The purpose of this modification is to extend closing date to 09/18/2017 by 10:00 AM Please note question will be accepted until Friday 09/14/2017 10:00 AM It is being requested to award a firm-fixed price for a 5 year term, however, I am unable to located what the date terms are of this agreement. Can you please provide the effective date of the award and the end of date of the awarded contract? Oct 1 2017 to Sep 30 2022 Can a column be added to the table of Schedule of Supplies and Services to provide clarity of notes as to what is being bid? No Line Item 1 Ga-67 Gallium Citrate UD: This item is typically billed and priced per mCi. Will you accept this as priced per mCi? No Line Item 4 I-123 NaI 5mCi UD: Can you please provide further clarification as to what is being requested? Is this Liquid I-123, and 5mCi is being requested? Not Liquid; 5mCi capsule Line Item 5 I-131 USP NaI Tx Hyperthyroidism Cap UD 1-30mCi: This item is typically billed and priced for the 1st mCi and then each additional mCi thereafter.
