- W91YTZ-17-T-0169, see attached Oncotype DX Breast/Colon cancer testing The Regional Health Contracting Office - Atlantic, Fort Bragg Health Care Cell, Fort Bragg, NC intends to solicit, negotiate, and award a sole source Blanket Purchase Agreement (BPA) for Womack Army Medical Center, Fort Bragg, NC. This agreement is needed in order to provide Oncotype DX (Colon/Breast) Cancer Testing in support of the Department of Pathology. The ordering period will be 1 October 2017 through 30 September 2019. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number W91YTZ-17-T-0169 is issued as a request for quotation (RFQ). The Standard Industrial Classification Code is 8071 and the NAICS Code is 621511. The Small Business Size Standard is $32.5M. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. Interested vendors may identify their interest and capability to respond to the requirement and submit proposals. Provide your Company's Dunn and Bradstreet number (DUNS); CAGE code; tax ID number; number of days to deliver, along with your Published Price list, by item number, for evaluation purposes. The resulting award will be made on a Standard Form 1449 (SF 1449), for commercial items. All proposals received after the date of publication of this synopsis will be considered by the Government. Period of Performance is 1 October 2017 - 30 September 2019. Clauses and provisions throughout this solicitation can be viewed by accessing website www.farsite.hill.af.mil or https://acquisition.gov/far. FAR provision 52.212-1 [Instructions to Offerors Commercial Items] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the System for Award Management (SAM). SAM can be accessed via the Internet at www.sam.gov. Provide with your submission to this request, your company's Dunn and Bradstreet number (DUNS); CAGE Code; Price Proposal (to include published price list) for evaluation purposes. Confirmation of SAM registration will be validated prior to award of the contract. Offerors shall include a completed copy of FAR provision 52.212-3 ALT 1, "Offeror Representations and Certifications" with their offer. Offerors shall include a confirmation of receipt of any amendments to this solicitation. Failure to submit a confirmation could result in the offer being determined non-responsive.
