Description
Solicitation Package The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, TX, intends to issue solicitation 15B10917Q00000021 for the provision of Mobile MRI Services. The services will be provided to individuals who are under the custody and care of the Federal Bureau of Prisons within a secure correctional environment, located at the Federal Medical Center, Lexington, KY. The contract type is an indefinite delivery/requirements contract with firm-fixed unit price. The award will be made to the responsible vendor or individual who submits the lowest quote to the Government based on price related factors only. The contract periods for the service will consist of a base year and four option years with an additional 6 month optional period. The Contractor will normally conduct these services on-site 6-12 times per year; approximately every other month or once per month, based on institution need. Services in this Statement of Work are defined at a prearranged date in which the Contractor will be scheduled to perform on-site services to the inmate population at the FMC. The length of stay will normally consist of a maximum of eight hours per visit. The visits will normally be scheduled between the hours of 7:30 am and 4:00 pm, Monday through Friday, excluding federal holidays. The Contractor will be notified in advance of the requested date of service and the number of inmates to be examined. The number of inmates will be determined by the Health Service staff. These services are subject to the actual requirements and may be adjusted to meet the needs of the FMC as determined by the Health Services Administrator. Changes to any subsequent schedule must be coordinated with the COR assigned under the contract. All necessary imaging supplies such as film, contract media, archival media (optical disks) shall be provided by the contractor. Ancillary items such as linens and patient gowns that the contractor may routinely need will be provided by the Health Services Department at the FMC. Additionally, the Government will furnish all required Government forms to be used in the performance of services. All records maintained by the contractor under this contract will remain the property of the institution. Currently, the institution is not capable of providing any electrical connections or electrical power. Therefore, the contractor's mobile MRI unit shall be fully self-contained and capable of generating its own power. The contractor or individual performing on behalf of the contractor must have graduated from a two-year radiography program approved by Kentucky and hold valid certifications/licensure as required by the Commonwealth of Kentucky. The solicitation will be available on or about October 31, 2017 and will be distributed solely through the General Services Administration's Federal Business Opportunities website at www.fedbizopps.gov. All future information concerning this acquisition, including solicitation amendments, will be distributed solely through this website. Interested parties are responsible for monitoring this site to ensure you have the most up-to-date information about this acquisition. Hard copies of the solicitation will not be available. The website provides all downloading instructions to obtain a copy of the solicitation. The anticipated date for receipt of quotes is on or about November 29, 2017. Faith-Based and Community-Based organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. All contractors doing business with the Federal Government are advised that this future solicitation includes the clause FAR 52.212-5(t) for Commercial Items and System for Award Management (SAM). This clause requires all contractors doing business with the federal government after September 30, 2003, to be registered in the SAM database at www.sam.gov. Contractors should include their Data Universal Number System (DUNS) number in their offers. The Contracting Officer will verify registration in the System for Award Management (SAM) database prior to award by entering the potential awardees DUNS number into the SAM database. The acquisition is restricted to Total Small Businesses. The North American Industrial Classification (NAICS) code is 621512 with a small business size standard of $15 million.
