- Draft PWS Vendor Information Sheet to return to POC. Request for Information (RFI) ACCREDITATION OF ARMY PREVENTIVE MEDICINE DEPARTMENTS U.S. Army Public Health Center (APHC) NAICS: 541690 General Information: This is a request for information for planning purposes only. This is an RFI is to ascertain whether a commercial service exists and to seek industry input from all sources (small and large businesses). Small business concerns are highly encouraged to respond to this request in order for the Government to determine if this acquisition should be set aside for small business participation only. The results of this market research will contribute to determining the method of procurement. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 541690 (Other Scientific and Technical Consulting Services) with a size standard of $15 million. Interested parties must be registered under NAICS 541690 in the System for Award Management (SAM) at www.sam.gov. The Government plans to consider all information submitted in response to this Sources Sought notice. All information received in response to this RFI will be treated as business confidential information. Nonetheless, submitters should properly mark their responses if the information is confidential. Responses to the RFI will not be returned. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expense associated with responding to this RFI. Scope: The accrediting body selected for this contract must provide accreditation of select Army PM Departments, through the assessment of their public health activities and capacities at the installation level utilizing performance standards that are psychometrically tested and validated and a process to measure the department's performance against these standards. RFI Response and due date: 28 November 2017, 9:00 AM EST Instructions to Responders: Responses from all capable and qualified sources are encouraged to respond to this request. White papers should adhere to the following formatting and outline instructions: 1. All submissions shall be made electronically and be in one of the following formats: Microsoft Word or Adobe PDF. 2. Cover Page (1page) • Title • Organization • Responders technical and administrative points of contact (names, addresses, phone, fax number and email addresses) • Topic area(s) addressed 3. Complete Attachment 1. Please note that the response shall provide sufficient technical data to determine how you fully meet the requirements as specified in Attachment 2 Performance Work Statement (PWS). Please note that responses are due no later than 28 November 2017, 9:00AM EST. The preferred method of submission is via email.
