various departments. Consistent with their mission, John D. Dingell VAMC, Detroit, MI has a need to procure equipment in order to upgrade the current Government owned system in place. The upgraded patient monitoring system will allow clinicians and staff new functionality which will assist with accurately and precisely monitoring patient information. The contractor shall provide a timely delivery and installation, an all-encompassing manufacturer maintenance warranty and training of VA personnel in the maintenance of equipment throughout the period of performance. The contractor shall provide products within the terms and conditions of the contract and resultant delivery orders. A Firm-Fixed Price contract will be awarded for the purchase of Brand Name equipment in accordance with all terms, conditions, provisions, specifications and schedule of this solicitation herein. Quotes shall contain the Quoter s best terms for price/cost of the brand name equipment due to proprietary justification. The Government reserves the right to award without discussions. All responsible sources may submit a quote, which shall be considered by the agency. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510 Electromedical and Electrotherapeutic Apparatus Manufacturing and the associated size standard is 1,250 employees. This acquisition is for a Firm-Fixed Price contract in accordance with FAR Part 13.501(a) for Spacelabs and related equipment and accessories as identified in Attachment A, Price/Cost Schedule. All interested companies shall provide a quote for all Contract Line Item Numbers (CLINs) listed in Attachment A, Price/Cost Schedule. The FAR provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition including attached addenda to the provision in Attachment E, Solicitation Provisions. The FAR provision at 52.212-2, Evaluation -- Commercial Items, and the specific evaluation criteria as attached addenda in Attachment E, Solicitation Provisions, also applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible Quoter whose quote conforms to the solicitation, Attachment F - Limited Source Justification (LSJ) and will be most advantageous to the Government. The award of the contract will be made to the lowest priced quote. The following factors shall be used to evaluate quotes: The Quoter is to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its quote. All Clauses are location in Attachment C Contract Clauses. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition.
