Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request for Quote: Provide maintenance services for (69) copier machines statewide. Place of performance will be Nevada Statewide but headquartered at the OTAG Complex, Nevada National Guard, Carson City, Nevada. The solicitation number is W9124X-17-T-0017 and is issued at a request for quotation (RFQ). The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular (FAC) 2005-72 effective as of 30 Jan 2014. Award will be made without negotiation. The applicable NAICS Code for this acquisition is 811212, Computer and Office Machine Repair and Maintenance and the small business size standard is $27.5 million dollars. The government plans to award a firm-fixed price contract for Copier Maintenance Services in support of sixty-nine (69) copier machines Nevada Statewide. The anticipated period of performance is date of award through 12 Months (Base Year) and with two (2) one-year options periods. The Statement of Work (SOW) is attached to this solicitation. Please note for budgetary & pricing purposes, the estimated monthly usage is 3,000 copies per month per machine (includes both black & white and color). Factors that shall be used to evaluate offers are (i) technical capability of the item offered to meet the Government requirement as listed in PWS and (ii) price. The Government intends to evaluate offers and make award without discussions. However, the Government reserves the right to hold discussions if it is in the best interest of the Government to do so. Technical capability will be rated as 'go' or 'no go' by technical expert, and will be based on the quote supplied by the vendor on their respective approach in meeting the PWS requirements -- all services and/or work provided with be in accordance with the terms, conditions, and standards set forth in this PWS. Vendors offering equivalent items shall be responsible for providing documentation of equivalency. Only those quotes with a 'go' rating will then be evaluated on price. (End of clause) 252.209-7995 REPRESENTATION BY CORPORATIONS REGARDING AN UNPAID DELINQUENT TAXLIABILITY OR A FELONY CONVICTION UNDER ANYFEDERAL LAW-FISCAL YEAR 2013 APPROPRIATIONS (DEVIATION 2013-O0010) (APR 2013) (a) In accordance with sections 8112 and 8113 of Division C and sections 514 and 515 of Division E of the Consolidated and Further Continuing Appropriations Act, 2013,(Pub. L. 113-6), none of the funds made available by that Act for DoD (including Military Construction funds) may be used to enter into a contract with any corporation that- (1) Has any unpaid Federal tax liability that has been assessed, for which all judicial and administrative remedies have been exhausted or have lapsed, and that is not being paid in a timely manner pursuant to an agreement with the authority responsible for collecting the tax liability, where the awarding agency is aware of the unpaid tax liability, unless the agency has considered suspension or debarment of the corporation and made a determination that this further action is not necessary to protect the interests of the Government; or (2) Was convicted of a felony criminal violation under any Federal law within the preceding 24 months, where the awarding agency is aware of the conviction, unless the agency has considered suspension or debarment of the corporation and made a determination that this action is not necessary to protect the interests of the Government. (b) The Offeror represents that- (1) It is is not a corporation that has any unpaid Federal tax liability that has been assessed, for which all judicial and administrative remedies have been exhausted or have lapsed, and that is not being paid in a timely manner pursuant to an agreement with the authority responsible for collecting the tax liability, (2) It is is not a corporation that was convicted of a felony criminal violation under a Federal law within the preceding 24 months. (End of provision) Offeror representations and certifications provided through the System for Award Management (SAM) are required and shall be verified prior to award.
