Sources Sought: This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 621498, Size Standard $20.5 Million. The Network Contracting Office at Syracuse (VISN 2) is seeking a potential qualified contractor to provide primary care and certain specialty care in a Capitated Community Based Outpatient Clinic (CBOC) setting in Cayuga County in the state of New York. Responses to this sources sought notice must include the following information: (1) General information about your company including name, address, contact person, phone number, fax number, email address(s), DUNS number and company website if available. (2) Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged business, Service Disabled Veteran Owned small business, Veteran Owned small business, large business. (3) Does your company have experience with the Department of Veterans Affairs capitated CBOC Program? (4) What supplies/services does your company primarily provide? (5) Will your company be a joint venture with another company? (6) How long has your company existed? Responses to this market survey should be in writing and shall be faxed (315-425-4883) to the attention of Lisa Tompkins, Contract Specialist or emailed to Lisa.tompkins@va.gov. No telephone responses will be accepted. Responses must be received no later than 3:00 pm November 10, 2017.
