Description
Request for Information (RFI) For Lifeguard and Certified Pool Operator (CPO) Services Purpose: The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business). Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, proof of applicable certification. DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in System for Award Management (SAM), formerly CCR (see internet site: http://www.sam.gov). The applicable North American Industry Classification System (NAICS) Code for this acquisition is 713940 with a Size Standard: SMALL. Summary: The Department of Veterans Affairs (VA), Bronx, NY Contracting department is requesting white papers providing the entire requirement discussed below. STATEMENT OF WORK Certified American Red Cross Lifeguard Services And Certified Pool Operator (CPO) Introduction and Overview for Lifeguard Services Therapeutic Pool Certified American Red Cross Lifeguard services shall be furnished to the VA Medical Service, 79 Middleville Road, Northport, NY 11768 to assure a safe environment. The primary function of the swimming pool (Building 88) is for patient use during clinical and/or self-directed scheduled programs. Background VAMC Northport Recreation Therapy Services (RTS) utilizes the swimming pool to implement the following clinics and maintenance sessions: Aquatic Therapy clinic: Provided by Recreation Therapy Service to assist in rehabilitation, increase muscular flexibility, aerobic endurance, weight reduction, pain management, and stress reduction. Maintenance Program: Provides an opportunity for patients to enhance their physical wellbeing as well as promote their social skills. Scope of Work The scope of work is to acquire a contacting agent to provide lifeguards to assure a safe environment for the usage of the Northport Veterans Affairs Medical Center therapeutic swimming pool. The primary function of the swimming pool (Building 88) is for patient use during clinical and/or self-directed scheduled programs. The following responsibilities shall be adhered to: Knowledgeable of the VAMC Safety plan. Hold a current American Red Cross (ARC)Lifeguarding/First Aid/CPR/AED certification and provide copy to RTS Office A minimum age of 16 years old Knowledge of Pool Operations in general to include but not limited to: Water Chemistry Check Temperature Settings Pool Equipment Operation LIFEGUARD DUTIES AND RESPONSIBILITIES TO INCLUDE BUT ARE NOT LIMITED TO: Assigned lifeguard comes to bldg. 6 room 245 to sign in and obtain the key to bldg. 88. Please note that the lifeguard will need to arrive 30 minutes prior to the start of each session and remain in the area until all Veterans have left the locker room and pool area. Lifeguard goes to Bldg. 88, open s the hallway, locker room and inspects area prior to allowing Veterans access and unlocks pool doors. After clinic is over and ALL Veterans are out of the locker room, Lifeguard must LOCK all doors to locker room, pool, and pool hallway. Upon completion of program, assigned lifeguard comes to bldg. 6 room 245 to sign out and return the key. * Contractor will provide a monthly invoice for services which will be compared to VAMC Lifeguard sign in sheets. RTS will fax monthly sign in sheet to contractor at the end of the month. Checking of Pool Chemistry every hour and record in log book provided by VAMC located on table in pool area If there is a change in the Pool Chemical additives, the lifeguard is to notify appropriate personnel in Recreation Therapy service If the Chemistry has to be changed, the Pool will be closed until the task is complete. Lifeguard does not change the chemistry. Ensures Proper Storage and Care of equipment; i.e. vests, floatation devices, etc. Under the indirect supervision of Northport, Recreation Therapy Service, Chief, and/or Assistant Chief, Recreation Therapy Service (RTS). All Veteran participants MUST be checked in and verified, from the weekly list provided by RTS, that they are medically cleared to engage in this activity. All lifeguard staff will need to wear appropriate swim wear. Ensure that all water safety rules are observed by swimmers, utilizing a good knowledge of all aspects of water safety procedures. Perform rescue work and when necessary render first aid until such time as emergency personnel is present, and complete appropriate accident reports. Ensure that all swimmers in the water are accounted for at all times. Contact RTS supervisors if there is any incident or accident to report the issue. Lifeguards shall ensure all rules and regulations pertaining to the pool are observed. Remove patrons that do not obey pool rules/regulations. All lifeguards shall be able to execute emergency action plans. Lifeguards shall handle inquiries or complaints from Veterans in a diplomatic and professional manner. LIFEGUARD WORKDAYS/HOURS: Schedule will vary to be between 5 to 7 hours per day from Monday through Friday, excluding Holidays. Exact schedule will be provided but may change according to service scheduling of patients. The following is the proposed schedule of programs. Please note that the lifeguard services will be needed 30 minutes prior and after each program. Monday Maintenance Program 9:00-11:00am Recreation Aquatic Therapy Program 1:00-4:00pm   Tuesday     Recreation Aquatic Therapy Program 10:00-11:00am Maintenance Program 1:00-2:00pm Recreation Aquatic Therapy Program 2:00-3:00pm   Wednesday Maintenance Program 9:00-11:00am Recreation Aquatic Therapy Program 1:00-4:00pm   Thursday     Recreation Aquatic Therapy Program 10:00-11:00am Maintenance Program 1:00-2:00pm Recreation Aquatic Therapy Program 2:00-3:00pm Kayaking program 3:00pm to 4:00pm Friday Maintenance Program 9:00-11:00am Recreation Aquatic Therapy Program 1:00-4:00pm   REPORTING REQUIREMENTS: Daily, Lifeguard shall report to RTS office located in bldg. 6, room 245 to sign in and obtain the key at the start of their shift. This check-in is mandatory for certification of invoice for payment. Objectives The primary objective for the contracting agent is to provide a Certified American Red Cross Lifeguards. Place of Performance Northport VA Medical Center, 79 Middleville Road, Northport NY 11768. Building 88, 35 X 60 Therapeutic swimming pool. Type of contract: Indefinite Quantity, base plus 4 option-year contract. Hours of contracted services required are accurate but may differ weekly depending on unexpected pool maintenance issues and/or program scheduling conflicts. Scope of Work The scope of work is to acquire a contacting agent to provide Certified American Red Cross Lifeguards to assure a safe environment for the usage of the Northport Veterans Affairs Medical Center therapeutic swimming pool. Government Furnished Equipment First aid kit, elevated lifeguard chairs, several rescue tubes, backboard with head/neck stabilizers, crock, and AED. Security All contractors will be approved. Introduction and Overview for Certified Pool Operator (CPO) Certified Pool Operator (CPO) services shall be furnished to the VA Medical Service, 79 Middleville Road, Northport, NY 11768 to assure a sanitary, healthful and safe environment for the usage of the Northport Veterans Affairs Medical Center therapeutic swimming pool. The primary function of the swimming pool (Building 88) is for patient use during clinical and/or self-directed scheduled programs. Scope of Work The scope of work is to acquire a contacting agent to provide a CPO to assure a sanitary, healthful and safe environment. The contractor will provide a clean and sanitary environment for patrons utilizing the swimming pool. The CPO will be responsible to provide the following tasks Monday through Saturday, 52 weeks per year with the exception of all federal holidays: Basic Maintenance Services: The pool is a community-sized pool with the following water capacity: Recreational Pool, Bldg. 88 (35 X 60 Therapeutic swimming pool, 80,000 gallons). The contractor shall maintain the swimming pool in accordance with this statement of work and accepted commercial practices. The contractor shall ensure water quality continues to meet or exceed the New York Department of Health, Part 6, Subpart 6-1 Swimming Pools standards. Corrective action shall be performed to correct unscheduled deficiencies noted on systems. The contractor shall prepare and maintain historical records of appropriate sampling and analysis to ensure compliance with all regulatory requirements. The contractor shall make available all records of services to government personnel upon request. The contractor shall advise the Chief, Recreation Therapy Service (RTS) or designee of planned water system shut downs, at least 7 days in advance, and shall immediately advise of any pool system condition that will adversely affect customer service. The swimming pool shall be maintained and operated in a clean, safe and sanitary manner at all times, checked daily Monday through Saturday. Check and monitor the pool level and ensure that the overflow system shall be maintained to continuously remove floating matter and surface water. The pool walls and bottom shall be vacuumed or brushed daily or as needed, at least one time per week, to remove visible settle able material. Pool deck will be rinsed down daily for sanitization using the sanitizing system the VA provides Monitor and maintain a recirculation system which includes filtration and disinfection facilities to provide water quality consistent with the bacteriological, chemical and physical standards required by the NYS Department of Health. The recirculation system shall be backwashed at a flow rate of 12 to 15 gpm/sf or at the design rate recommended by the manufacturer at least 1 time per week. Maintain appropriate chlorine and PH levels in accordance with NYS Department of Health guidelines daily. Swimming pool water temperature will be adjusted daily to be within 88-92 degree s. CPO will contact RTS office at extension 7275 to report temperatures outside of this range. Microbiological samples shall be collected from swimming pool daily to evaluate water quality. Complete daily operational records (chlorine, pH levels, and temperature) in the operators log book. Cracks in the pool wall, floor, perimeter, overflow systems and decks will be reported to RTS office at extension 7275 when determined to be potential leakage or slip hazards. Water Sampling: An analysis and sampling function shall be developed and operated to comply with Government and regulatory agency requirements. Tests shall be accomplished in accordance with applicable conditions, procedures, and commercial practices. Records of all tests shall be maintained which include, the name and type of test, date and time, final test results, and employee name/number who performed the test Corrective Maintenance: Corrective maintenance includes adjustments or service, required to return water quality to proper level. Any required repair of hardware identified by the contractor shall be immediately reported to Chief RTS or designee. Chemicals: Starting with contract award, the contractor is not responsible for supplying chemicals required to maintain pool water quality. Contractor Qualifications: The contractor shall be bonded, licensed, and properly insured as a pool operator with a minimum of 5 verifiable years of experience maintaining Chlorine water treatment methods. Government Quality Assurance: The government will regularly evaluate the contractor s performance to ensure compliance with contract terms and conditions. Hours of Operation: The hours of operation for the recreational swimming pool are subject to minor modifications. The contractor shall be notified by RTS of any changes to the hours of operation. Maintenance will continue at the normal rate throughout the year. The pool is closed for all federal Holidays. Day Open Close Monday 9:00am 9:00pm Tuesday 9:00am 9:00pm Wednesday 9:00am 9:00pm Thursday 9:00am 9:00pm Friday 9:00am 7:00pm Saturday 9:00am 5:00pm Sunday 9:00am 5:00pm Objectives: The primary objective for the contracting agent is to provide a Certified Pool Operator (CPO) to perform the duties to maintain a clean and sanitary environment. To have a swimming pool that is managed by a CPO and to meet the NYS Department of Health guidelines. Place of Performance: Northport VA Medical Center, 79 Middleville Road, Northport NY 11768. Building 88. Contractor Point of Contact: The contractor shall provide the name of a point of contact and an alternate that shall be responsible for the performance of services. The contractor point of contact shall be available via telephone for service requests and discussion of maintenance issues with the designated Chief, RTS or designee. The contractor shall designate these individuals with telephone numbers, in writing, to the Contracting Officer (CO) before the contract start date. Emergency Services: The government will provide fire and police services. The contractor shall dial 2555 for emergency medical services. Security: All contractors will be approved and issued an ID badge to be worn at all times. Contractor employees shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured. Work Area: The contractor shall, at all times, keep work areas free from accumulations of waste material, rubbish, tools, scaffolding, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition. The contractor is responsible for removal and proper disposal of all job related waste material at a permitted site. Safety: The contractor shall comply with all applicable laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the government will assume no liability or responsibility for the contractor s compliance or noncompliance with such responsibilities.
