REQUESTS FOR A SOLICITATION WILL NOT RECEIVE A RESPONSE. SOURCES SOUGHT: Veterans Health Affairs Network Contracting Office 4 is issuing this request for information/sources sought notice in order to identify contractors that can provide direct shipment of medical and surgical supplies both to VA Medical Centers and Veteran homes within the VISN 4 service area. SERVICES TO BE PROVIDED: A Statement of Work (SOW) is attached to this Sources Sought Notice. The SOW details the requirement. A list of estimated required items is attached to this Sources Sought Notice (Attachment 1 Item List). The estimated required quantities of the listed items are unknown at this time. Partnerships and subcontracting to meet the requirement is permitted. However, FAR 52.219-14 applies, which states that at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. As such, the prime contractor must be able to perform at least 50% of the required work. All proposed partnerships and subcontracting relationships should be discussed in the capability statement. The North American Industry Classification System Code (NAICS) is 624221 and the small business size standard is $11.0 million RESPONSES: NCO 4 is seeking responses from contractors that are interested in this requirement and can demonstrate their capability to provide the required services. Please respond with the following information: Company Information: Company Name Address, including all facility locations Point of contact Phone Number E-mail Address DUNS Number CAGE Code Tax ID Number Socio-economic size status (SDVOSB, VOSB, WOSB, SB, Large, etc.) A capability statement addressing your organization s ability to perform the required services. This must include a statement explaining your capability to service all VISN 4 sites, along with home-direct shipping to Veterans within those 9 sites service areas. If you are unable to provide service to all sites, indicate specifically which sites you would be able to service.
