Description
This is a sources sought notice only. The purpose of this sources sought notice is to gain market knowledge of potential businesses to provide (see description of services and qualification requirements below.) The applicable NAICS Code is 621511, with a small business size standard of $32.5 M. If a solicitation is issued, it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from this response. Responses to this sources sought notice is not a request to be added to a prospective source list or to receive a copy of the solicitation. Information received as a result of this notice will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. This notice does not represent a commitment by the Government to pay for costs incurred in the preparation and submission of information or any other costs incurred as a response to this announcement. All businesses currently providing this service are encouraged to reply, as responses will be used by the Government to make determinations about for the appropriate set-aside. No solicitation is currently available at this time. After review of the responses to this sources sought announcement, a synopsis and/or solicitation announcement may be published at a later time. The duration of the services is anticipated to be required for one (1) year. Responses must include: (1) Business Name, GSA contract number (if applicable), or other contract number (2) DUNS (3) Point of Contact Name, Phone Number, and E-mail Address (4) Business Size SMALL or LARGE (5) Type of Business SERVICE DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUBZone, WOMEN-OWNED etc. (6) Description of your firm s capability in providing these types of services (Include relevant past performance/ contract number and point of contact for services provided) VIP Registration - Veterans First Contracting Program and the VA Rule of Two (Kingdomware v. United States) 38 U.S.C. 8127 - 8128: Service-disabled veteran owned small business (SDVOSB) or Veteran owned small business (VOSB) concern must be registered and verified in VA's Vendor Information Pages (VIP) database at www.vip.vetbiz.gov to be eligible for award as a SDVOSB or VOSB if/when a solicitation is issued for this requirement as a SDVOSB or VOSB set aside. Possible Requirements Provide laboratory services to include pickup and transport of specimens to its laboratory; preanalytic processing as defined in its laboratory user s manual; analysis, reporting of analytic results, and consultation regarding selection, collection, transportation and result interpretation as requested by the Anatomic Pathology Service, San Francisco VA Medical Center, hereinafter referred to as the SFVAMC. The contractor shall provide the following services: All work shall be performed at the Contractor s facility. The services shall include the following: Tissue procession and sectioning Hematoxylin and eosin (H&E) staining Histochemical stains and Immunohistochemistry A statement of the total number of cassettes received, blocks made and total number of slides sent Check the daily sheet for accuracy in case of discrepancy Specimen carriers Notify the SFVAMC Pathology Service of any missing or mislabeled cassettes All supplies for special tests (e.g., but not limited to, viral studies) Description of Non-H&E Histochemical (Special) Stain Recut with 1 H&E Stain Tissue and Cell Blocks (Embedding & 1 H&E Slide Each) Decalcification Immunoperoxidase Unstained Routine. The Contractor shall provide routine scheduled specimen pickup at a time mutually agreed upon by the SFVAMC COTR but not less than once each day. The SFVAMC COTR or designee shall notify the Contractor during weekends and federal holidays, via telephone, when a pick-up courier is required. Transport samples in such a manner as to ensure the integrity of the specimen. If the results are unsatisfactory, the block will be reprocessed and additional stained slides will be reproduced at no additional charge to the Government. Each incident shall be reported monthly at the pathology QA meeting. Contractor shall supply any special preservatives required for specimen preservation. Analyze samples and provide accurate test results on the correct patients. Routine test results shall be reported within 2 to 21 days of specimen testing by fax to the department of Anatomic Pathology Service, SFVAMC depending on the TAT specified for that particular test. Consult with Laboratory on test results by telephone as needed. Provide VAMC Laboratories with a means of communication to permit immediate inquiry regarding the status of pending tests. Laboratory/Staff License: The Contractor shall make sure the laboratory has all licenses, permits; accreditation and certificates required by law and specifically maintain current certifications/accreditation through: CAP (College of American Pathologists) CLIA (Clinical Laboratory Improvement Act) All medical facilities providing laboratory services under the contract must possess a valid state license and full accreditation from the JCAHO and the College of American Pathologists (CAP). The Contractor shall perform pathology procedures compatible with the participating VA operating capacity and equipment. The Contractor shall not introduce new medical procedures and services for specimens provided by VAs without prior recommendation to, and approval of, the COR, or authorized laboratory representative. The Contractor shall ensure that all procedures performed are of current acceptable standard deviation with at least (minimum) daily controls conducted to determine procedure accuracy. The standard deviation and daily control records are to be available for review upon request by the Contracting Officer or contracting officer s technical representative. The Contractor shall perform repeat tests at no charge to the government if the results do not, in the government requesting physician's opinion, fit the clinical picture of the patient. When requesting these tests, the government requesting physician or representative will explain the conditions of the patient or any discrepancies observed. Repeat tests shall be the same test originally ordered. The Contractor shall notify the VA COR or designee of instances in which, in the professional judgment of the Contractor, the patient requires services or tests, which were not originally requested. Tests referred to another laboratory shall be at no additional transfer charge or confirmation charge to the government. Contractor shall fill out and maintain for specimens the VA provided test request form necessary to meet clinical and administrative requirements of VA at no additional expense to the government. The Contractor shall contact the VA COR to coordinate the furnishing and delivery of specimen collection and transportation supplies, terminal or data fax and supplies, and the installation of equipment. The Contractor shall provide to the VA the supplies and equipment within two (2) work days after receipt of the delivery order. The VA COR will request replenishment of supplies from the Contractor on an as needed basis. SPECIMEN PICK-UP TIMES Routine. The Contractor shall provide routine scheduled specimen pickup at a time mutually agreed upon by the VA COR but not less than once each day. The VA COR or designee shall notify the Contractor during weekends and federal holidays, via telephone, when a pick-up courier is required. TEST RESULTS Routine. The Contractor shall provide routine test results to the ordering SFVAMC, Anatomic Pathology Service (113B) facility within twenty-four (24) hours following test completion, except when specimens are picked up the day before a weekend or holiday (in which case the results will be provided the following business day). Test procedures requiring a turnaround time longer than 24 hours (excluding the above exceptions) shall be identified by the Contractor before contract award and approved by the Contracting Officer. Reference values will be provided for all tests conducted, as appropriate. Telephone Consultation. The Contractor shall consult with the VA COR, requesting Clinician or designee at the VAMC by telephone as needed. The Contractor shall immediately telephone the respective VA COR; requesting Clinician or designee to report Critical Values or test result that may indicate a life-threatening condition. Reporting of Results: Quality Assessment of histology slide staining and correct labeling of the slides 98% of the time will be the level of acceptance. SLIDES The Contractor shall return representative slides, paraffin blocks, anatomic, hematologic, etc., for quality control review when requested. Quality control slides shall represent duplicate copies original slides prepared by the Contractor from permanent tissue blocks. The Contractor shall provide quality control slides to the participating VAMC or clinics at the same time the contractor submits the final reports. The Contractor shall prepare additional blocks and slides, in quantities specified by the participating VAMC for review by pathologists. For malignant cases, the Contractor shall call results to the submitting VAMC, and prepare paraffin blocks and slides review by the Armed Forces Institute of Pathology (AFIP) or other Government Pathology Organization (GPO). For benign cases, the Contractor shall prepare slides for review by participating VA. STORAGE OF SPECIMENS The Contractor shall store all specimens as required by regulatory agencies (RA). Upon request by the VAMC COR or designee, the Contractor shall provide a copy of the regulatory requirements for each participating RA. In the absence of RA requirements, the Contractor shall store specimens, after all testing has been completed: clinical specimens seven (7) days, surgical specimens six (6) months, cytology and histopathology slides indefinitely; and paraffin blocks for five (5) years. TEST SAMPLES Upon provision of sample slides by the government, the Contractor shall provide interpretation of sample slides. The sample may include, at a minimum, breast biopsy, thyroid biopsy, mastectomy with nodes, colon resection, lymph node biopsy, and gynecology and non-gynecology cytology. DOCUMENTATION The Contractor shall ensure all required documentation shall include, at a minimum, timeliness, legibility, accuracy, content, and signature. Contract personnel shall indicate responsibility for the content and correctness of all prepared and transcribed reports by affixing the contract personnel s own signature to the documents and validating their contents. MONTHLY TESTS SUMMARY The Contractor shall provide to each VA a cumulative (year-to-date) monthly report of all tests received during the month and for all outstanding results. The report shall identify the tests that were received to include accession number, patient s name, Social Security Number, and specimen collection and test report dates required by accreditation agencies. The Contractor report shall indicate the requesting physician, tests, TAT for each test, corresponding cost per test. CONTRACTOR PERSONNEL The Contractor shall make sure employees have current and valid professional certifications before starting work under this contract. Technologist, medical technicians, and cytotechnologist shall meet personnel qualifications required by Clinical Laboratory Improvement Act (CLIA) '88 Guidelines. Pathologists must have graduated from an approved school of medicine or osteopathy and completed a residency or fellowship in pathology acceptable to the United States Surgeons General must possess a valid license to practice clinical reference laboratory services from a United States (U.S.) jurisdiction; and shall have provided pathology services a minimum of 24 months within the past 36 months and pathology consultation services a minimum of 36 months within the past 48 months.
