- Mindfulness Based Stress Reduction Instructor The Great Lakes Acquisition Center (GLAC) NCO 12, on behalf of the Jesse Brown VA Medical Center, 820 South Damen Avenue, Chicago, IL 60612-3728, is seeking a Mindfulness Based Stress Reduction (MBSR) Instructor. The Instructor will provide Mindfulness Based Stress Reduction services to Veterans for approximately eight (8) hours per week for a one-year contract at the Jesse Brown VA Medical Center. Services shall be provided by an individual who has been trained as a Certified MBSR Instructor and/or has a minimum of 5 years experience in teaching MBSR, Mindfulness-Based Cognitive Therapy (MBCT) or Mindfulness-Based Relapse Prevention (MBRP). Interested Small Businesses are required to submit a capability statement that includes: resume/CV (to include a reference that has witnessed these services) and proof of professional/liability insurance. The relative North American Industry Classification System (NAICS) Code is 611699 (All Other Miscellaneous Schools and Instruction) with a size standard of $11.0 million. VOSBs and SDVOSBs are required to be registered and verified in VIP for consideration. If a firm believes it can provide these services, please provide written quotes specifying the type of certified instruction to the Contract Specialist and Contract Officer listed below. Any questions concerning the solicitation must be submitted by Friday, September 22, 2017 at 10:00 am CST.
