- Required Clauses Performance Work Statement and Requirements This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement N4523A17R0049, constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Puget Sound Naval Shipyard & Intermediate Maintenance Facility (PSNS & IMF), Industry Management Department, has a requirement to provide leasing & laundry services for various locations. Laundry is picked up and dropped off on base at designated locartions, on specific days. Billing will be done either bi-weekly or monthly. Local safety and hazardous material guidelines must be followed. This is full and open competiton RFQ, under NAICS 812332 with a business standard of $38.5M. CLIN0001 IMF LAUNDRY SERVICES SUBCLIN 0001AA SHOP 71A 1 JOB SUBCLIN 0001AB SHOP 51A 1 JOB SUBCLIN 0001AC SHOP 06A 1 JOB SEE SECTION 2 OF THE ATTACHED PERFORMANCE WORK STATEMENT FOR INVENTORY LIST AND MORE INFORMATION. The period of performance is expected to be from 1 October 17 to 30 September 18. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. Provision 52.212-1 applies to this acquisition. *Offerors shall include a completed copy of 52.212-3, Offeror Representations and Certifications-Commercial Items, with your offer. Clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acqusition with no commercial changes. Clause 52.212-5 is attached to this solicitation notice. All vendors submitting a quote shall be registered in the System for Award Management, SAM.gov. Additionally, each offeror must provide the following information with the quote: 1. The Request for Quote (RFQ) number; 2. The Name, Address, Telephone Number, and Cage Code or Duns number of the Offeror; 3. Include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the RFQ. Quotes that reject the terms and conditions of the RFQ may be excluded from consideration; 4. Acknowledgment of Solicitation Amendments (SF-30), if any, and; 5. Provide a technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary. 6. Address technical capability in meeting the requirement per the Statement of Work.
