- Wage Determination No. 2015-4477 REV5 This is a combined synopsis/solicitation for commercial items prepared in accordance with format Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation HSFLGL-17-Q-00324 is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 05-95. This procurement is a 100% Small Business set-aside and the associated NAICS code 561492 with Size Standard $15M. The Federal Law Enforcement Training Centers (FLETC) has a requirement for Court Reporter Services: LINE ITEM 001- The Office of Chief Counsel is requesting QUANTIY: One (1) Court Reporter for EEOC Hearing PERIOD OF PERFORMANCE: Three (3) days, OCTOBER 24, 2017 - OCTOBER 26, 2017 TIME: 9:00 a.m. to 5:00 p.m. PLACE: To be held at Department of Homeland Security (DHS), Federal Law Enforcement Training Centers (FLETC), 1131 Chapel Crossing Rd, Glynco, GA 31524. INCLUDES: An original transcript, and one copy (not a condensed copy) that is compatible with RealLegal E-Transcript Viewer. Submission of Information: 1) Name of Company, Address and DUNS Number. 2) Point of Contact, Email and Phone Number. 3) Pricing 4)Technical description of service being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include resume, company literature, or other documents, if necessary. 5) Past Performance information to include minimum two (2) recent or relevant contracts for the same service. Provide contract/order number, points of contact with telephone number and email address. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR Part 13 Evaluation Procedures to be used on the basis of award using lowest price technically acceptable approach. Evaluation criteria factors: technical, price, and past performance. FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.219-6, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.222-50, 52.232-33. HSAR 48CFR 3052.209-70 PROBITION ON CONTRACTS WITH CORPORATE EXPATRIATES (JUNE 2006), 3052.205-70 Advertisements, Publicizing Awards, and Releases. (SEP 2012), 3052.209-70 Contract Terms and Conditions Applicable to DHS Acquisition of Commercial Items (SEP 2012), Special Contract Requirements 1-29. Offerors shall provide a statement of compliance with FAR 52.222-50, Combating Trafficking in Persons, requirements and confirm subcontractors will accomplish a statement of compliance upon award. Offeror must be registered in The System for Award Management (SAM) database before an award can be made to them. If the offeror is not registered in the SAM, it may do so through The System for Award Management (SAM) website at http://www.sam.gov. Contractor must be able to pass Criminal History Record check to gain access to FLETC secured facility. Quotes shall be submitted electronically via email Gretchen.Lovell@fletc.dhs.gov and are due no later than September 25, 2017 4:30 PM EST.
