Page 1 of 3 SOURCES SOUGHT This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this is to gain knowledge of potential qualified sources and their size relative to NACIS 562910. Responses will be used by the Government to make appropriate acquisition decisions. Responses are not considered responses to any potential solicitation announcement. All interested firms shall provide the following information: Company name, Address, Point of Contact, Telephone number, Email address, Capability statement, Business size, DUNS and GSA contract (if any) pursuant to the NAICS code 562910. Responses to this notice shall be e-mailed to jeneice.mcclinton@va.gov. Telephone responses will not be accepted. Responses shall be received in writing no later than Tuesday, October 10, 2017 at 10 a.m. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties shall respond to that solicitation announcement separately from the responses to this announcement. Responses are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. SCOPE OF WORK MOLD REMEDIATION SERVICES SITE LOCATION: Michael E. DeBakey VA Medical Center 2002 Holcombe Blvd. Building 100, Nursing Unit 1C Houston, TX 77030 DESCRIPTION OF WORK: The contractor and sub-contractor shall be currently certified in mold remediation. The Contractor s personnel shall be responsible for conformance with Federal, State codes and Regulations. The contractor shall provide all labor, materials, tools, personnel, transportation, equipment, and services necessary for this project as described here in other specific tasks as further defined. The contractor shall provide removal and remediation of building materials that are showing visible signs of mold growth, excessive water saturation or moisture intrusion on drywall, plaster, ceiling tiles, wall insulation, in building 100, Nursing unit 1C for 15 rooms. The work shall be completed in two phases. Phase I The Contractor shall remove excessively moisture, saturated and mold contaminated sheet rock and wall paper. Work shall include the opening of wall cavities, scrub, and clean with a mold inhibitor and allow to completely dry. Prior to any build-back, the Contractor shall perform test moisture level checks. Work shall be performed in such a way to minimize the impact on any user/building occupants and patients, and will minimize the amount of construction dust outside of the work areas. The contractor shall be responsible for the daily cleaning and disposal of construction debris. Work shall be coordinated with the Contracting Officer Representative (COR). The Contractor shall have wall paper and contaminated sheetrock removed in the following fifteen (15) rooms: 1C-120 1C-122 1C-124 1C-126 1C-128 1C-130 1C-134 1C-140 1C-142 1C-144 1C-146 1C-148 1C-150 1C-152 1C-154 Phase II The Contractor shall build back portions of the project. The Contractor shall build back fifteen (15) remediated rooms as followed: Install vapor barrier Install Green board sheet rock at the bottom of the window and sink Install regular sheet rock on remaining walls Replace insulation where removed Tape Float Paint The Contractor shall perform IAQ monitoring to be conducted by a certified IAQ with final clearance sampling to be completed at the end of the project, prior to patient and staff re-occupancy. The Contractor shall provide a final IAQ report within 5 days of project completion. PERFORMANCE PERIOD: All Contractor work shall be accomplished within 60 calendar days, including any time for shipping and handling of equipment. WORKING HOURS: The normal hours of work at the Michael E. DeBakey VA Medical Center/Building 100, shall be from 7:00a.m. to 4:30p.m., CST, Monday through Friday, excluding holidays. All work will be inspected by the Safety Office prior to building material re- installation. Holidays observed by the MEDVAMC are: New Year s Day Martin Luther King Day President s Day Memorial Day Independence Day Labor Day Columbus Day Veteran s Day Thanksgiving Day Christmas Day NOTIFICATION RESPONSIBILITIES: The Contractor shall notify the Contracting Officer Representative, via phone or E-mail, a minimum of 24 hours in advance of: (1) initially starting work or starting work after a multi-workday pause, (2) ceasing work for a multi-workday pause, and (3) deliveries of major materials or equipment. CONTRACTOR QUALIFICATIONS: Contractor personnel shall possess all current certifications required by federal, state and local regulations to perform the work. Copies of these certifications shall be no older than 1 year, and shall be submitted as a part of the package to contracting. SITE SUPERVISOR AND LANGUAGE REQUIREMENTS: The contractor shall provide a job foreman or site manager who can speak in English. An English-speaking contractor employee shall be on site at all work times. WORK AREA MONITORING: Personnel must be a certified Industrial Hygienist, certified by the American Board of Industrial Hygiene, and licensed by the Texas Department of State Health Services as a mold consultant. Contractor shall provide the VA a final report inclusive of air monitoring clearance results within 5 days of project completion. CONTRACTOR STORAGE: An on-site open and/or closed storage area for Contractor project material may be designated and set-aside by the Contracting Officer Representative. Security of the storage area and project materials shall be the responsibility of the Contractor. The Contractor shall keep the storage area neat, orderly and clear of all debris. The Contractor shall not allow trash or other debris to be scattered from his area by wind or other means. All storage areas shall be kept clear of trash and building material debris. OPEN FLAME, WELDING PERMITS: The Contractor shall receive all required permits for any welding or other open flame work to be done on from the Safety Office before such work is performed. UTILITIES SERVICE: The Contractor may access available utilities (electricity and water) at the building site. It shall be the responsibility of the Contractor to coordinate with local utility companies for the hook-up of utilities to trailers or other site offices. No utilities will be provided by the government at any contractor open storage area. ELECTRICAL SERVICE AND COMMUNICATIONS FACILITIES: The Contractor shall not disturb electrical and/or communications facilities and wiring. Where such facilities require removal for accomplishment of the task, the Contractor shall notify the Contracting Officer Representative at least 24 hours of the requirement, for the accomplishment of such removals by the Government. The Contractor shall allow the telephone company and communication personnel access to the systems required. If damage by construction activities occurs, the Contractor shall restore service within 24-hours with no cost to the government. MATERIAL REMOVED: All materials not reused in the project will be designated as either salvageable or unsalvageable by the Contracting Officer Representative. Salvageable materials shall remain the property of the government and shall be disposed of by the Contractor as directed by the Contracting Officer Representative. Materials to be removed shall be removed without damage to adjacent areas. All damage resulting from removal shall be repaired by the Contractor at no cost to the government with materials of like nature, type, and construction as were damaged or with materials to match existing adjacent areas. Unsalvageable materials shall be considered Construction and Demolition wastes to be disposed of by the contractor. If the unsalvageable materials are relatively small, the contractor may dispose of the materials in the solid waste dumpster behind the boiler plant. CONSTRUCTION AND DEMOLITION WASTES: The Contractor shall separate and handle all hazardous wastes according to the Environmental Protection Statement. All remaining nonhazardous waste shall be considered construction and demolition (C&D) waste. The Contractor shall dispose of all waste in accordance with all applicable regulations.
