- The Department Of Veterans Affairs, Network 15 CMOP Contracting Office has requirement to procure Patient Centric Label 2x5.5 - BPA - Base + 4 Opt Yr - for CMOP Chelmsford 761 & CMOP Tucson 762 Product Code is 7510, Office Supplies NAICS code 325992 Heat sensitized (i.e., thermal) paper made from purchased paper SDVOSB Set-Aside Nonmanufacturing Rule Waiver applies All SDVOSBs submitting Quotes, will provide Sample Label Rolls, per label specs, for Eval and Testing Sample Label Rolls two rolls permanent adhesive two rolls temporary adhesive. Sample Labels provided at Vendor expense. Sample Label Roll Ship To: Network 15 Contracting Office (NCO) CMOP Division 3450 S. 4th St Trafficway Leavenworth, KS 66048 Attn: Julian Lake Vendor Name should be listed on the Sample Label box with Manufacturer. Vendor should notify via email Julian.Lake@va.gov when Samples have shipped. Sample Label Rolls can be shipped for Eval & Testing before closing date.
