Description

Synopsis: In accordance with Federal Acquisition Regulation 5.201, this is a notice of the VA s intent to enter into a sole source contract with Alvarez & Associates LLC. for AviTracks Software.   The West Palm Beach VA Medical Center has a requirement for AviTracks Software. AviTracks is a is a Chronic Disease Management tool for the SCAN-ECHO Center to improve patient monitoring by allowing clinicians a disease oriented view of the patient's data that is organized in a user friendly review related to patient treatment plan. The Government intends to award a sole source contract for this requirement. The suggested contractor is Alvarez & Associates, LLC.   8601 Georgia Ave, Silver Spring, MD 20910-3476. This is not a solicitation. However, companies which assert that they can provide this product or an equal product should submit information to include technical specifications of their product, past performance information, price, product availability and delivery time.  Information should be submitted by email to the Point of Contact below by September 30, 2017 at 5 p.m. EST.   This information will be reviewed for consideration for future requirements.
