- Provisions and clauses applicable to solicitation W912JB17T2067 Performance Work Statement for Medical Surveillance This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W912JB17T2067 and is being issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses in effect through Federal Acquisition Circular 2005-95. CLIN Description Quantity U/M 0001 Medical Surveillance 1 Job 0002 Contractor Manpower Reporting 1 Job The Michigan Army National Guard has a requirement for medical surveillance from a medical facility within 50 miles of Lansing, MI. Please see the attached Performance Work Statement for exact requirements. This requirement is firm fixed price. Offers are due at 3:00 pm, ET on 22 Sep 2017. Email quotes to amanda.e.geissler.civ@mail.mil. Questions are due by 3:00 pm ET, 20 Sep 2017, and must be submitted via email. Questions will not be answered via telephone. Questions that occur after the cut off, but are considered to be significant to the requirement, will be accepted and discussed only with those vendors that have submitted a quote. Quotes must include price, technical approach and past performance information that is not more than three years old. Offers will be evaluated based on the Lowest Price, Technically Acceptable offer. Technical and past performance will be rated as Acceptable or Unacceptable. The following provisions and clauses are applicable to this solicitation: The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. See attached addenda for quote instructions. Offers should include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer.
