Description
This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number VA262-17-Q-2068 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation. It is the offeror s responsibility to monitor this site for the release of amendments (if any). This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95, effective January 19, 2017. This is a competitive solicitation; 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB). Offeror must be verified SDVOSB at the time of offer submission due date. The North American Industry Classification System (NAICS) code for this acquisition is 221310 Water Treatment and Distribution and the small business size standard is $27.5 million. Offeror must be registered in the System Award Management (SAM) at https://www.sam.gov in order to be considered for award. See Attachment I, Section B.2 Schedule of Services and Price for the contract line item numbers (CLIN), quantity and unit of measures. Description of services: The Government is seeking for a qualified and licensed contractor to provide a comprehensive solid water treatment services at VA Loma Linda Healthcare System. See Attachment I, Section B.3 for the Statement of Work (SOW). Period of Performance: The work to be performed is anticipated to begin November 15, 2017 thru November 14, 2018. Place of Performance: VA Loma Linda Healthcare System, 11201 Benton Street, Loma Linda, CA 92354 FOB Terms: Destination The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instruction to Offerors - Commercial Items (Oct 2015), applies to this acquisition. See Attachment I, Section E.1 for the addendum to FAR 52.212-1. A similar provision at FAR 52.212-2, Evaluation Commercial Items is used. See Attachment I, Section E.2 for the Basis for Award. Offeror shall include a completed a copy of FAR 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer. The clause at FAR 52.212 4, Contract Terms and Conditions --Commercial Items, applies to this acquisition and full text may be accessed electronically at http://farsite.hill.af.mil. See Attachment I, Section C.1 for the addendum to FAR 52.212-4. The clause at FAR 52.212 5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition. See Attachment I, Section C.2. Additional contract requirements necessary for this acquisition is attached. See Attachment 1, Section B.4 for Special Contract Requirements. Defense Priorities and Allocations System (DPAS) and assigned ratings are not applicable to this acquisition. Offer is due on October 18, 2017 at 1:00 PM PDT. All email attachments in response to this RFQ shall be in either Adobe or Microsoft Office format. Offeror is advised that the Government may be unable to receive other types of electronic files (e.g. compressed or zip files) or files more than ten (10) megabytes (MB). It is the offeror s responsibility to ensure all required documents are included and completed as required by this solicitation.
