Page 1 of 1 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures, specifically FAR Subpart 13.5 Simplified Procedures for Certain Commercial Items; and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-96, dated November 6, 2017. Solicitation # 36C26218Q0112 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award from this solicitation. The North American Industry Classification System (NAICS) Code is 811219 and the Small Business Administration s (SBA) size standard is $20.5 million for this procurement. This procurement will be issued as unrestricted. The Government is seeking a Contractor to furnish all labor, materials, supplies, equipment and transportation necessary to perform repairs, exchanges, and maintenance excluding misuse and abuse of the Alcon Centurion Phacoemulsifiers (3 each) and Alcon Constellation Vision System LXT Ophthalmic Laser (1 each) outlined in this request for the VA Loma Linda Healthcare System (VALLHS). Contractor shall adhere to all VA directives and policies regarding electrical equipment testing and maintenance, local, state, Federal and all other applicable laws / regulations. Place of Performance: VA Loma Linda Healthcare System (VALLHS) at the following facilities: (1) VA Loma Linda Healthcare, 11201 Benton Street Loma Linda, Ca. 92357; Period of Performance: 1 year. Provisions / Clauses: The following provisions / clauses: 52.212-1, Instructions to Offerors Commercial, 52.212-2, Evaluation Commercial Items, 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items are applicable to this acquisition. Please see the attached Statement of Work (SOW), additional clauses / provisions, and other pertinent information relative to this solicitation. Basis of Award: Award will be made to the responsible contractor whose quote conforms to the solicitation as it will be the most advantageous to the Government. Lowest price is the only factor that will be used to evaluate quotes (see FAR Provision 52.212-2). Award will be made to the lowest priced quote that is determined to be reasonable. Submitting Quotes: Quoter(s) are reminded to thoroughly review the attached Statement of Work (SOW) in its entirety and shall submit their quotes using the Your Company Letterhead. Quoter is advised the Government may be unable to receive other types of electronic files (e.g. compressed or zip files) or files larger than ten (10) megabytes (MB). It is the quoter s responsibility to ensure all required documents are included and completed as required by this solicitation. Quoter(s) shall submit their quotes to Corey R.
