- THIS IS NOT A SOLICITATION. In accordance with FAR Part 10, Market Research, this is a sources sought to determine the availability of potential sources/vendors having the skills and capabilities necessary to provide temporary housing for homeless veterans and their families services, for the Department of Veterans Affairs Medical Center located at 3200 Vine Street, Cincinnati, Ohio 45220. All interested contractors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. This sources sought notice is for information and planning purposes only, and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). The VA is currently conducting market research to locate qualified, experienced and interested potential sources relative to NAICS 624221. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this sources sought announcement. General Requirements: The Cincinnati Veterans Affairs Medical Center (VAMC) located in Cincinnati, Ohio. requires Contractors to provide services as part of its Community Based Health Care for Homeless Veterans (HCHV) program. The goal of the HCHV program is to remove homeless Veteran families from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient supportive services to meet their needs and ultimately facilitate the improvement of their housing situation. Veteran families may be comprised of one or more household members. The purpose of this request for information is to obtain interest from Contractors facilities located within 20 miles from the Cincinnati VAMC for services delivered in Hamilton County, Ohio; Campbell County, Kentucky and/or Kenton County, Kentucky who can provide housing, and supportive services to Homeless Veteran families in community-based Contracted Emergency Residential Services (CERS) facilities offering a safe and secure environment that supports their goals for recovery from homelessness. Future awards may be based on residential capacity to place Veterans in safe, community-based residences. Future multiple awards may be considered. Maximum number of beds funded will not exceed 4380 bed days of care per year. The Contractor will be required to provide a low- barrier therapeutic and rehabilitative milieu and attendant services targeting the underlying factors contributing to homelessness. The Contractor will not be required to provide detoxification or other hospital level treatment. Special emphasis on privacy and security must be noted in mixed gender and multifamily facilities. The living environments of HCHV Contract housing must include: Separate and secure sleeping arrangements (unit or wing) for each gender or Veteran Family (separate sleeping arrangements must be available in the unit for children of opposite sex that are 12 years or older), Separate and secure bathroom arrangements for each gender or each Veteran family, Screening procedures and criteria for sex offenders (mixed gender facility), and Common areas (lounges, laundry rooms, group rooms) with appropriate security arrangements. Additionally, mixed gender, multifamily facilities that share entrance and common areas (living room, kitchen, dayroom, etc.) must include onsite staffing available 24 hours a day, 7 days per week. Requested submittal Information: Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response of no more than 10 pages in length, single spaced, 12 point font minimum. The Government will not review any other data or attachments that are in excess of the 10 pages. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill these requirements. Interested parties shall furnish the following minimum information: Company name and address. Company point of contact name, telephone number and email address. Is your business large or small? If so, please provide. If not, you must register with Dun & Bradstreet at: http://www.dnb.com Is your firm registered with the System for Award Management (SAM) at https://www.sam.gov/portal/SAM/#1? If not, please register as soon as possible. If the company holds a Federal Supply Schedule (FSS) Contract, list the GSA Contract Number and relevant SINS. Does your firm have a website address? Please provide. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills your company possesses to perform services described in the scope of work. Do you need to subcontract any portion of work to provide these services stated herein? Past Experience: Brief summary of the company history relative to similar requirements. Offer may provide the following information on a maximum of three similar projects for which the responder was a prime or subcontractor. The name, address, and value of each project. The Prime Contract Type, Firm Fixed-Price, or Time and Material. The name, telephone and address of the owner of each project. A description of each project and why it is relevant to this requirement. Include difficulties and successes. Your company s role and services provided for each project.
