Description
Statement of Work The Navajo Area Indian Health Services, Gallup Indian Medical Center (GIMC), Acquisitions Department has a requirement for Preventative maintenance services for the anesthesia machines located at Gallup Indian Medical Center, Gallup, New Mexico. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation for this acquisition is IHS1338948. The solicitation is issued as a request for quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. This solicitation document and its incorporated provisions are those in effect through the Federal Acquisition Circular (FAC) 2005-95, effective January 19, 2017. The North American Industry Classification System (NAICS) Code for this acquisition is 811219 - Other Electronic and Precision Equipment Repair and Maintenance with a business size standard of $20.5 million. All responsible sources may submit an offer that will be considered by the Agency. The Defense Priorities and Allocations System (DPAS) and assigned rating does not apply to this requirement. All responsible Offerors shall submit the following NO LATER THAN 3:00 PM MST, November 13, 2017. Fax submissions are not authorized. A. Fee Schedule a. Total Cost of Fee Schedule b. Responses to Evaluation Factors Offerors must submit representations and certifications in accordance with the provision at FAR 52.212-3 Offeror Representations and Certifications - Commercial Items. Responsible sources must be registered and active in the System for Award Management (SAM) database, website accessed through http://www.acquisition.gov. METHOD OF AWARD The Government anticipates awarding a firm fixed price award to the Offeror whose quote is the most advantageous to the Government, price and other factors considered. Technical quotes will be evaluated based on the factors described below. Award will be made to the Offeror whose quote is determined be the best value for the Government. Quote shall set forth full, accurate, and complete information as required by this solicitation package (including Attachments). FEE SCHEDULE Offeror shall indicate below the Total Cost to perform stated services, in accordance with the Statement of Work. Total Cost shall be inclusive of labor, travel, per diem, tools, equipment, technical, materials, supplies and federal/state/local taxes and all other necessary expenses to perform Preventative Maintenance (PM) and unscheduled maintenance in accordance with the attached Statement of Work. CLIN# Description Total Cost 001 Base Year - One Year of Preventative Maintenance for $ Anesthesia Machines/Equipment 002 Option Year 1 - One Year of Preventative Maintenance for $ Anesthesia Machines/Equipment 003 Option Year 2 - One Year of Preventative Maintenance $ Anesthesia Machines/Equipment 004 Option Year 3 - One Year of Preventative Maintenance for $ Anesthesia Machines/Equipment 005 Option Year 4 - One Year of Preventative Maintenance for $ Anesthesia Machines/Equipment PERIOD OF PERFORMANCE The period of performance will be for one (1) base year with four (4) one-year option periods: Base year: December 1, 2017 to November 30, 2018 First Year Option: December 1, 2018 to November 30, 2019 Second Year Option: December 1, 2019 to November 30, 2020 Third Year Option: December 1, 2020 to November 30, 2021 Fourth Year Option: December 1, 2021 to November 30, 2022 EVALUATION FACTORS The government intends to award one firm fixed-price award to the Offeror whose offer will be most advantageous to the Government, considering pricing and other factors. (b) The Contractor shall account for all forms of Government-provided identification issued to the Contractor employees in connection with performance under this contract. The Contractor shall return such identification to the issuing agency at the earliest of any of the following, unless otherwise determined by the Government; (1) When no longer needed for contract performance. (2) Upon completion of the Contractor employee's employment. (3) Upon contract completion or termination. (c) The Contracting Officer may delay final payment under a contract if the Contractor fails to comply with these requirements. (d) The Contractor shall insert the substance of clause, including this paragraph (d), in all subcontracts when the subcontractor's employees are required to have routine physical access to a Federally-controlled facility and/or routine access to a Federally-controlled information system. It shall be the responsibility of the prime Contractor to return such identification to the issuing agency in accordance with the terms set forth in paragraph (b) of this section, unless otherwise approved in writing by the Contracting Officer.
