This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is: VA259-17-Q-1060, and it is issued as a request for quotation (RFQ) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular, 2005-95, effective January 13, 2017. Surgeons will utilize the system for coronary artery bypass grafting, traumas, transplants, orthopedics and other procedures. Date and place of delivery (FOB Destination): 10/30/2017 Oklahoma City VA Health Care System 921 N.E. 13TH St. Oklahoma City, OK 73104-5007 The provision at 52.212-1, Instructions to Offerors Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Lowest Price Technically Acceptable (LPTA) and contractor must be able to provide proof of being an authorized distributor for all quoted items, and any award made as a result of this request for quote, will be made on an All or Nothing Basis. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable, if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror, within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items, with the offer. The clause at 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. Date and Time offers are due: September 26, 2017- 5:00 PM (EST) Name and telephone number of the individual to contact for information regarding the solicitation: Sean Rice, Contract Specialist, sean.rice2@va.gov JUSTIFICATION FOR BRAND NAME AWARDS IAW FAR 13.106-1 (OVER MICRO-PURCHASE THRESHOLD($3K) BUT NOT EXCEEDING THE SAT ($150K)) IAW FAR13.104, COs must promote competition to the maximum extent practicable to obtain supplies and services from the source whose offer is the most advantageous to the Government, considering the administrative cost of the purchase. When competition is not practicable, IAW FAR13.106-1(b), COs solicit from a single source or a limited number of responsible sources for purchases not exceeding the simplified acquisition threshold or. COs may solicit from one source, or limited type of supplies or services, if the CO determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). IAW FAR13.106-3(b)(3),COs are required to include additional statements explaining the absence of competition (see 13.106-1 for brand name purchases) if only one source is solicited and the acquisition does not exceed the simplified acquisition threshold (does not apply to an acquisition of utility services available from only one source) or supporting the award decision if other than price-related factors were considered in selecting the supplier. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE: The Oklahoma City Veterans Health Care System has a requirement for the procurement of two (2) Haemonetics CSE-E-US- Cell Saver Elite systems for the Surgery department. Surgeons will utilize the system for coronary artery bypass grafting, traumas, transplants, orthopedics and other procedures. 3. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE SOURCE, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED: The Cell Saver Elite results in high hematocrit with excellent red blood cell recovery providing more red cells for transfusion, optimal washout of undesirable components, and exceptional fat removal performance which provides maximizes patient safety with auto-transfusion and sequestration. Our physicians and staff have used the Haemonetics Cell Saver Elite and we currently own one. The Haemonetics Cell Saver Elite will require minimal downtime for training of physicians, OR staff and Sterile Processing staff. It will also decrease the possibility of mistakes being made due to confusion between different machines. 4. SUGGESTED MANUFACTURER INFORMATION (Contractor Name, Address, City, State, Zip, and Telephone Number) Haemonetics - Multiple small business distributors 5. REASON THAT SUGGESTED MANUFACTURER IS THE ONLY BRAND, WHICH WILL MEET THE MINIMUM NEEDS OF THE GOVERNMENT: Haemonetics is the only manufacturer of the Cell Saver Elite with the characteristics as noted in 3. Our physicians and staff have used the Haemonetics Cell Saver Elite and we currently own one. The Haemonetics Cell Saver Elite will require minimal downtime for training of physicians, OR staff and Sterile Processing staff. It will also decrease the possibility of mistakes being made due to confusion between different machines. 6. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED: This is a Brand Name only procurement for standardization; it is in the best interests of patient safety and handling to maintain standardization of the equipment. Market research has shown there are SDVOSB vendors able to provide items regarding this requirement annotated in line 4. 7.
