Description
COMBINED SYNOPSIS/SOLICITATION Audiology Calibration and Maintenance Contract (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and FAR part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number 36C24218Q0010. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1. (iv) This procurement is being issued as competitive small business set-aside. The North American Industry Classification System (NAICS) code is 811219, Other Electronic and Precision Equipment Repair and Maintenance, with a small business size standard of $20.5 Million. (v) The Contractor shall provide pricing to provide a service support agreement for Audiology Calibration and Maintenance located at the NJ VA Medical Center. Please see attachment and include price of service agreement in the attachment table along with 4 option years. (vi) Statement of Work This is a firmed fixed price purchase order, to include a base period of performance and four option years, for semi-annual calibration and preventative maintenance services for audiology equipment located at the VA New Jersey Health Care System, East Orange campus (located at 385 Tremont Avenue East Orange, NJ 07018), Lyons campus (located at 151 Knollcroft Road Lyons, NJ 07939), and Brick Campus (located at 970 Rt.70 Brick, NJ 08724). Please refer to the attached pricing sheet for a listing of equipment to be serviced. Base Period of Performance (POP) shall end on 09/30/2018 and POP for subsequent option years shall be from October 1st through September 30th of the subsequent year. Requirements: All work shall be performed by qualified NASED certified technicians to service the equipment using only new Original Manufacturer Parts (OEM). All calibrations shall be done on site East Orange Campus calibration and preventative maintenance service shall be performed in the months of October and April. For the base period of performance only one calibration and preventative maintenance service shall be required to be performed in the month of April. Brick and Lyon s Campuses calibration and preventative maintenance services shall be performed in the months of December and June. All work shall be scheduled with the Contract Officer Representative (COR) at least 1 week in advance during regular business hours Mon-Fri 8:00-4:30 p.m. excluding federal holiday. Federal holiday s shall be defined as: a. New Year s Day b. Memorial Day c. Columbus Day d. Christmas Day e. Martin Luther King Day f. Independence Day g. Veteran s Day h. President s Day i. Labor Day j. Thanksgiving The Contractor shall provide to the Biomedical Division a full service report and Calibration Certificates within three (3) days after completion of all service performed. This service report shall include, but not be limited to: contract number, contractor s log number, detailed description of the services(s) performed, the completion date and time, model and serial numbers, and the name of the technician. Calibration and Preventative Maintenance Services: Upon completion of calibration equipment listed in this purchase order shall meet the original equipment manufacturers (OEM s) specifications. Services shall also include replacement of minor parts including ear cushions and tymp tubing, and OAE probe nozzles. Biomedical Engineering Dept. shall be notified if any equipment is found to be inoperable. In this case a modification shall be issued by the Contracting Officer before repairs can begin. Equipment Modification Upgrades: The contractor shall only incorporate OEM specified modifications, alterations and upgrades. A purchase order modification shall be obtained from the Contracting Officer prior to the Contractor installation of any modification, alteration or upgrades. The contractor shall maintain contact with the OEM to determine the requirement for field modifications and to ensure accomplishment of these modifications in accordance with the time schedule set forth by the OEM. (vii) Awardee shall coordinate with the Contracting Officer s Representative (COR) prior to performance. The following provisions are included as addenda to FAR 52.212-1: 852.215-70 Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors (JUL 2016) (DEVIATION) (ix) Evaluation will be determined based on Best Value to the Government IAW FAR Subpart 13.106-1(a)(2). The government shall award this procurement to the vendor with the lowest priced quote that is technically acceptable and has acceptable, or neutral, past performance. Vendors with no past performance shall be rated as neutral. Please provide a maximum 5 page capability statement that indicates the vendor has the trained personnel, equipment, and track record of providing service support for the Audiology Calibration and Maintenance Contract. Past Performance: The Government shall request the contractor provide a list of contracts completed during the past three (3) years and all contracts and subcontracts currently in process for providing service support for the Audiology Calibration and Maintenance Contract. Include the following information for each contract: a) Name and address of contracting activity b) Name and telephone number of contract person c) Dates of contract performance The Government reserves the right to make award without discussions. Except when it is determined in accordance with FAR 17.206(b) not to be in the Government s best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by using the prices offered for the last option period to determine the price for a 6-month option period, which will be added to the base and other option years to arrive at the total price. Evaluation of options will not obligate the Government to exercise the option(s). (x) The provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items applies to this acquisition. * All firms or individuals submitting a quote shall include a completed copy of this provision, OR have current Representations and Certifications in the System for Award Management (SAM - https://www.sam.gov). Representations, Certifications, and SAM must be current at award. IF NOT CURRENT AND/OR YOU DO NOT SUBMIT A COMPLETED COPY OF THIS PROVISION, YOUR QUOTE WILL BE CONSIDERED NON-RESPONSIVE. (xiv) N/A (xv) This is an open-market combined synopsis/solicitation for service support agreement for Audiology Calibration and Maintenance at the NJ VA Medical Center.    The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" The Quoter shall list exception(s) and rationale for the exception(s) Submission shall be received not later than 12:00 PM EST, Friday, November 3, 2017. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
