Payment Info Certified Foods Cover Page Combined Solicitation 15BBR018QTA000001 Kosher RFQ This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation of offers that are being requested. The solicitation number is 15BBR018QTA000001 and this solicitation is issued as a Request for Quote (RFQ). PLEASE READ THROUGH ITS ENTIRITY. Solicitation is for Second Quarter Subsistence, FY 2018 (Kosher Items) For a list, description and total quantities of all items refer to Section 2 Commodity or Services Schedule. Solicitation is a 100% small business set aside. The NAICS for this requirement is 311991, 311812 & 311511 TERMS OF DELIVERY ARE F.O.B. DESTINATION TO: MDC Brooklyn 80 29th Street Brooklyn, NY 11232-1305 Contact the FSA or Assistant at 718-840-5000 x5301 x5304 x5305 x5307 Delivery Hours 8 AM to 1 PM, All frozen must be delivered by 12:00 noon For Items: Frozen Certified Food Entrees (Common Fare) For Kosher CRC Dairy & Bread Items Delivery two times a week: Wednesday & Friday 8:00AM to 10:00AM -or- MDC Brooklyn Food Service Warehouse Building 4, Flushing Avenue & Waverly Street (Across from 181 Flushing Avenue) Brooklyn, NY 11205 Contact Dexter Hilliman, 646-836-6591 Delivery Hours 8 AM to 12 noon All of the other items ordered will be delivered here The delivery is expected January 2, 2018, through January 31, 2018 for all items except Kosher CRC dairy and bread items. Any items shipped before or after the required delivery time-frame will be returned at vendor's expense. Upon Award, MDC Brooklyn Food Service or the Food Service Warehouse must be contacted prior to delivery for approval or adjustments in delivery schedule using the information above. Quoter(s) are advised the government reserves the right to make single or multiple awards. The government will award to the responsible quoter(s) whose quote, is conforming to the solicitation. Evaluation Factors are past performance, price, and delivery. Past performance is as important as price and non-price related factors. Award will be based on the best value to the government considering the evaluation factors listed above. Best value may be determined using tradeoffs among the evaluation factors, so that award may be made to other than the lowest-priced offer. Vendors who submit an offer concurs that delivery date can be met, unless otherwise noted. PAST PERFORMANCE RECORD OF "FAILURE TO DELIVER" ON PREVIOUS AWARDS MAY RESULT IN A LESS THAN FAVORABLE PERFORMANCE RATING. Piggy back offers will NOT be evaluated and therefore NOT considered for award. DO NOT CHANGE CASE COUNTS!! YOUR OFFER WILL NOT BE CONSIDERED! ALL BIDS MUST BE CLEAR! SUBSTITUTIONS TO SPECIFICATIONS IN LINE ITEMS ARE PROHIBITED (ex., SIZES, CASE COUNTS). CHANGES TO SPECIFICATIONS WILL NOT BE EVALUTED AND THEREFORE, NOT CONSIDERED FOR AWARD. THE GOVERNMENT RESERVES THE RIGHT UPON INSPECTION TO REJECT ITEMS IF THEY ARE DAMAGED OR NOT TO THE SPECIFICATIONS AS LISTED IN THE SOCLITATION. ALL BIDS MUST MEET SPECIFICATIONS For bid consideration for any kosher items offered, follow the Certified Religious Diet Specifications Quote Sheet in this solicitation. All kosher items offered on this bid must be certified by and accepted by the Orthodox Kosher Certification Agency or CRC Certificate for CRC items from the United States or Canada. Quoter(s) MUST include a Letter of Certification from the religious certification agency with all quote(s) SUBMITTED. Please submit the Kosher Certification(s) (date valid from time of bid submittal through the delivery quarter) with your quote. Follow the Certified Religious Diet Specifications Quote Sheet. Products other than canned, dairy, and bread items must be 3 months or younger (canned goods must be 1 year or younger) upon delivery with the date (including expiration date, best by date, sell by date, use by date or consumer by date) on the label. No stamped /written dates allowed. The completed package should be returned no later than 7:30 a.m. Eastern Standard Time, November 29, 2017. Vendors shall submit signed and dated offers to Carrie Lau, Contract Specialist by email to mRamos@bop.gov ONLY. No fax, hand delivery, nor mail-in will be accepted. Be sure to complete blocks 17a, 23, 24, 30a, 30b, and 30c of SF-1449 and the continuation sheets. Please include your TELEPHONE number, e-mail address AND DUNS in block 17a. ALL offers shall be submitted on the specific RFQ/SF1449. Information will be posted to the General Services Administration Federal Business Opportunities website: http://www.fbo.gov. All future information about this acquisition, including amendments, cancellations, award letter and low bid quote(s) will also be distributed through this site. Interested parties are responsible for monitoring this site to ensure they have the most up-to-date information about this solicitation. The anticipated award date is September 15, 2017. The only format in which questions "clarity only" will be answered by e-mail, (no phone calls please). Faith Based and Community Based Organizations have the right to submit offers or bids equally with other organizations for contracts for which they are eligible. All vendors must be completely registered in the System for Award Management, http://www.sam.gov prior to award, during performance and through final payment of any contract resulting from this solicitation. Do not provide a Unit Price past the 4th decimal point for your quote or that line time will not be considered. Please review all attachments. Religious Certification Requirements for Meals All Kosher hot entrée's must be certified Glatt Kosher "OU" all shelf stable kosher items must be certified "OU": Quoter(s) MUST include a valid Letter of Certification from the religious certification agency with all Quote(s) SUBMITTED. The Kosher Certification(s) must include the date valid from time of bid submittal through the delivery quarter with your quote. Items requiring "CRC" certification will be annotated. Kosher items will not contain alcohol, emulsifiers, mind altering substances, enzymes and/or chemicals. General Meal Specifications 1) The tray for all meals will be: a) Made with non-metallic packaging that allows microwave and conventional oven heating and be heat sealed directly to the tray with clear plastic and wrapped with a separate sealed clear plastic wrapper which encloses the entire tray. b) A three compartment tray with a large compartment of 21 fluid ounces and two compartments of 6 fluid ounces each. 2) All Meals will contain the food products per the individual product specification including the stated volume and/or weight of item as listed. 3) No substitutions may be made to the meats, vegetables, starches, sauces or preparation methods. 4) Meals is quoted as frozen. 5) Each individual meal must be marked with the certification symbol from the religious certification agency. 6) Heating directions must be provided for various wattage microwaves or oven heating including recommended method. 7) Mixed Vegetables will contain carrots, corn, peas, green beans, and lima beans with: a) Not more than 40 percent by weight carrots. b) Not more than 40 percent by weight corn (sweet, golden or yellow, whole kernel com). c) Not more than 40 percent by weight green peas. 8) Peas and Carrots will contain: a) Not more than 40 percent by weight carrots. b) Not more than 40 percent by weight green peas. 9) Mashed Potatoes will be made with commercially prepared potato flakes without peel, adding only water according to the manufacturers' preparation instructions; or fresh potatoes prepared using only water and spices with only the amount of binder which will ensure the product does separate in processing. No extenders of any type are allowed. 10) Tomato Sauce will be commercially prepared or crushed and/or pureed tomatoes, tomato paste and spices using only the amount of binder required to ensure the product does not separate during processing.
