- Statement of Work The Navajo Area Indian Health Services, Gallup Service Unit, Purchasing Department has a requirement for a Urinalysis Analyzer System for the Tohatchi Health Center, Tohatchi, New Mexico. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is 021-T51-17-0036. The solicitation is issued as a request for quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. This solicitation document and its incorporated provisions are those in effect through the Federal Acquisition Circular (FAC) 2005-94, effective January 19, 2017. The North American Industry Classification System (NAICS) Code for this acquisition is 334516 and the small business size standard is 1,000. The system and service requirements are cited in the attached Statement of Work. Fee Schedule: Contractor shall prepare a quote for the following: Total Cost shall be inclusive of labor, travel, per diem, equipment, supplies, materials, lodging, personnel, federal/state/local taxes and all other necessary expenses. 1) Cost per Test based on annual testing requirements specified in Exhibit 1 of SOW. 2) Reagents and supplies 3) Industrial Funding Fee 4) Installation of Equipment 5) Service Agreement 6) Training Period of Performance: Awarded contract shall be for one base year with four (4) one-year option years for a total period of performance of sixty (60) months. If the Government exercises its option pursuant to 52.217-9 Option to Extend the term of the Contract, the period of performance will be extended in accordance with the following schedule. YEAR OPTION PERIODS FIRST YEAR OPTION: October 01, 2010 through September 30, 2011 SECOND YEAR OPTION: October 01, 2011 through September 30, 2012 THIRD YEAR OPTION: October 01, 2012 through September 30, 2013 FOURTH YEAR OPTION: October 01, 2013 through September 30, 2014 Place of Performance: Navajo Area Indian Health Service, Tohatchi Health Center, 07 Choosgai Drive, Tohatchi, New Mexico. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at FAR clause 52.212-2, Evaluation - Commercial Items, applies to this acquisition. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: The government intends to award one firm fixed-price award to the offeror whose offeror will be most advantageous to the Government, price and other factors considered. The Government shall evaluate offers based on the following factors: 1. Schedule: Contractor shall provide a project timeline (equipment delivery, lead-time through installation and completion of training). 2. Technical Capability: Contractor shall provide evidence of equipment specifications and technical personnel available to meet the requirements. 3. Logistics: Contractor shall provide evidence of past performance and experience, comparable scope and complexity with the Indian Health Service or in remote locations within the past three (3) years. Provide 3 references. The quotes will be rated as acceptable/unacceptable in adherence to the SOW. All evaluation factors other than cost or price, when combined, are approximately equal to cost or price. b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Offerors must submit representations and certifications in accordance with the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Item. Responsible sources must be registered and active in the System for Award Management (SAM) database, website accessed through http://www.acquisition.gov. The FAR clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in SAM at https://www.sam.gov/ prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation [Note: Lack of registration in the System for Award Management will make an offeror ineligible for award.] All responsible sources may submit an offer that will be considered by the Agency.
