- TO: Industry FROM: Contact Specialist RE: Sources Sought posted on FBO ATTENTION: This is a Sources Sought ONLY The United States Department of Veterans Affairs, Providence Health Care System, Providence, RI, is seeking information pursuant to the Statement of Work (SOW) contained within this document. Namely, we are gathering information on possible vendors who would be able to fulfill the enclosed requirement description. Specifically, we are seeking VetBiz Certified SERVICE DISABLED VETEREN OWNED SMALL BUSINESS (SDVOSB), VetBiz Certified VETEREN OWNED SMALL BUSINESS (VOSB), and SMALL BUSINESS (SB) vendors but are open to information and responses from ALL vendors as well. Potential vendors must provide the following information on the ability to meet the requirements: The number of contracts with similar scope and size. State the number of presently trained technicians that are qualified to complete the required service. This Request for Information may be followed by a related solicitation for this same facility with a substantially similar SOW. GENERAL The Contractor shall furnish preventive maintenance and repair services for Medtronics Orthorhino Fusion ENT System. 2. STATEMENT OF WORK The contractor will do preventive maintenance (PM), performance evaluation and calibration with parts as needed for the PM. The contractor shall perform monthly preventive maintenance inspections annually in accordance with the approved PM Schedule. HOURS OF OPERATION All service/repairs will be performed during normal working hours, Monday through Friday from 8:00 a.m. to 5:00 p.m. except Federal Holidays listed on Section 4, paragraph 4.1.4. UNSCHEDULED MAINTENANCE/REPAIRS Contractor shall maintain the equipment in accordance with the Conformance Standards. Service Calls are unlimited for servicing of equipment due to malfunctioning or inoperative condition requiring repairs and/or replacement of parts in order to return equipment to first class operating condition. Contractor shall provide a 24-hour telephone number. Contractor shall report on site at VAMC West Roxbury within two (2) hours. Contractor shall complete the necessary emergency services as expeditiously as possible, normally within the same 8-hour workday as the notification is received, but not longer than twenty-four (24) hours from time of notification. Emergency is defined as service or maintenance provided during other than regular business hours. 2.5 SCHEDULE MAINTENANCE Contractor shall maintain the equipment in accordance with Conformance Standards. The Contractor shall provide, within fifteen (15) days after notification of award, but not later than the performance of the first servicing of the equipment, one (1) copy of its Preventive Maintenance Procedures to be used during the PM services of the contract. Contractor shall complete the scheduled PM within two (2) working days. These procedures are required by VA as a condition of the facility's JCAHO Accreditation and MUST be received before the invoice can be certified for payment. PM service shall include as a minimum the following: Cleaning of the equipment. Reviewing operating system to ensure that the system is operating to the manufacturer s specifications. Inspecting, calibrating and lubricating the equipment. Leak checks according to manufacturer s service manuals. Perform remedial maintenance of non-emergent nature. Testing and replacing faulty and worn parts or parts which are likely to become faulty, fail or worn. Inspecting all components as necessary. Measuring, adjusting and calibrating as necessary. Inspecting and replacing where indicated, electrical wiring and cables for wear and fraying. Inspecting and replacing where indicated, all mechanical components. Returning the equipment to operating condition as defined. Provide documentation of services performed. 2.6 REPLACEMENT PARTS No part shall be utilized for any purpose other than for which it has been designed and manufactured and shall be completely compatible with the existing equipment. Only new standard parts shall be furnished in their performance of this contract. All parts shall be of current manufacture and shall have full versatility with the presently installed equipment and systems. All parts replaced and new parts become the property of VA and will be retained by VA. All new parts furnished and parts replaced are subject to inspection by the POC or his/her designee. Whenever the equipment utilizes the software provided by the manufacturer for operating, diagnostics, or maintenance, the Contractor shall provide all software upgrades, which shall become the property of VA. Documentation of proof of accessibility and license to distribute software and upgrades shall be provided the Contracting Officer upon request. VAMC WILL ISSUE SEPARATE PURCHASE ORDERS FOR PARTS AND/OR SERVICES BEYOND THE SCOPE OF THE STATEMENT OF WORK. OFFEROR SHALL SUBMIT A DETAILED INVENTORY OF THE MOST USED PARTS OF THE EQUIPMENT. 2.7 SERVICES BEYOND THE CONTRACT SCOPE Only the services specified by this agreement are authorized. Contractor shall immediately notify to the Contracting Officer (CO) and POC, in writing, no later than 24 four hours any work required beyond the contract scope. The report must provide an estimate of the cost of repairs prior to begin services. Contractor must wait for authorization from the CO to perform services. Contractor is cautioned that only the Contracting Officer may authorize additional services and that reimbursement will not be made unless prior authorization is obtained for such services. SERVICE MANUALS The VAMC will not provide service manuals to the Contractor. The Contractor shall obtain, have on file and make available to their service/maintenance personnel the equipment service manuals. The location and listing of the service data manuals, by name, and/or the manuals shall be provided to the CO and POC upon request. 2.9 SERVICE REPORTING REQUIREMENTS The Contractor is required to contact the Point of Contact (POC) upon arrival at VA Medical Center. After working hours and weekends, Contractor must report to: _____________________________________located at the ____________________________________, tel. Number: _________ No after duty hours or weekend services will be provided at _________. This check-in is MANDATORY. After all work is completed, the Contractor must submit a report of services rendered to the POC or his/her designee detailing the services performed, including a summary of repairs and a listing of replacement parts, when applicable. The reports shall also include all of Contractor's recommendations necessary to maintain the equipment in first class operating condition. 2.10 WORK CONTROL All maintenance and repair shall be fully warranted against defects in the products and replacing defective parts for 30 days and/or workmanship 60 days. Contractor shall ensure that all maintenance and repair is given the priority and manpower to accomplish all assigned work within the allowed time. Lack of parts, materials, supplies and equipment which delay the service shall not be an acceptable cause for non-performance. 2.11 CONDITION OF THE EQUIPMENT: The Contractor accepts responsibility of the equipment in as is condition. Failure to inspect the equipment prior will not relieve the Contractor from performance of the requirements of this equipment. COMPETENCY OF PERSONNEL Contractor personnel must be fully qualified/trained on the equipment to be serviced. The contractor personnel must have two (2) years of experience on scheduled/unscheduled maintenance and repairs of equipment. The POC may authenticate the training requirements, request training certificates or credentials at any time. The CO and POC reserve the right to reject any of the contractor personnel. The Contractor must provide written assurance of the competency of his/her personnel. CERTIFICATION OF COMPETENCY We hereby certify Mr./Mrs./Ms. ____________________ meets all the qualifications requirements for the position of___________________________, is properly trained, has all described in the statement of work and that we maintain employee records updated with the following: health annual certificates, good conduct report from the Boston Police Department, educational documents, job descriptions, evaluation methods and qualification standards. _____________________________ __________ Service/Operation Manager Date 2.13 TEST EQUIPMENT Prior to commencement of work the Contractor shall provide the POC a copy of the current calibration certification of all equipment listed in the Schedule. This certification will be performed and provided on a periodic basis. Test equipment calibration shall be in accordance with industry standards. 2.14 IDENTIFICATION, PARKING, SMOKING AND VAMC REGULATIONS The Contractor s maintenance/service personnel shall wear visible identification at all times while working on the premises of the VAMC. It is the responsibility of the Contractor to park in the designated areas. Information on parking is available from the VA Police Section. The VAMC will not invalidate or reimburse for parking violations. Smoking is prohibited inside the buildings. Possession of weapons is prohibited. Any violation to these rules may result in citation in the United States (Federal) District Court, not a local district, state, or municipal court. 2.15 SAFETY REQUIREMENTS In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer or his/her designee may determine to be reasonably necessary to protect the lives and the health of the occupants of any building. The Contracting Officer or his/her designee will notify the Contractor of any noncompliance with the foregoing provisions and the action to be taken. After receipt of such notice, the Contractor or his/her representative, at the site of work, shall be deemed sufficient for the purposes aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue a stop-work order in accordance with FAR 52.212-4, Contract Terms and Conditions Commercial Items, for all or any part of the work and hold the Contractor in default as provided elsewhere in this contract. 2.16 HAZARDOUS MATERIALS Contractor shall be responsible for the proper disposal of all hazardous materials. Contractor shall provide copies of all Manifests and or Billing of Lading related to the disposal of hazardous wastes generated during the performance of the contract. 2.17 MATERIAL SAFETY DATA SHEETS (MSDS) Contractor shall have available MSDS for all chemicals brought into VAMC West Roxbury site. 3. LOCKOUT/TAGOUT PROGRAM 3.1. Contractor shall provide copy of the lockout/tag-out safety program for review and acceptance.
