- Statement of Work Veterans Benefits Administration Louisville Regional Office 321 West Main Street, Suite 390 Louisville, KY 40202 (502) 566-4344 Service: Department of Veterans Benefits Administration Louisville Regional Office Service Period: November 27, 2017 through May 26, 2017 Scope: This is a request for court reporting transcription services for Regional Office (RO) Louisville, KY. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. Contractor will provide court reporter services upon request from RO within 5 days of notification with exception of first task (see paragraph 9). Performance Requirements: Vendor will provide transcription services and be competent to administer oaths. Place of Performance: To Be Determined on an as needed basis except first task. Description of Work & Contractor Responsibility: Contractor will be available on Monday through Friday, beginning at 8:30 AM EST until 5:00 PM EST to transcribe testimony. A one hour lunch period and intermittent breaks will be provided each day. Dress should be professional. Equipment for this service will be provided by the contractor. In the event of a mechanical breakdown of the transcribing equipment or as a result of negligence by the contractor or any subcontractor, the contractor is required to provide the services necessary to retake the testimony at no additional charge to the Department of Veterans Affairs. The contractor shall bear all transportation and subsistence costs. It is permissible to use a different reporter each day. The contractor shall furnish complete transcripts, which accurately reflect the full and complete verbatim record of the proceedings. If the presiding official determines there are errors in the transcript, the contractor shall be required to correct the errors and furnish the corrected transcript within five (5) calendar days after receipt of notification at no additional cost to the Government for the same, regardless of the delivery time the original order specified. If one or more copies of transcripts are found to be illegible, the contractor shall correct and replace the same with acceptable copies within five (5) business days after receipt of notification for the presiding official at no additional cost for such replacement. At the conclusion of the hearing, the hard copy documents will be provided to the Regional Office within five (5) working days. One (1) transcribed hardcopy is required and two (2) transcribed electronic copies in the form of a CD or DVD. Documents will either be hand delivered (preferred) or sent by certified mail to allow for tracking. Transcripts shall be standard formatting. All information during this hearing will be treated and handled as extremely confidential. Funding: Contractor will notify RO when funding reaches 80% of the Not to Exceed (NTE) Amount and before it reaches 90%. Place of Performance: EEOC Office, 101 W. Ohio St. Suite 1900, Indianapolis, Indiana 46204. First task: November 28 29, 2017. Contractor will be available on Tuesday and Wednesday, beginning at 8:30 AM EST until 5:00 PM EST to transcribe testimony in the EEO Hearing at EEOC Office located at 101 W. Ohio St. Suite 1900, Indianapolis, Indiana in accordance with paragraph 4.
