Courier Service for VA Medical Center Chillicothe and CBOC locations 3. Background. Courier service needed to transport lab specimens, sterile instruments, soiled instruments, urgent lab supplies, and pharmacy documents A) From Chillicothe VAMC to 6 separately tasked CBOC locations with two stops at most locations. B) From 6 separately tasked CBOC locations back to the Chillicothe VAMC 4. Scope. Contractor is needed to transport Lab Specimens from the Chillicothe VAMC to the locations as described below in section 5. Additionally Contractor is needed to deliver sterile instrumentation and lab supplies (if lab supplies are required prior to facility routine delivery schedule), and transport lab specimens and soiled medical instruments from 6 separately tasked CBOC locations to the Chillicothe VAMC on a daily basis excluding days CBOCs are closed for business as described below in section 5. In order to try to cut the increased cost from the initial proposal, the revised proposal for RME pickup/delivery is as follows: (this allows 1 driver to pickup/deliver 2 locations each day, thus decreasing the number of required drivers and decreasing cost) Athens Pickup/Delivery: - Pickups shall be made, on a daily basis, at the VA CBOC/ADS, Monday through Friday excluding federal holidays. Pickups shall be made NO EARLIER THAN 4:00 p.m. but NO LATER THAN 4:15 p.m. -Contractors(s) shall pickup sterile/clean instrumentation from SPS building 24 for delivery to the CBOC, Athens, Ohio on Monday, Wednesday, and Friday each week. Instruments will be delivered NLT 4:15 p.m. on the specified day. Marietta Pickup/Delivery: - Pickups shall be made, on a daily basis, at the VA CBOC/ADS, Monday through Friday excluding federal holidays. Pickups shall be made NO EARLIER THAN 2:30 p.m. but NO LATER THAN 2:45 p.m. and then the driver will travel to the Athens, Ohio CBOC for their pickup. -Contractors(s) shall pickup sterile/clean instrumentation from SPS building 24 for delivery to the CBOC, Marietta, Ohio on Monday, Wednesday, and Friday each week. Instruments will be delivered NLT 2:45p.m. on the specified day. Cambridge Pickup/Delivery: -Pickups shall be made, on a daily basis, at the VA CBOC/ADS, Monday through Friday excluding federal holidays. Pickups shall be made NO EARLIER THAN 2:15 p.m. but NO LATER THAN 2:30 p.m. and then the driver will travel to the Lancaster, Ohio CBOC for their pickup. -Contractors(s) shall pickup sterile/clean instrumentation from SPS building 24 for delivery to the CBOC, Cambridge, Ohio on Monday, Wednesday, and Friday each week. Instruments will be delivered NLT 2:30p.m. on the specified day. Lancaster Pickup/Delivery: -Pickups shall be made, on a daily basis, at the VA CBOC/ADS, Monday through Friday excluding federal holidays. Pickups shall be made NO EARLIER THAN 4:00 p.m. but NO LATER THAN 4:15 p.m. -Contractors(s) shall pickup sterile/clean instrumentation from SPS building 24 for delivery to the CBOC, Lancaster, Ohio on Monday, Wednesday, and Friday each week. Instruments will be delivered NLT 4:15 p.m. on the specified day. Portsmouth Pickup/Delivery: -Pickups shall be made, on a daily basis, at the VA CBOC/ADS, Monday through Friday excluding federal holidays. Pickups shall be made NO EARLIER THAN 4:00 p.m. but NO LATER THAN 4:15 p.m. -Contractors(s) shall pickup sterile/clean instrumentation from SPS building 24 for delivery to the CBOC, Portsmouth, Ohio on Monday, Wednesday, and Friday each week. Instruments will be delivered NLT 4:15 p.m. on the specified day. Wilmington Pickup/Delivery: -Wilmington DOES NOT have RME, pickup/Delivery would be for LAB ONLY. -Pickups shall be made, on a daily basis, at the VA CBOC/ADS, Monday through Friday excluding federal holidays. Pickups shall be made NO EARLIER THAN 2:00 p.m. but NO LATER THAN 2:15 p.m. and then driver will travel to the Portsmouth, Ohio CBOC for their pickup. -Contractor(s) shall pickup VA-owned empty lab transport containers from Laboratory at the VA Medical Center, Chillicothe, Ohio and return the empty lab transport containers to the CBOC, Wilmington, Ohio on the next day pickup. (This note applies to all CBOCS). There are other LAB requirement (pickups/deliveries) that do not apply to RME pickup/deliveries. 5. Specific Tasks. 5.1 Task 1 Laboratory Transport to Cincinnati VAMC and Dayton VAMC a. Contractor shall provide courier/transport services on a daily basis Monday through Saturday of each week, excluding Sundays and holidays to the VA Medical Center, Cincinnati, Ohio; located 3200 Vine Street, Cincinnati, Ohio 45220 VA Medical Center, then to Dayton, OH; located 4100 W. Third Street, Dayton, OH 45428 from the VA Medical Center, Chillicothe, Ohio located 17273 State Route 104, Chillicothe, Ohio. Services include the pickup, transport, and delivery of laboratory specimens. b. Contractor(s) shall pickup VA-owned empty lab transport containers from Laboratory at the VA Medical Center, Cincinnati, Ohio and from Laboratory at the VA Medical Center, Dayton, OH and return the empty lab transport containers to the VA Medical Center, Chillicothe, Ohio on the next day pickup. c. Pickups shall be made, on a daily basis, at the VA Chillicothe, Building 31, Pathology and Laboratory Medicine Service, Monday through Saturday excluding federal holidays. Pickups will be made between 7:30 a.m. and 8 a.m. d. The contractor shall deliver all specimens to the VA Pathology and Laboratory Medicine Service, Room B211, VA Medical Center, Cincinnati, Ohio, no later than 10:00 a.m. and the contractor shall deliver all specimens to the VA Medical Center 4100 W. Third Street, Pathology and Laboratory Medicine Service, Building 310 Room 2E-130, Dayton, OH 45428 no later than 12:30 p.m. on the same day specimens are picked up. e. Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract. f. Sunday courier service will be on an as needed basis. Laboratory personnel will notify contractor prior to 11:00pm on Saturday if service is required. 5.2 Task 2 Laboratory Specimen Transport to Adena Regional Medical Center a. Contractor shall provide courier/transport services including the pickup, transport and delivery of laboratory specimens on an as needed basis Monday through Sunday of each week, including holidays from the Laboratory at VA Medical Center, 17273 State Route 104, Chillicothe, Ohio to Adena Regional Medical Center, Hospital Road, Chillicothe, Ohio, Contractors shall pickup VA-owned empty containers from Adena Regional Medical Center and return the containers to the laboratory at the VA Medical Center, Chillicothe, Ohio. b. The contractor shall report within 30 minutes of notification of VA staff of the need for courier services. c. The contractor shall only charge based on actual pickup. d. Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract 5.3 Task 3 Transport from Marietta CBOC and Athens CBOC to Chillicothe VA Medical Center a. Contractor shall provide courier/transport services on a daily basis Monday through Friday of each week, excluding federal holidays to the VA Medical Center, 17273 SR 104 Chillicothe, OH 45601 from the VA Community Based Outpatient Clinic (CBOC) located at 27843 St. Rt. 7, Marietta, OH 45750 and the VA Community Based Outpatient Clinic (CBOC) located at 510 West Union Street, Athens, OH 45701.. b. Courier services for the Marietta CBOC include the pickup, transport, and delivery of laboratory specimens and urgently needed supplies, sterile/unsterile instruments, and pharmacy documents. c. Pickups shall be made, on a daily basis, at the VA CBOC/ADS, Monday through Friday excluding federal holidays. Pickups shall be made NO EARLIER THAN 2:30 p.m. but NO LATER THAN 2:45 p.m. and then the driver will travel to the Athens, Ohio CBOC for their pickup but NO LATER THAN 4:15 p.m. d. The contractor(s) shall deliver all specimens to the VA Pathology and Laboratory Medicine Service, Building 31, VA Medical Center, Chillicothe, Ohio, by 6:00 p.m. but no later than 6:30 p.m. on the same day pickups are made. In the event there is no one in the VA Laboratory at the time of delivery, specimens will be delivered to the admissions area on Building 31. e. The contractor(s) shall deliver all soiled instrumentation to building 24, room GC106A on a daily basis. f. Due to the nature of this business it is necessary for the contractor to provide daily pickups and deliveries unless the CBOC s are closed due to inclement weather. Cancellations of the courier service should only occur as a last resort. g. Contractor(s) shall pickup VA-owned empty lab transport containers from Laboratory at the VA Medical Center, Chillicothe, Ohio and return the empty lab transport containers to the CBOC, Marietta, Ohio and the CBOC, Athens, Ohio on the next day pickup. h. Contractors(s) shall pickup sterile/clean instrumentation from SPS building 24 for delivery to the CBOC, Marietta, Ohio and the CBOC, Athens, Ohio on Monday, Wednesday, and Friday each week. Instruments will be delivered NLT 2:45p.m. and NLT 4:15pm respectively, on the specified day. i. Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract. 5.4 Task 4 Transport from Cambridge CBOC and Lancaster CBOC to Chillicothe VA Medical Center a. Contractor shall provide courier/transport services on a daily basis Monday through Friday of each week, excluding federal holidays to the VA Medical Center, 17273 SR 104 Chillicothe, OH 45601 from the VA Community Based Outpatient Clinic (CBOC) located at 2146 Southgate Parkway, Cambridge, OH 43725 and from the VA Community Based Outpatient Clinic (CBOC) located at 1703 North Memorial Drive, Lancaster, OH 43130. b. Courier services for the Cambridge CBOC includes the pickup, transport, and delivery of laboratory specimens and urgently needed supplies, sterile/unsterile instruments, and pharmacy documents. c. Pickups shall be made, on a daily basis, at the VA CBOC/ADS, Monday through Friday excluding federal holidays. Pickups shall be made NO EARLIER THAN 2:15 p.m. but NO LATER THAN 2:30 p.m. and then the driver will travel to the Lancaster, Ohio CBOC for their pickup. Pickups shall be made NO EARLIER THAN 4:00 p.m. but NO LATER THAN 4:15 p.m. d. The contractor(s) shall deliver all specimens to the VA Pathology and Laboratory Medicine Service, Building 31, VA Medical Center, Chillicothe, Ohio, by 6:00 p.m. but no later than 6:30 p.m. on the same day pickups are made. In the event there is no one in the VA Laboratory at the time of delivery, specimens will be delivered to the admissions area on Building 31. e. The contractor(s) shall deliver all soiled instrumentation to building 24, room GC106A on a daily basis. f. Due to the nature of this business it is necessary for the contractor to provide daily pickups and deliveries unless the CBOC s are closed due to inclement weather. Contractor(s) shall pickup VA-owned empty lab transport containers from Laboratory at the VA Medical Center, Chillicothe, Ohio and return the empty lab transport containers to the CBOC, Cambridge, Ohio and to the CBOC, Lancaster, Ohio on the next day pickup. h. Contractors(s) shall pickup sterile/clean instrumentation from SPS building 24 for delivery to the CBOC, Cambridge, Ohio on Monday, Wednesday, and Friday each week. Instruments will be delivered NLT 2:30p.m. on the specified day. i. Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract. 5.5 Task 5 Transport from Wilmington CBOC and Portsmouth CBOC to Chillicothe VA Medical Center a. Contractor shall provide courier/transport services on a daily basis Monday through Friday of each week (excluding federal holidays) to the VA Medical Center 17273 SR 104 Chillicothe, OH 45601 from the VA Community Outreach Clinic 448 West Main Street Wilmington, OH 45177 and from the VA Community Based Outpatient Clinic (CBOC) located at 840 Gallia Street, Portsmouth, OH 45662. b. Courier services for the Wilmington and Portsmouth CBOC s include the pickup, transport, and delivery of laboratory specimens and urgently needed supplies and pharmacy documents. c. Pickups shall be made, on a daily basis, at the VA CBOC/ADS, Monday through Friday excluding federal holidays. Pickups shall be made NO EARLIER THAN 2:00 p.m. but NO LATER THAN 2:15 p.m. and then driver will travel to the Portsmouth, Ohio CBOC for their pickup. Pickups shall be made NO EARLIER THAN 4:00 p.m. but NO LATER THAN 4:15 p.m. d. The contractor(s) shall deliver all specimens to the VA Pathology and Laboratory Medicine Service, Building 31, VA Medical Center, Chillicothe, Ohio, by 6:00 p.m. but no later than 6:30 p.m. on the same day pickups are made. In the event there is no one in the VA Laboratory at the time of delivery, specimens will be delivered to the admissions area on Building 31. e. Due to the nature of this business it is necessary for the contractor to provide daily pickups and deliveries unless the CBOC s are closed due to inclement weather. Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract h. Contractors(s) shall pickup sterile/clean instrumentation from SPS building 24 for delivery to the CBOC, Portsmouth, Ohio on Monday, Wednesday, and Friday each week. Instruments will be delivered NLT 4:15 p.m. on the specified day. 5.6 Task 6 Laboratory Specimen Transport to American Red Cross a. Contractor shall provide courier/transport services on a as needed basis Monday through Sunday of each week, including holidays to the American Red Cross 995 E Broad St, Columbus, OH 43205 from the VA Medical Center, Chillicothe, Ohio located 17273 State Route 104, Chillicothe, Ohio. Services include the pickup, transport, and delivery of laboratory specimens. b. Contractor(s) shall pickup VA-owned empty lab transport containers from the American Red Cross and return the empty lab transport containers to the VA Medical Center, Chillicothe, Ohio by the next day. c. Pickups shall be made, on an as needed basis, at the VA Chillicothe, Building 31, Pathology and Laboratory Medicine Service, Monday through Sunday, including federal holidays. d. Contractors shall only charge based on actual pickup. e. The contractor shall deliver all specimens to the American Red Cross 995 E Broad within 2 hours pickups are made. f. Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract. 5.7 Task 7 Specimen and Instrumentation Preparation The VA Medical Center personnel will properly prepare and package specimens and instruments for transport by the contractor. Courier personnel will sign for receipt of specimens to be transported. The lab specimens shall be transported in such a manner as to ensure the integrity of the specimen(s). Instruments shall be transported in such a manner as to ensure bins remain flat, not on end or side, to prevent instrument damage. 6. Performance Monitoring RECORD OF PICK-UP AND DELIVERY The contractor shall maintain a record and log of daily courier pick-ups and deliveries indicating time and date of pick-up, delivery, number of transport containers and signatures. The logs shall be submitted to the COR or authorized representative of the COR on a monthly basis. The contractor shall sign a log indicating date and time of daily courier pick-ups and number of transport containers from the VA Chillicothe 7. Security Requirements LICENSES/PERMITS/CERTIFICATIONS Contractor shall have all licenses, permits, and/or certification as required by the State of Ohio for the transport of laboratory specimens. Contractor shall be responsible for complying with all regulations that define appropriate transportation of specimens/soiled instruments and shall supply, in writing, proof that all drivers have been adequately trained on an annual basis. The contractor s employees/couriers transporting test specimens/soiled instruments shall be trained in Universal Precautions in the handling of biohazard materials (specimens). Training shall include utilizing clinical procedures in case of biohazard spills and accidents. The contractor shall furnish a record of all required training. All drivers shall have attained legal age (the age of 18 years) and shall maintain neat, clean and professional appearance. Drivers shall be licensed in accordance with the State of Ohio to include any special licensure required for the transport of diagnostic or infectious substances. CONTRACTOR PERSONNEL a. The contractor is responsible for having sufficient qualified personnel to meet the requirements as stated herein. This includes backup personnel to cover in case of illness, injury, vacations, etc. b. The contract services and or personnel shall not be considered VA employees for any purpose. c. Personnel Clearances and Privacy Act Considerations. (1) All Contractor employees shall be required to observe the requirements imposed on sensitive data by law, Federal regulations, VA statutes and policy, and associated requirements to ensure appropriate screening of all personnel. Because of the inherent sensitivity of the data at the clinic facilities and data to be maintained at the facilities, the level of security clearance required of the Contractor s employee shall reflect the same level of security clearance carried by the VA personnel for the function under contract. The Contractor shall ensure that its personnel meet the above restrictions and that confidential and proprietary information shall be divulged only to those officers and officials of the VA who are authorized to receive such information. (2) All contractor employees who require physical access to the Department of Veterans Affairs facility shall be subject of a background investigation and must receive a favorable Adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the contractor will be responsible for the actions of those individuals they provide to perform work for the VA. Position Sensitivity The position sensitivity has been designated as Low Risk. Background Investigation The level of background investigation commensurate with the required level of access is national Agency Check with Written Inquiries. (3) Contractor Responsibilities The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM), the contractor shall reimburse VA within 30 days. The contractor shall submit or have their employees submit the following required forms to the VA Office of Security and Law Enforcement within 30 days of receipt: (i) Standard Form 85P, Questionnaire for Public Trust Positions (ii) Standard Form 85P-S, Supplemental Questionnaire for Selected Positions (iii) FD 258, U.S. Department of Justice Fingerprint Applicant Chart (iv) VA Form 0710, Authority for Release of Information Form (v) Optional Form 306, Declaration for Federal Employment (vi) Optional Form 612, Optional Application for Federal Employment The contractor shall provide a vehicle used for transport of laboratory specimens, instruments or soiled instruments that must be temperature and humidity controlled. (4) Government Responsibilities The VA Office of Security and Law Enforcement will provide the necessary forms to the contractor or to the contractor s employees after receiving a list of names and addresses. Upon receipt, the VA Office of Security and law Enforcement will review the completed forms for accuracy and forward to OPM to conduct the background investigation. The VA Facility will pay for investigations conducted by the Office of Personnel Management (OPM) in advance. In these instances the contractor will reimburse the VA Facility within 30 days. The VA Office of Security and Law Enforcement will notify the contracting officer and contractor after adjudicating the results of the background investigations received from OPM. The Contracting Officer will ensure that the contractor provides evidence that investigations have been completed or are in the process of being requested. d. The VA reserves the right to restrict any Contractor employee from performing services under this contract who does not meet the required qualifications for the services they perform and/or who violates federal regulations or is identified as a potential threat to the security, safety, health or operational mission of the Department of Veterans Affairs Medical Centers and their populations. The restriction of such a person shall not relieve the Contractor from providing all the required services as described herein. e. At all times during the performance of this contract, Contractor employees shall conduct themselves in a professional manner, wear clean, neat uniforms, and be identified by having company name and/or logo, and employee name attached to their uniforms, by name tag or other means. The Contractor shall be responsible for acquiring an appropriate number of uniforms, and tags or badges to meet this requirement. Employee identification shall be worn in clear view above the waist. f. Under no circumstances shall any person other than those personnel designated in writing by the contractor will be allowed to handle items being transported while in transit or deliver specimens to the laboratory. g. Prior to the commencement of contract, the Contractor shall provide the Contracting Officer a personnel roster identifying all Contractor personnel who will be performing contract services, including any personnel used as "backup" personnel. In this document the Contractor shall identify the name, position title, and work assignment area of each employee. Thereafter, all changes shall be submitted no later than two (2) business days prior to substitution. PRIVACY/RELEASE OF INFORMATION a. Personnel Clearances and Privacy Act Considerations. Other Pertinent Information or Special Considerations. EVIDENCE OF INSURANCE COVERAGE. Before commencing work under the contract, the contractor shall furnish to the Contracting Officer, a certificate from his/her insurance company indicating that the contractor has obtained liability insurance coverage and that it may not be changed or canceled without written notice to the Contracting Officer. The furnishing of such evidence of insurance coverage may not be waived and certificate of insurance shall be submitted with quote. SCHEDULE OF INSURANCE COVERAGE. The following minimum insurance coverage shall apply to this contract: Workers' Compensation and Employer's Liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when the contract operations are so commingled with a contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers (2) General Liability: $500,000 per occurrence. (3) Automobile Liability: $250,000 per person; $500,000 per occurrence and $100,000 property damage. To carry out this responsibility, the contractor shall provide the following for their personnel: Worker s Compensation Professional Liability Insurance Health Examinations (as may be required) Income Tax Withholding, and Social Security Payments b. The contract personnel shall not be considered VA employees for any purpose. SAFETY a. While performing services under any resulting contract, numerous potential hazardous conditions may develop and/or exist. These conditions can result in serious personal injuries if not identified and if corrective action is not taken to prevent injury. Adherence to OSHA regulations is not always sufficient to provide the degree of safety that is required. Extra precautions should be observed when performing the required services. b. The safety and health of patients, visitors, and VA employees at the VA Medical Center, as well as contractor employees, must be considered at all times while services are being performed. Never leave hazards unattended or unsecured. Psychiatric, blind, hearing impaired, and wheelchair patients are to be expected. Extra caution must be exercised when in buildings, which are occupied, and doing business. HARASSMENT POLICY a. It is the policy of the VA Medical Center, 17273 S.R. 104, Chillicothe, Ohio, to create an environment free of discrimination and to promote an atmosphere for the residents, visitors and workforce free of discrimination and/or harassment, without regard to race, color, religion, sex, national origin, and age, physical or mental handicap. b. It is the responsibility of the contractor awarded a contract to perform services on a Federal worksite to take any and all necessary measures to prevent sexual harassment and discrimination from occurring, by assuring that his/her employees and those of his/her subcontractors maintain the highest degree of conduct and standards in this regard. c. These steps should include, but not be limited to, affirmatively raising the subject with employees and subcontractors, expressing strong disapproval when discovering or being informed that an incident has occurred, and developing appropriate guidelines and sanctions for conduct and/or misconduct pertaining to such matters. d. Any complaints received by the Contractor, alleging misconduct toward contractor employees from other than contractor related personnel, i.e., VA personnel, patients and/or visitors shall be reported to the Contracting Officer in charge of the awarded contract. SMOKING POLICY. a. The VA Medical Center, Chillicothe, Ohio, is a smoke free facility. NO SMOKING is permitted within the confines of any building or within 35 feet of any entrance to a building. Smoking is permitted in designated areas only. Failure to comply with the smoking policy will result in immediate removal of contractor/contractor personnel from the facility. WAGES TO BE PAID. Wages to be paid in accordance with Wage Determination No. 2005-2423 (Revision No. 11) dated 8/3/2010 applicable to any resulting contract and a copy of the WD is attached. DOL Classification Driver Courier 31043. PAYMENT/INVOICE Payment for services will be made monthly, in arrears, upon receipt of properly prepared invoices. Invoices must contain location name, dates of service, unit cost, and total amount due on the invoice. The applicable VA Contract Number and purchase order number must be indicated on each invoice. Invoices are to be electronically mailed to: Department of Veterans Affairs FMS VA-2 (101) Financial Services Center P. O. Box 149971 Austin, TX 78714-8971 10. Risk and Quality Control The contractor shall develop and maintain a quality control program that ensures courier services are performed in accordance with these specifications. The contractor shall develop and implement procedures to track, identify, prevent, remedy and ensure non-recurrence of defective services. The contractor shall provide a method to accept and resolve customer complaints and notify the customer of the resolution. The contractor shall immediately notify the customer of the resolution. The contractor shall immediately notify the CO/COR upon receipt of a customer complaint in order that joint validation may be accomplished. The contractor is responsible for quality control and specification compliance. 11. Place of Performance. As described in section 5 12. Period of Performance (Date of Delivery for one time purchase). 11/1/2017- 10/31/2018 with 4 option years beginning 11/1/2018 and ending 10/31/2021. 13. Delivery Schedule. As described in section 5
