- SF-18: NN-18-IHS1339991 This is a solicitation for a Rapid Thermal Infuser in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. Northern Navajo Medical Center is requesting proposals from this solicitation. A written solicitation will not be issued. This is a Brand Name or Equal Solicitation for proposal. DESCRIPTION OF EQUIPMENT: ThermaCor Model 1200 Rapid Thermal Infuser: Quantity = 4 Each DNC-1200, Cassette, Dual patient Line, 1 (4/box) - 4 Each PNC-1200, Cassette, 6' patient Line, 1 (4/box) - 4 Each PTC-1200, Cassette, new Infusion Triple Line, 1 (4/box) - 4 Each Equipment is rapid thermal infusion system and must meet the following salient characteristics: -Shall have a one-step setup cassette process - Shall be available for transport use for 2 hours minimum or better - Maximum fluid warming temperature shall be 45 degrees Celsius or better - Maximum flow shall be 1200 ml/min or better - Shall be capable of maintain heating and full flow rate for 30 minutes or better - Maximum size shall be W8.75" x H16.5" x D13" or better - Shall have pressure controlled flow with the option of 2 settings minimum, ranging from 100 mmHg to 300mmHg - Shall allow the administration of programmed boluses and automatically return to the previous flow rate once the bolus is complete The solicitation number for this procurement is NN-18-IHS1339991. The North American Industry Classification System (NAICS) is 334510 - (Electromedical And Electrotherapeutic Apparatus Manufacturing). The Government intends to evaluate proposals based on the following evaluation factors: technical capabilities and meets specifications; past performance (within the last 3 years) of similar items provided to Federal Government Agencies); and Warranty provisions. The government will award a contract resulting from the solicitation to the responsible offeror whose offer is conforming to the quote is most advantageous to the Government, evaluation factors, best value and price considered. The Government intends to award one contract resulting of this solicitation. An award will be based on the requirement and evaluation factors. If there is a tie, the tie breaker will be the price. To be considered for an award the documents that are relative to the evaluation factor shall be included as an attachment with the submission of the quote. SUBMISSION OF OFFERS: Submit signed and date offers to the email specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the attached SF-18, a letterhead stationary or company's quotation form as specified in this solicitation. The following information shall be on the proposal: (1) The solicitation number; (2) Name, address, email and telephone number of the offeror; (3) Technical description of the items offered in sufficient detail to evaluate compliance with requirements in the solicitation. This shall include the company literature or brochure or other documents related to the items; (4) Company EIN and DUNS number; (5) Price for each line item as specified in the attachment; (6) Terms for Payment; (7) Delivery Time ARO; (8) Warranty Provision; (9) Evaluation Factors - submit documents for each evaluation factor; Offer shall be submitted to Linda Conn, Contract Specialist via email at linda.conn@ihs.gov.
