- Evaluation Criteria Statement of Work Request for Quote This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 17-013353 and the solicitation is issued as a Request For Quotation. This acquisition is for commercial item or service and is conducted under the authority of the Federal Acquisition Regulation (FAR) Part 13-Simplified Acquisition procedures; FAR Subpart 13.5 -Simplified Procedures for Certain Commercial Items; and FAR Part 12-Acquisition of Commercial Items, streamlined procedures for evaluation and solicitation for commercial items - as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a separate written solicitation document will not be issued. This solicitation is a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94 (JAN 2017). This requirement is set-aside for small businesses. The associated North American Industrial Classification System (NAICS) code for this procurement is 325412 with a small business size standard of 1250 Employees. The Contractor shall provide consulting and technical support services to systematize care transitions and communication handoffs, so we can mitigate the risks associated with communication lapses. This procurement is aimed at improving communication among the healthcare team members - with the ultimate outcome of reducing risk, medical errors and patient harm.
