- ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS COMMERCIAL ITEMS Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The following provisions are incorporated into 52.212-1 as an addendum to this solicitation: Only VA verified VOSBs in the Vendor Information Pages (VIP) database (http://www.vip.vetbiz.gov) at the time of submission of offer will be considered for award and non-VIP verified firms will be considered non-responsive and ineligible for award. Offerors shall be registered as a Small Business for NAICS 621111 and shall be represented as such within its System for Award Management (SAM) registration. All questions shall be received via email to carlos.leon@va.gov NLT October 19, 2017 at 2:00 pm ET. The Government reserves the right to refrain from answering questions not meeting this timeline. SUBMITTAL INSTRUCTIONS: The quote must be complete and respond directly to the requirements of the solicitation using the following format. 1. File#1: Schedule a. Signed SF 1449 (complete blocks 17, 30a, 30b and 30c) 2. File #2: Technical Provide curricula vitae for their proposed candidate/s and the additional applicable documentation as stated below; Full, active, current and unrestricted license in a State, Territory, Commonwealth of the United States, or in the District of Columbia. List the State(s) currently licensed to practice. (provide certificates) Prior work experience with a VA Medical Center. (if applicable). Those physicians with VA Medical Center experience will be rated higher than those without. Must have a Degree of Doctor of Medicine or an equivalent degree. The hospitalist locum shall be proficient in arthrocentesis, paracentesis, thoracentesis, lumbar puncture, and central line inset ions at minimum. Provide all malpractice related history. (if applicable). Shall be Board Certified or Board Eligible Physician within the United States with no restrictions. DEA Controlled Substance Registration Certificate. Complete Training History. Basic and/or Advanced Cardiac Life Support Certification. A provider with both certifications will be rated higher than a provider with only one certification. Curricula Vitae shall list three (3) references (name, title, address, telephone numbers & email address), including the current or last supervisor, that can validate professional personnel performance. Updated VetPro registration. (yes or no) Current background investigation: National Agency Check & Inquiry (NACI). (if applicable & date) Proposed Start Date Dates will be adjusted due to credentialing, background and availability. 3. File # 3: Timely patient access/ Quality Control Provide a plan detailing actions that will be taken by your company to ensure contract services will provide timely patient access and how are you going to insure quality control File #4: Past Performance Provide at a minimum two past performance references (i.e. name, title, telephone number, and project title/description) who can verify that YOUR company provided recent PHYSICIAN services either as the prime Contractor or Sub-Contractor. Recent is defined as within the past three years. All documents shall be submitted via email to carlos.leon@va.gov before the closing date and time of the solicitation (October 25, 2017). No other submission formats will be accepted.
