- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; offers are being requested, and a written solicitation document will not be issued. The Government intends to issue a purchase order in accordance with FAR Part 13 for using Simplified Acquisition Procedures. The solicitation number is VA247-17-Q-1175. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-95 effective January 19th, 2017. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 and the Small Business Size standard is 1000 employees. This combined/synopsis solicitation is set aside for Veteran Owned Small Businesses (VOSB s) in accordance with VAAR 852.219-11. Only VA verified VOSB s in the VIP at the time of contract award will be considered. All non-VIP verified firms will be considered non-responsive and ineligible for award. The acquisition will be made pursuant to the authority in FAR 13 to use simplified procedures for commercial items. SPECIFICATIONS Introduction to Required Specifications The National Program Office of Sterile Processing (NPOSP) and the ISO Consultation Division (ICD) has teamed up to provide a specific purpose funding program to assist the Sterile Processing Service in acquiring equipment, instruments and equipment used to reprocess these items through standardization. The equipment being requested is a Small Steam Sterilizers, 16x16x26. This piece of equipment will be replacing the sterilizers located in the Operating Room Core which has met its life expectancy. The capabilities of this sterilizer will be to run pre-vacuum, gravity, leak test and DART test cycles. The sterilizer should have the ability to sync with the CensiTrac instrument tracking system. The sterilizer should be free standing, cabinet enclosed unit. The vacuum system should consist of a water ejector. This piece of equipment will be used as an immediate use steam sterilization method for any surgical instrument dropped during surgery. The purpose of this sterilizer would be to guarantee terminal sterilization of these items without the potential of BBP exposure. This also meets the VHA stance on standardization of equipment throughout the medical center. This sterilizer is a new model of the existing and will fit into the existing space with no structural modification to the facility. Note: This is for a steam sterilization. One machine. 16x16x26 Prevacuum Single Sliding Cabinet Steam 2 EA Deinstallation - Small Sterilizers 2 EA Install Small Steam Sterilizer 2 EA Install Cabinet Pkg on the unit 2 EA Service Agreement - 1ST Year PM Small Steam 1st Year Maintenance Service is a preventative maintenance inspection program that is designed to keep new equipment operating according to manufacturers requirements throughout the warranty period. 2 EA Service Agreement - 2 Year Comprehensive Small Sterilizer Steam coverage; parts, labor and travel for all scheduled Preventive Maintenance inspections and unlimited repairs are included. Excluded from coverage are accessories and consumable/expendable items 2 EA Trade In Products - 16" Century Sterilizer 2 EA Delivery Date: ARO 30 DAYS Contract Type: Firm Fixed Price (FFP) (a) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (b) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. Offerors shall provide the information required by FAR 52.212-1 (APR 2014), Instructions to Offerors-Commercial Items, which is incorporated by reference. If the end product (s) is other than domestic end product(s) as defined in the clause entitled Buy American Act Supplies, the offeror shall so state and shall list the country of origin. FAR 52.212-2 (OCT 2014) Evaluation Commercial Items- The following factors shall be used to evaluate offers: Only certified Veteran Owned Small Businesses (VOSB) in VetBiz will be evaluated Technical Capability of the item offered to meet the Government requirement Price Offerors, if not registered in SAM, must include completed copies of the provision at 52.212-3 (MAR 2015), Offeror Representations and Certifications - Commercial Items with their offer. The provision may be obtained via the internet at URL: http://farsite.hill.af.mil/reghtml/regs/far2afmcfars/fardfars/far/52_000.htm. These representations and certifications will be incorporated by reference in any resultant contract. FAR 52.212-4 (DEC 2014), Contract Terms and Conditions-Commercial Items is applicable. The following identified clauses are incorporated by reference: 852.219-11, 852.232-72, 852.237-70, 852.246-70, 852.246-71, 52.211-16, 52.216-1, 52.222-41, 852.270-1, and 852.273-74. FAR 52.212-5 (MAR 2015), Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items is applicable and the following identified clauses are incorporated by reference: 52.233-2, 52.203-13, 52.203-17, 52.204-9, 52.228-5. Quotes for the items described above are due by 12 NOON PM EST on Wednesday September 27, 2017 to Darlene Chase, Darlene.Chase@va.gov and shall include solicitation number, FOB destination to the Wm. Jennings Bryan Dorn VA Medical Center, 6439 Garners Ferry Rd, Columbia, SC 29209 - operating room., proposed delivery schedule, discount/payment terms, warranty duration and DUNS number.
