- Foreign Vendor Information Sheet Combined Synopsis Solicitation PDF Brand Name JA CJTF HOA-REEFER Request for Quote (RFQ) 11/07/2017 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. CJTF-HOA/CCO is seeking the following for delivery in accordance with the included Statement of Work (SOW). This will be a firm-fixed price contract awarded to the lowest price technically acceptable. All quotes must be in USD. 1. All Completed quotes must be submitted to MSgt Ostromogilsky, Maxim via email at maxim.ostromogilsky.mil@mail.mil. All offers are due to the responsible Contracting Office no later than Friday, 17 Nov 2017 @ 08:00 AM local time. Quotes may not be accepted past the aforementioned date & time specified. 2. The U. S. Government intends to award a Firm Fixed Price purchase order to the successful offeror of this RFQ. Award will be made to the responsible offeror whose offer conforming to the request for quote will be most advantageous to the Government, price and other factors considered. All quotes must comply as follows: a. All quotes shall be evaluated on the ability to meet required specifications, delivery, and cost. A quote that is so lacking in information, that a proper evaluation would require elaborate explanation, may be deemed unacceptable. b. Quotes must confirm that the offered items and/or services are in accordance with the specifications/statement of work (SOW) as outlined within this RFQ. c. The offeror shall submit a quotation that includes all prices for the listed items/services and the delivery dates for each item/service. 3. To be eligible for award under this RFQ, contractors must respond to this RFQ by the closing date and time as referenced above. Failure to submit the required information may result in your quotation being considered non-responsive. 4. The Government intends to make one aggregate award resulting from the RFQ but reserves the right to make multiple awards without discussions. The Government will determine the awardee based on the quote that provides the overall "Lowest Price Technically Acceptable." To be found technically acceptable, vendor must meet all the requirements in the Statement of Work. Quotes shall be evaluated based on price, technical acceptability, delivery. 5. The Contractor shall quote the requested items as detailed below in US Dollars (USD). 6. SCREENING AND VETTING The vendor is required to submit the Foreign Vendor Information Sheet with the quote. Before issuing contract award or at the time of award all contractors must provide completed company vetting information to include company vetting information for all subcontractors in which supplies and services will be used to execute this contract. At any time during the vetting process, if contractors, or subcontractors are found to have any involvement and/or connection with violent extremist organizations(s) the contract will be terminated immediately. Any cost incurred by the contractor or subcontractor as a result of contract award or contract termination will not be paid by the US government. 7. L. 113-235), Government agencies are not permitted to use funds appropriated (or otherwise made available) under that or any other Act for contracts with an entity that requires employees or subcontractors of such entity seeking to report fraud, waste, or abuse to sign internal confidentiality agreements or statements prohibiting or otherwise restricting such employees or contractors from lawfully reporting such waste, fraud, or abuse to a designated investigative or law enforcement representative of a Federal department or agency authorized to receive such information. (b) The prohibition in paragraph (a) of this provision does not contravene requirements applicable to Standard Form 312, Form 4414, or any other form issued by a Federal department or agency governing the nondisclosure of classified information. (c) Representation. By submission of its offer, the Offeror represents that it does not require employees or subcontractors of such entity seeking to report fraud, waste, or abuse to sign or comply with internal confidentiality agreements or statements prohibiting or otherwise restricting such employees or contractors from lawfully reporting such waste, fraud, or abuse to a designated investigative or law enforcement representative of a Federal department or agency authorized to receive such information.
