No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. -------- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FA6643-17-R-0011, and the solicitation is being issued as a request for proposal (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. (iv) This solicitation is a competitive 8(a) effort. Offers are only being solicited from the vendors identified in accordance with FAR 19.804-3. These vendors will be notified via email that the solicitation has been released. Proposals from any other vendors will not be considered. The associated NAICS code is 541511 and small business size standard is $27.5M. Questions related to this effort will only be accepted from the vendors identified, and must be submitted via email to katherine.whelchel@us.af.mil no later than 3PM EST 18 Sep 17. Please sanitize the questions to remove any company-specific information. Answers will be provided via email within two business days of the Q&A closing. (v) A list of line item number(s) and items, quantities, and units of measure, (including option(s), if applicable): A requirements "D-type" contract will be utilized for these services. The basic period will be 6 months, with four additional 12-month options thereafter. The total length of the contract including all options is 4.5 years. With a requirements contract, there are no minimum or maximum guarantees of work, nor is a funded order required to be placed at time of basic contract award. A best estimated quantity (BEQ) has been provided for each CLIN. More specifically, these services support Health Information and Readiness Systems Sustainment (HIS) and Electronic Case Tracking (ECT). Services shall be performed in accordance with the Performance Work Statement (PWS). (vii) Date(s) and place(s) of delivery and acceptance and FOB point: N/A. Place of Performance: On-site Contractor personnel shall perform at the office of the Command Surgeon (SG), Robins AFB GA, Bldg. 549; 78 MDG, Bldg. 700 Rm 1064 and AFRC Network Operations Support Center (NOSC), HQ AFRC/A6NS Robins AFB GA 31098, Bldg. 210. Inspection and Acceptance is at Destination. Performance during the contract will be evaluated in accordance with the Quality Assurance Surveillance Plan (QASP). The awardee will receive a Contracting Officer's Representative (COR) designation letter in conjunction with the award. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. The following addenda to the provision also apply: *Proposals must be submitted to HQ AFRC/PKA (katherine.whelchel@us.af.mil) electronically via the AMRDEC system. Instructions are attached. Hard copy proposals will not be accepted. Proposals must be valid through 15 Dec 17. *Offerors must include a completed pricing sheet with the proposal. Two sample sheets have been provided for guidance; do not make any edits to these sheets. *For the purpose of determing contractor responsibility, offerors must provide proof of all qualifications listed in PWS paragraph 5.22 with their proposals. (ix) The provision at 52.212-2, Evaluation -- Commercial Items applies to this acquisition and the specific evaluation criteria to be included in paragraph (a) of that provision are as follows: Award will be based on price only; no other factors (technical, past performance, etc.) will be evaluated. Offerors must submit a completed pricing sheet, which will be used to calculate the total evaluated price. The basic period and all four options will be considered in the evaluation. The methodology for calculating the total evaluated price is described in the pricing sheet.
