Description
PERFORMANCE WORK STATEMENT DESCRIPTION OF SERVICES: Contractor shall provide all labor, tools, equipment, material, travel, parking fees, vehicles, and supervision necessary for providing snow removal, snow hauling (off campus), and de-icing services for the Lovell Federal Health Care Center (FHCC), 3001 Green Bay Road, North Chicago, IL 60064. Services are limited to Parking Lot G/11 only, in accordance with all terms, provisions and conditions herein. Services shall be performed in a manner that will maintain a satisfactory and safe environment for patients, families, staff, and visitors during periods of snow and ice. Contractor shall provide snow removal equipment to include, but not limited to, Contractor-owned trucks, plows, loaders, and salt spreaders. Contracted services are estimated to be required from approximately November 1 through April 30 for each period. Contractor shall provide a phone number and contact name of a Contractor employee(s) who shall be available on a 24/7 basis. 1.1 ACRONYMS: A. AOD Administrator On Duty B. CO Contracting Officer C. QASP Quality Assurance Surveillance Plan H. MSDS - Material Safety Data Sheets 1.2 Contractor shall remove snow haul snow off campus in Deice from all areas when there is a minimum snow accumulation of one (1) to three (3) inches. 1.3 When overnight snowfall is less than six (6) inches, the contractor shall have an area cleared no later than 6AM CST. 1.4 When daytime or overnight snowfall is greater than (6) inches, blizzard or extreme ice conditions the contractor Shall clear all areas to the best capability of his or her labor and equipment. 1.5 Snow and ice Shall be removed from all hydrants. 1.6 No snow should be plowed or placed on any landscaping or flower beds. 1.7 All sidewalks in Parkway shall be plowed. Salted, and maintained to a safe usable condition at all times. 1.8 In the event of extreme accumulation the contractor shall load in haul in deposit snow in areas on the VA campus when designated and approved by COR. 1.9 Contractors shall be responsible for any damage to concrete curbs sidewalks ramps or other when incurred during the performance of this contract 2. BASIC SERVICES: 2.1 The Contractor shall remove snow and ice from all paved areas of Parking Lot G/11, which is located behind Building 29. The parking area is approximately four (4) acres of asphalt surface. In addition to the parking lot, the area contains approximately 4,400 linear feet of perimeter and internal concrete sidewalks which shall also be cleaned. The Contractor shall monitor weather conditions for the Lovell FHCC area and provide services as needed and without notification from the COR or other Lovell FHCC personnel. 2.2 Contractor shall remove snow, haul snow off campus, and de-ice from all areas when there is a minimum snow accumulation of one (1) to three (3) inches. 2.3 When overnight snowfall is less than six (6) inches, the Contractor shall have all areas cleared no later than 6:00 AM CST. 2.4 When daytime or overnight snowfall is greater than six (6) inches, blizzard, or extreme icing conditions exist, the Contractor shall clear all areas to the best capabilities of his or her labor and equipment. 2.5 Snow and ice shall be removed from all hydrants. 2.6 No snow shall be plowed or piled onto any landscape planting beds. 2.7 All sidewalks and parkways shall be plowed, salted, and maintained to a safe and usable condition, at all times. 2.8 In the event of excessive accumulation, the Contractor shall load, haul, and deposit snow in areas on the FHCC campus when designated and approved by the COR. 2.9 Contractor shall be responsible for any damage to concrete curbs, sidewalks, ramps, or other when incurred during the performance of this contract. DESCRIPTION OF SITE: 3.1 Parking lot G/11. A map of parking lots located on the FHCC North Chicago campus is attached to this document. 3.2 Within this site, there is a mixture of pavement and sidewalks, curbs, and ramps. 3.3 Contractor shall be completely responsible for removing the snow and its disposal off the FHCC premises. All snow that is removed and required to be hauled off campus shall be accomplished at time of removal. No snow shall be stored on campus for later hauling off campus. All other snow removal shall be piled on grass areas, which will be identified with the successful contractor and the COR following award. 4. REMOVAL OF ICE: The Contractor shall immediately remove ice to provide secure footing or safe driving conditions by the snow removal operation. All salt for the areas for which the Contractor is responsible for shall be provided by the Contractor under this agreement. The successful Contractor shall be liable to ensure that adequate source of salt is readily available throughout the duration of the contract. Under no circumstances shall the contract price be increased for the Contractor s failure to obtain salt necessary to perform services. ADDITONAL SERVICES: The Contractor may be required to provide snow removal and de-icing services for areas on the Lovell FHCC campus other than Parking Lot G/11. Services will be limited to those provided under the terms and conditions of this contract. The Contractor will be notified by the COR when additional services are needed. GOVERNMENT FURNISHED PROPERTY AND SERVICES: NONE GENERAL INFORMATION: 7.1 QUALITY CONTROL: Contractor shall develop and maintain a Quality Control Plan to ensure services are performed in accordance with commonly accepted commercial practices including the proper, safe and efficient removal of the snow off the FHCC premises. Contractor shall also develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. At a minimum, the Contractor shall develop quality control procedures that address the areas identified in section 16.0, QASP. Contractor shall submit his/her QCP with their offer. 7.2 QUALITY ASSURANCE: The Government will evaluate the Contractor s performance in accordance with the QASP. 7.3 GOVERNMENT REMEDIES: The Contracting Officer shall follow FAR 52.212.4, Contract Terms and Conditions - Commercial Items, for Contractor s failure to perform satisfactory services or failure to correct non-conforming services. 7.4 HOURS OF OPERATION: A.     Contractor shall provide snow removal, snow hauling off campus and de-icing services twenty-four (24) hours per day, seven (7) days per week, including holidays at no additional cost to the VA. B. Normal business hours are 7:00 a.m. to 4:30 p.m., Monday through Friday, excluding National Holidays:   New Years Day Martin Luther King Presidents Day Labor Day Memorial Day Independence Day Columbus Day Thanksgiving Christmas Veterans Day GENERAL REQUIREMENTS: 8.1 Contractor shall perform work in a professional manner and to the highest standards of snow removal, snow hauling off sites and deicing. 8.2 Contractor shall only use chemicals for Snow Removal and Deicing that are approved by the Environmental Protection Agency (EPA) and are considered to be the safest and most effective chemicals. Contractor shall follow all OSHA standards in the performance of all tasks.   Contractor shall provide Material Safety Data Sheets (MSDS) to the COR for any chemicals used under this contract.   The product, timing and rate of all of applications will be adjusted to meet the needs of each winter season. 8.3 During non-business hours, the COR s designee (the AOD) will call the Contractor when service is needed. 8.4 During normal business hours, the COR or his/her designee will contact the Contractor when service is needed. 8.5 Contractor will request the FHCC Police to help locate owners of vehicles that need to be moved during snowplowing. FHCC Police will be contacted to help control vehicle traffic. 8.6 All snow removal, snow hauling and de-icing services shall be done at night, when possible, so that contracted areas will be cleared by the start of the workday. 8.7 After contract award, the Contractor shall provide the COR all current names and phone numbers for requesting snow removal, snow hauling and de-icing services. 8.8 Contractor shall have one (1) responsible person on the job site during the snow removal, snow hauling and/or de-icing to supervise and guide Contractor s snow crew. 8.9 Contractor shall be responsible for all fences, gates, grounds, lawns, and any FHCC property damaged during snow removal, snow hauling or de-icing. The COR will notify the Contractor of any damages after services have been performed. 8.10 Services may be required at any time after normal hours of operation as described in Paragraph 4. CHECK-IN AND KEYS:  9.1 Upon arrival at Lovell FHCC, the Contractor shall check-in with the FHCC Police before performing any snow removal, snow hauling or de-icing services. 9.2 Contractor can check out a key from the FHCC Police for entry into fenced-in areas. NOTE: Contractor shall be held responsible for all keys, which are issued for the purpose of services defined herein. It is agreed upon and understood that if Contractor loses any keys, there is a $25.00 replace ment fee per key plus any costs for the replacement of hardware/locks. Contractor shall return all keys at the request of the COR or prior release of final payment under last period of performance.  SPECIAL QUALIFICATIONS: 10.1 Offers will be considered only from Contractors who are regularly established in the snow/ice removal business and hauling snow off sites for the last three (3) current years with experience in commercial facility contracts with similar size or larger, with conditions and scope as required under this contract. 10.2 Provide a list or three (3) references from a hospital, commercial facility, or Government agency (especially other VA hospitals) where your company has performed similar duties, within the last three (3) years, for snow removal, snow hauling off sites and de-icing. Include the facility name, location, contact person who is responsible for the administration of the snow removal contract and the telephone number of each reference. PERSONNEL POLICY: 11.1 Contractor shall be responsible for protecting their personnel providing services under this contract. 11.2 The parties agree that such personnel shall not be considered FHCC or VA employees for any purpose and shall be considered employees of the Contractor. PROTECTION OF GOVERNMENT PROPERTY: During work execution, the Contractor shall take special care to protect Government property including, but not limited to, all exterior porches, turf, patio areas, walkways/pathways, vegetation and any Government property where services will be performed. Any damage must be reported to the COR immediately for appropriate action. Damage resulting from Contractor operations shall be repaired by the Contractor to the satisfaction of the COR at no additional cost to the Government. Contractor shall be responsible to meet all OSHA/Safety requirements in the performance of the work. This shall include, but not be limited to, taking all necessary precautions to protect patients, visitors and/or staff at each location. Contractor shall be held responsible for any injuries and/or damage caused as a result of the Contractor s failure to adhere to these requirements. CONFORMANCE STANDARDS: Contractor shall ensure that the equipment functions in conformance with the latest published edition of the applicable EPA, NFPA-96, UL, OSHA, VA, Joint Commission, state, local, federal and all applicable regulations for the work identified herein. THE JOINT COMMISSION REQUIREMENTS: FHCC will schedule a pre-work orientation meeting prior to the commencement of work on site, which the Contractor shall attend. This meeting will include discussions on the following topics: Fire and Safety Infection control Disaster procedures Security NOTE: FHCC will provide information to the Contractor regarding these topics and will document the meeting. B. The Contractor shall be responsible to ensure that Contractor employees coming to the work site will receive the required information above. C. The Contractor shall be responsible to ensure Contractor employees providing work on this contract are fully trained and completely competent to perform the required work. SECURITY REQUIREMENTS: Contractor s employees shall wear visible identification at all times while on the FHCC campus. It is the Contractor s responsibility to park in appropriate designated parking areas. Information on parking is available from the FHCC Police or the COR. Smoking is prohibited inside any buildings on the FHCC campus. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of FHCC regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. It is designed to provide an effective surveillance method of monitoring each listed objective. These objectives are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each objective, which are critical to mission success. The QASP is based on the premise that the Government desires to maintain a quality standard in the operation of the systems to be serviced under this agreement and to provide the service is the best means of achieving that objective. The Contractor, and not the Government, is responsible for management and quality control actions to meet the terms of the contract. The role of the Government is quality assurance to ensure contract standards are achieved. Performance Objective Performance Threshold Type of Surveillance Remove Snow and Ice. Ensure paved areas are safe for vehicle and/or pedestrian traffic. Ensure areas have adequate de-icing material for the conditions. No more than 3 customer complaints per service month. Monitored by COR Haul Snow and Ice off campus to prevent accumulation of snow. Ensure snow is hauled off campus in a safe, timely and efficient manner as required by the PWS. No more than 2 customer complaints per service month Monitor by COR thru Periodic Inspections at least Monthly 16.1 SURVEILLANCE: The Government QA (COR) will evaluate the performance objectives through periodic inspections during each service month. 16.2 STANDARD: A Notice of Violation (NOV) is not permitted. The Contractor shall perform all work required in a satisfactory manner in accordance with the PWS. The QA shall not consider the services complete until all deficiencies have been corrected. 16.3 PROCEDURES: The Government QA (COR) will inspect all performance objectives at least monthly to ensure Contractor compliance with PWS and record results of these inspections, noting the date and time of inspection. If inspection indicates unacceptable performance, notify the Contractor's Quality Control Inspector (QCI) of the deficiencies needing correction. The Contractor shall be given two hours after notification to correct the unacceptable performance. The QA may approve additional time if considered appropriate. If deficiencies are not corrected within the required time, the QA should notify the Contracting Officer for action.
