- This is an AMENDMENT to a PREVIOUS COMBINED SOLICITATION for: Gas Fuel Pump, Diesel Fuel Pump, Card Reader and U Bollards (See RFQ for item list) Amendment Items: Amended to 100% small business set aside. NAICS Code changed to 333913 Amended response date to 9/15/2017 Responses to this Combined Synopsis/Solicitation must be in writing and submitted on the attached Request for Quotation. This acquisition implements a 100% set aside for Small Businesses. The NAICS Code is 333913; size standard is 750 employees. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) and must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor and provide the commodities described within the Request For Quotation. Winning contractor must be registered in www.SAM.gov for NAICS Code 333913 at time of award. Gas Fuel Pump, Diesel Fuel Pump, Card Reader and U Bollards See Requirements/Specifications below. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this Combined Synopsis/Solicitation. This notice shall not be construed as a commitment by the Government to award a contract, nor does it restrict the Government to a particular acquisition approach. Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Small Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. Requests for information and resultant responses shall be submitted directly to the contracting officer, Paul Speedling at Paul.Speedling@va.gov. NO TELEPHONE CALLS WILL BE ACCEPTED. All submissions should be received no later than 5:00 pm Eastern Standard Time on September 15, 2017. Requirements/Specifications Scope of Work Gas Fuel Pump, Diesel Fuel Pump, Card Reader and U Bollards LOCATION: Northport VA Medical Center 79 Middleville Road Northport, NY 11768 PROJECT SCOPE: Provide all labor, equipment and materials necessary to install Gas Fuel Pump, Diesel Fuel Pump, Card Reader and U Bollards at Northport VA Medical Center complete with all design, hookups, hoses, nozzles, pump, freight, electrical, explosion proof conduit and any and all ancillary equipment. Old pumps are to be removed and disposed. Pumps are to be fully operational when completed. REQUIREMENTS: 10:1 volume pulser & junction box. Internal filter. Explosion proof AC junction box & light. Two-stage II solenoid vale. Hose mast vapor recovery clamp kit Supply and install gas and diesel pumps. Supply and install Islander Plus card reader system with 100 keys. Decommission stage II vapor recovery line under dispenser and perform pressure decay test for the NYS DEC. Emergency spill containment and removal of any spill. Supply and install 2 U Bollards and secure to the existing concrete island pad. OTHER REQUIREMENTS: Minimum of four references related to gas pumps installation. (Statement of Qualifications) Supply all manuals, parts, book and service requirement upon completion of the pumps installation. Contractor must comply with all applicable regulations, including but not limited to those established by the NY DEC, National Fire Protection Association (NFPA), State of New York Division of Oil and Safety Petroleum Regulations. Occupational Safety and Health Administration (OSHA), Underwriters laboratories 2085 and 142, and American Petroleum Institute 1104 or American Society of Mechanical Engineers B31.3. If a difference between regulations exists the most stringent shall apply. Pumps must fit within the current foot print. System must be fully operational within one week of delivery. TESTING: Contractor is responsible for all testing, calibration, and regulatory inspection required, ensuring proper system operation, tightness and functionality. Prior to pumps start-up, the contractor shall have a tightness test performed on the pumps and associated piping installed. The test results must meet the minimum specifications established by the manufacturer. Copies of the test results shall be submitted to the Northport VA Medical Center GEM Officer. Testing shall be documented by the contractor and witnessed by the GEMS Officer. Testing shall be done in accordance with the manufacturer s specifications and regulatory guidance. The Contractor shall demonstrate the operation of all system to the owner at the time of final start-up test. The contractor shall document all testing and provide copies to the GEMS Officer.
