Description
DEPARTMENT OF VETERANS AFFAIRS SEEKING COMMUNITY BASED OUTPATIENT CLINIC SERVICES The Department of Veterans Affairs (VISN 22 Gilbert Office) desires to establish community based outpatient clinic services in Payson, Arizona in order to provide our veterans in those areas with quality medical care. The Phoenix VA Health Care System (PVAHCS) requires a Contracted Community Based Outpatient Clinic (CCBOC) service to be provided in a private hospital, office or clinic environment to veterans primarily residing in Payson, Arizona. A Primary care CBOC often provides home-based primary care (HBPC) and home telehealth to the population it serves to meet the primary care and mental health needs of Veterans who have difficulty accessing clinic-based care. Primary care in VA includes both medical and mental health care services, as they are inseparable in providing personalized, proactive, patient-centered health care. Primary Care CBOCs are required to provide both primary care and mental health services. Sites that do not provide both primary care and mental health services are classified as Other Outpatient Services. Access to specialty care is not provided on site, but may be available through referral or telehealth. Primary Care CBOC: offer both medical (physically on site) and mental health care (either physically on site or by telehealth) and may offer support services such as pharmacy, laboratory, and x-ray. The contractor performing the services shall provide a continuum of care from prevention to diagnosis and treatment, to appropriate referral and follow-up. The contractor shall provide all personnel, facilities, and equipment necessary to provide primary care to veteran patients in the Payson, Arizona area. The contractor s outpatient site of care must have the necessary professional medical staff, diagnostic testing and treatment capability, and referral arrangements needed to ensure continuity of health care. The contractor shall support the delivery of clinical care in situations in which patient and provider are separated by geographic distance (i.e. telehealth support). The estimated number of vested patients in Payson, Arizona and surrounding area is 800 veterans. The facility shall be open during normal business hours (M-F 8:00am 4:30pm). The contractor is responsible for meeting or exceeding all VA and Joint Commission (or equivalent) standards, National Fire Protection Association (NFPA) Life/Safety requirements, the Americans with Disabilities Act, and Occupational Safety and Health Administration (OSHA). To meet VA s requirements for a Veterans Only clinic in a co-located facility, the Contractor s site  must have separate signage, a separate waiting room, and dedicated staff for the site. Services will be procured under Title 38 United States Code 8153 on behalf of PVAHCS. Contract period is date of award for one base year with four option year renewals. The NAICS is 621498; size standard is $8.5M.
