Attachment 4 - FAR 52.212-2 Addendum I Cranes and Hoists Evaluation Attachment 3 - FAR 52.212-1 Addendum I Cranes and Hoists Instruction to Offerors Attachment 2 - Wage Determination No 2015-5483 Rev No 3 dated 3 Aug 2017 Attachment 1 - Cranes and Hoists Performance Work Statement Request for Proposal FA8201-17-R-0012 The contractor shall provide all management, tools, supplies, equipment, parts, and labor necessary to inspect, load test, maintain, and repair the cranes and hoists, including both powered and manually operated units, at Hill AFB and Little Mountain (LM) Utah in a manner that will ensure continuous and safe operation. This is a combined synopsis and solictation. This solicitation is a Request for Proposal (RFP). Please see RFP FA8201-17-R-0012 for incorporated provisions and clauses, along with the RFP attachments as follows: Attachment 1 - Performance Work Statement (PWS), Attachment 2 - Wage Determination No 2015-5483 REv 3 Dated 3 Aug 2017, Attachment 3 - FAR 52.212-1 Addendum I Cranes and Hoists, and Attachment 4 - FAR 52.212-2 Addendum I Cranes and Hoists. The North American Industry Classification System (NACIS) for is acquisition is 811310 "Commercial and industrial machinery and equipment (Except automotive and electronic) repair and maintenance". This acquisition is a 100% Small Business Set-Aside. The Small Business Size Standard is $7.5 Million. The FSC H239. A list of the line items are identified in RFP FA8201-17-R-0012. A description of services are identified in RFP FA8201-17-R-0012 and the PWS. The dates and place of performance and acceptance are identified in RFP FA8201-17-R-0012.
