(i) This is a combined synopsis/solicitation for Smith and Nephew Endoscopic Punches, BRAND NAME, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24418Q0029 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS code 339112 has a small business size standard of 1000. These punches are designed specifically for and to be used with certain procedures for frozen shoulder syndrome. This request is being made to purchase Smith & Nephew Endoscopic Punches because they currently are not in the facility and are necessary for the procedures being performed here. The Smith & Nephew punches are an industry standard due to surgeons preference on the tactility, performance and specific angle of these punches. The distinct cupped underbite feature is what sets these punches apart from any other on the market. This facility scheduled shoulder surgery regularly and these punches are needed to accommodate those procedures.
