- This solicitation is a "REQUEST FOR INFORMATION" only. This Sources Sought Notice is not a request for formal proposals or quotes. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. The intent is to award a Firm Fixed Price contract. Purpose and Objectives: STATEMENT OF WORK FOR PEST CONTROL SERVICE FOR THE DEPARTMENT OF VETERANS AFFAIRS WESTERN NEW YORK HEALTH CARE SYSTEM Contract Title. Pest Control Services for VAWNY Healthcare Service. Background. Contract to provide pest control service for VA Medical Centers at Buffalo and Batavia. Scope. Description of locations to be serviced: Department of Veterans Affairs Western New York Healthcare System (VAWNY), sites located at: VA Medical Center Buffalo - 3495 Bailey Avenue, Buffalo, NY VA Medical Center Batavia 222 Richmond, Batavia, NY Overview Description of Program: This specification is part of a comprehensive Integrated Pest Management (IPM) Program for the premises listed herein. IPM is a process for achieving long term, environmentally sound pest suppression through the use of a wide variety of technological and management practices. Control strategies in an IPM Program extend beyond the application of pesticides to include structural and procedural modifications that reduce the food, water, harborage, and access used by pests. Contractor Service Requirements: The Contractor shall furnish all supervision, labor, materials, and equipment necessary to accomplish the surveillance, trapping, pesticide application, and pest removal components of the IMP Program. The Contractor shall provide detailed, site-specific recommendations for structural and procedural modifications to aid in pest prevention. Administrative Tasks/Requirements: Service Calls: Contractor shall report to the Environmental Management Service (EMS) office, sign out badge and obtain complaints forms. Contractor shall conduct regular service and address complaints. Upon completion of service(s) the contractor shall fill out inspection report and provide disposition of actions for complaints. Contractor shall leave inspection reports with the Contracting Officer s Representative (COR) or his/her designee at the completion of each service. Complaints received shall be placed in a folder for the next service date. Pest Controller to check out with COR and let her/him know if any work orders are needed from that days service call. 24 hour/On-Call Communication: The Contractor shall provide a 24 hr/7 Day a week Emergency Telephone number. All calls must be responded to within thirty (30) minutes with a call back and estimated time of arrival within one hour, unless other arrangements are made at time of call back. Service Inspection Reports: Contractor shall fill out and return inspection report at end of service call. They should be dated and indicate time in/out and signed by technician. The VA Inspection Report and Complaint Form are attached below. There will be one point of contact for the VA, the COR or their designee, complaints will be provided to the contractor by this contact person. The inspection report will detail conditions found, service(s) provided, chemical(s) used and structure and sanitation issues. The contractor will also recommend appropriate actions that are consistent with Federal, State, Municipality, Joint Commission, and OSHA regulations. Where there is a conflict between applicable regulations, the most stringent will apply. Contractor shall report any structural weaknesses with regards to pest control to the COR in writing. Contractor must communicate in person with EMS prior to and immediately after service and will submit all documentation as defined herein. Contractor shall fill out the information below monthly and supply copy to EMS, can be faxed to 716-862-8618, emailed to COR or dropped off on first service call of new month: PEST CONTROL SERVICES MONTH / YEAR: ________________________ GALLONS POUNDS HERBICIDES RODENTICIDES INSECTICIDE MONTHLY TOTAL SDS Sheets: Provide COR with a hardcopy of all Safety Data Sheets (SDS) and a hardcopy of all specimen labels for all pesticides to be used at this facility before contract award. Facility Keys: Contractor shall ask COR or EMS Representative for keys to sign out and return them at end of each service call. Keys shall NOT leave the facility. If facility keys are lost by contractor, contactor is responsible for payment to replace locks and keys. Keys in Buffalo are obtained in the EMS office and in Batavia in the engineering office. For after hours service calls the Contractor shall report to the VA Police. Facility Contractor Badge: Contractor shall sign out badge from EMS office at beginning of service and sign back in badge at end of service call. Badges shall NOT leave the facility. Badges are obtained in the EMS offices in Buffalo and Batavia. Buffalo Office is in Building 1, Room 63 and Batavia Office is in Building 3, Room 104. For after hours service calls the Contractor shall report to the VA Police. Service Visit Requirements/Hours of Service: Buffalo site will have inspections, treatments, and recommendations twice a week to be conducted as noted herein. Batavia site will have inspections, treatments, and recommendations twice a week to be conducted as noted herein. Working Hours: Regular business hours of the EMS office are between the hours of 6:00 AM and 2:00 PM Monday-Friday. Regular service can be provided during this time frame. The Contractor shall provide a work schedule with a time frame and day of service (i.e. Tuesday August 7th, 2012 7:00 AM EST) to the COR no later than the contract start date. The work schedule must be approved by the COR. This schedule will include some off tour service times and will include the outer buildings so notification can be given to those staff members. Pest management tasks shall be scheduled so as to be performed on definite appointed days and in no event shall the Contractor carry on work outside regular agreed-upon working days or hours without prior approval of the EMS Manager or COR. This service will include scheduled pest management of areas stated in section 5(iv) to include rotating inspection of each floor, as well as treatment required as a result of VA pest complaints. Schedule of Buildings: The contractor shall formulate an IPM Program for VAWNY Buffalo and Batavia. A copy of this will be submitted for review and comment within 30 thirty of Notice to Proceed. A final copy will be kept in the EMS Office. Twice-Weekly Service: Buffalo: Building #1 (Main Building): 14 floor high rise including sub-basement, basement, and ambulatory care wing. Each floor (sub-basement through 13th floor) shall be inspected and serviced once per month (on a rotational basis) in addition to complaint forms and below requirements: Each of the below areas shall be inspected and serviced a minimum of once a week: Canteen dining room(s), vending rooms, kitchen and retail store located in basement, Building #1. Nutrition and Food Services Kitchen and storerooms located in basement, Building #1. Trash compactor, trailers, waste storage areas, and laundry located within Building #6. Exterior areas around trash compactor, trailers, and waste storage areas. Two (2) locker rooms located in basement, A wing, Building #1 (Men s and Women s). Warehouse located in basement, C wing, Building #1. Child Day Care, Building #3 and Children Waiting Center, 4th floor of Building #1- Any and all types of control in the child care facilities must be done with baits only. If more aggressive treatment is necessary it must be scheduled one (1) week prior to work as it requires the pre-notification of Staffs, Parents, and Children. Animal Research facility, Building #20. Sub-basement with specific attention given to the crawl spaces, pump rooms, elevator pit, and four (4) soiled linen chute rooms, Building #1. Sub-basement will be treated once a week, rotating the Wings, i.e. A-Wing, B-Wing, C-Wing, D-Wing and E-Wing. Quarterly Fog treatments in sub-basement. Manholes outside the facility for American cockroach control. Quarterly fog sub-basement after hours Twice-Weekly Service: Batavia: Buildings #1 (Main Building): Four (4) story building including basement and ambulatory care area. Each floor (basement through 4th floor) and Buildings 2, 3, 4 shall be inspected and serviced once per month (on a rotational basis) in addition are the below requirements. Each of the below areas shall be inspected and serviced a minimum of once a week: Canteen dining room, vending room, kitchen and retail store located in basement, Building #1, and basement, Building #2. Nutrition & Food Services kitchen and storeroom located on 1st and basement floor of Building #2. Trash compactor and waste storage areas located within basement, Building #2. Exterior areas around trash compactor and waste storage areas. One (1) chute room located on basement floor, Building #1. Warehouse, Building #10. Two (2) locker rooms located on basement, Building #2. Buildings #5 (3 story building) and #6 (2 story building and basement) Steam tunnels shall be inspected and serviced for American cockroaches once a week. Legal Holidays: New Year s Day, Martin Luther King Day, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving and Christmas Day. Refer to Emergency Telephone Requests paragraph below if services are needed on a holiday. Inclement Weather: In the event that climate or weather conditions become unsuitable for work or may induce an environmental hazard, the work shall be rescheduled to a date and time satisfactory to the Contractor and COR. Postponement will not reflect any penalties assessed to the Contractor, nor should there be any additional cost to the Government. Response Time: The contractor will respond verbally within thirty (30) minutes of initial contact from VAWNY staff and will work with the COR or designee to negotiate and establish on-site response time based on severity levels of pest situations. Emergency telephone requests made by EMS shall require a call back within 30 minutes and on-site service within one (1) hour unless other arrangements are made at time of call back. Emergency service calls due to unforeseen issues in addition to the contractual requirements will be billed at prevailing wage. If the call is a result of a recurring issue or an oversight in the execution of contractual requirements the cost will not be reimbursed. This includes issues that should have been observed during the execution of contract work (this may include, but is not limited to, call backs for pests like roaches and bees that should be identified during regular services). Pest Management Specifications: Standard service shall include the following: Inspection to determine which pest management measures are appropriate and required. Contractor shall develop a comprehensive IPM Program to address the management of pests at VAWNY Buffalo and Batavia. Provide recommendation of environmental sanitation practices that restrict or eliminate food, water or harborage for pests. Provide recommendation of general structural changes to eliminate pests. Recommendations shall be complete with detail of the issue and expected result. All SDS sheets for products to be used will be sent to COR before contract start date. The intent of the IPM Program is the control and removal of insects, pests and rodents detrimental to the health and welfare of VAWNY Employees, patients, and visitors. IPM Program services include, but are not limited to the following pests: General structural arthropod pests (i.e. cockroaches, ants, spiders, yellow jackets, wasps, and hornets). Flying insect (i.e. housefly, stable fly, blowflies, drain flies) This also requires the servicing of the interior fly units. Stored product pests (beetles, moths) Mice and rats Predatory pests (i.e., lice, bedbugs, fleas, mites, ticks, bees, wasps, mosquitoes). Nuisance Wildlife (i.e. squirrels, gophers, moles, skunks, snakes, rabbits, raccoons, bats) Wood destroying organisms (i.e. subterranean termites, dry wood termites, fungi, wood boring beetles, carpenter ants) Wildlife Control: If wildlife such as, but not limited to: raccoons, squirrels, skunks, or bats become a nuisance in or around the facilities, the contractor will provide humane removal under the guidelines of applicable state and federal regulations. Often wildlife calls will be emergencies and should be responded to within one (1) hour or less. If human and animal contact has occurred it may be required to provide the captured animal to the County Department of Health for testing. Insect Control: Non-pesticide Methods of Control: The Contractor shall use non-pesticide methods of control wherever possible. For Example: Portable vacuums rather than pesticide sprays shall be used for initial cleanouts of cockroach infestations, for swarming (winged) ants and termites, and for control of spiders in webs wherever appropriate. Trapping devices rather that pesticide sprays shall be used for indoor fly control wherever appropriate Application of Insecticides to Cracks and Crevices: As a general rule, the Contractor shall apply all insecticides as crack and crevice treatments only, defined in the contract as treatments in which the formulated insecticide is not visible to a bystander during or after the application process. Application of Insecticides for Exposed Surfaces or as Space Sprays: Application of insecticides to exposed surfaces or as space sprays (including fogs, mists, and ultra-low volume applications) shall be restricted to unique situations where no alternative measures are practical. The Contractor shall obtain the approval of the COR prior to any application of insecticide to an exposed surface or any space spray treatment. No surface application or space spray shall be made while tenant personnel are present. The Contractor shall take all necessary precautions to ensure patient and employee safety, and all necessary steps to ensure the containment of the pesticide to the site of application. Residual, contact, and dust insecticides will be limited to food and beverage and common areas. The use of relatively non-toxic baits will be used in patient rooms, ICU, surgery, child care areas and other medical units. Insecticide Bait Formulations: Bait formulations shall be used for cockroach and ant control wherever appropriate. All baits must be odorless, non-flammable, and non-corrosive, have no inhalation hazard and must be classified as category IV pesticides, the lowest EPA toxicity rating. Monitoring: Sticky traps shall be used to guide and evaluate indoor insect control efforts wherever necessary. Rodent Control: Exterior: Contractor shall intercept and control rodents before they enter the facilities. Contractor shall install and maintain sufficient bait stations and will document placement on an equipment diagram. A copy shall be given to the EMS office to be kept on file. Contractor shall service all bait stations every week. Indoor Trapping: As a general rule, rodent control inside occupied buildings shall be accomplished with trapping devices only. Contractor shall dispose of all trapped rodents and all rodent carcasses off-site in an appropriate manner. Contractor shall document placement on a rodent equipment diagram. A copy of the diagram shall be given to the EMS Office. All traps shall be placed discretely and not to interrupt staff or patient care. Use of Rodenticides: In exceptional circumstances, when rodenticides are deemed essential for adequate rodent control inside occupied buildings, the contractor shall obtain the approval of the COR prior to making any interior rodenticide treatment. All rodenticides, regardless of packaging, shall be placed either in locations not accessible to children, pets, wildlife, and domestic animals, or in EPA approved tamper-resistant bait boxes. As a general rule, rodenticide application outside buildings shall emphasize the direct treatment of rodent burrows wherever feasible. Use of Bait Boxes: Frequency of bait box servicing shall depend upon the level of rodent infestation. All bait boxes shall be maintained in accordance with EPA regulations, with an emphasis on the safety of non-target organisms. The Contractor shall adhere to the following five points: All bait boxes shall be placed out of the general view, in locations where they will not be disturbed by routine operations. The lids of all bait boxes shall be locked shut Bait shall always be placed in the baffle-protected feeding chamber of the box and never in the runway of the box. All bait boxes shall be labeled on the inside with the Contractor s business name and address, and dated by the contractor s technician at the time of installation and each servicing. Reporting: A rodent diagram will be included in the service log book. All bait stations will be locked and secured. All traps and bait stations will be dated during monthly inspection and log sheets will note the conditions found during the time of service. General Requirements: Interference To Normal Function: Contractor personnel are required to interrupt their work at any time so as not to interfere with the normal functioning of any VAWNY areas, including utility services, fire protection systems and passage of facility patients, personnel, visitors, equipment and carts. Use of Pesticides: The Contractor shall be responsible for application of pesticides according to the label. All pesticides used by the Contractor must be registered with the U.S. Environmental Protection Agency (EPA). Transport, handling, and use of all pesticides shall be in strict accordance with the manufacturer s label instructions and all applicable Federal, state, and local laws and regulations. The Contractor shall adhere to the following rules for pesticide use. The Contractor shall not apply any pesticide product that has not been included in the IPM Program or approved in writing by the COR. Storage of Pest Control Materials: No pest control materials or equipment shall be stored or kept at the VA Medical Center without express approval of the COR. Formulated Pesticides: Contractors shall not formulate pesticides from concentrates on VAWNY property without written approval by the COR. Application by Need: Pesticide application shall be according to need and not by schedule. As a general rule, application of pesticides in any inside or outside area shall not occur unless visual inspections or monitoring devices indicate the presence of pests in that specific area. Preventive pesticide treatments of areas where surveillance indicates a potential insect or rodent infestation are acceptable on a case-by-case basis. Written approval must be granted by the COR prior to any preventive pesticide applications. Disposal of Hazardous Wastes: The Contractor shall not dispose of any excess pesticide containers or any other materials contaminated by pesticides at any location on the medical facility premises except as specified by the facility GEMS Coordinator. Ban of 2,4D -The Department of Veterans Affairs has banned the usage of 2,4-Dichlorophenoxy Acetic Acid (2,4-D) Contractor Personnel Certification: Throughout the term of this contract, all Contractor personnel providing on-site pesticide application must maintain certification as a Commercial Pesticide Applicators in the category of Industrial, Institutional, Structural, and Health Related Pest Control. Uncertified individuals working under the supervision of a Certified Applicator will not be permitted to apply pesticides under this contract. Contractor shall supply a copy to the Contracting Officer for each employee and shall provide copies of updated certification as needed. Copies of all certifications shall be kept in the EMS Office. Safety and Health: The Contractor shall observe all safety precautions throughout the performance of this contract. All work shall comply with the applicable requirements of Title 29 Code of Federal Regulations (CFR) 1910, 29 CFR 1926, and 40 CFE 751. All work shall comply with applicable state and municipal safety and health requirements. Where there is a conflict between applicable regulations, the most stringent will apply The Contractor shall assume full responsibility and liability for compliance with all applicable regulations pertaining to the health and safety of personnel during the execution of work. Special Entrance: Certain areas within some buildings may require special instructions for persons entering them. Any restrictions associated with these special areas will be explained by the COR. The Contractor shall adhere to these restrictions and incorporate them into the IPM Program. Structural Modifications and Recommendations: Throughout the term of this contract, the Contractor shall be responsible for advising the COR and EMS Manager about any structural, sanitary, or procedural modifications that would reduce pest food, water, harborage or access. The Contractor shall be responsible for adequately suppressing all pests included in this contract regardless of whether or not the suggested modifications are implemented. The Contractor will not be held responsible for carrying out structural modifications as part of the pest control effort. However, minor adjustment, such as the removal of awnings, gutters, or minor applications of caulk and other sealing materials by the Contractor to eliminate pest harborage or access may be requested by the EMS Manager or COR on a case-by-case basis. The Contractor shall obtain the approval of the COR prior to any application of sealing material or other structural modification. This contract does include the removal of flying pests (i.e. birds, bees, and bats) and some work may require the use of lifts or ladders which are to be provided by the Contractor at no additional cost to the government. Uniforms and Protective Clothing: All Contractor personnel working in or around buildings designated under this contract shall wear distinctive uniform clothing. The contractor shall determine the need for and provide any personal protective items required for the safe performance of work. Protective clothing, equipment, and devices shall, as a minimum, conform to the United States (U.S.) Occupational Safety and Health Administration (OSHA) standards for the products being used. Vehicles: Vehicles used by the contractor shall be identified in accordance with state and local regulations. Invoices: Invoices shall be submitted in arrears to the Fiscal Services Center. Invoices submitted electronically. Contractor shall clearly state the purchase order number on each invoice. ANY ADDITIONAL CHARGES CLAIMED MUST BE APPROVED BY THE CONTRACTING OFFICER BEFORE SERVICES ARE RENDERED. Quality Assurance Surveillance Plan (QASP): The QASP under the proposed contract shall be based upon the timeliness and quality of the services provided. These measurements are defined as: Timeliness Required services and reports are provided in accordance with the stated period of performance, scheduled timelines, and hours of operation as set forth within this solicitation package. Quality Services provided under the proposed contact will be measured based upon the Government s determination of the validity, accuracy, clarity and usefulness of the service or if the work performance has met the requirements listed as communicated to the contractor by the Government. Quality will be determined by the number and type of complaints, results of random and scheduled inspections, and the quality of reporting as defined in Section 4ii Service/Inspection Reports. A successful program will be considered satisfactory when pest numbers are being steadily reduced to the point where they are not interfering with operations, destroying property, or adversely affecting health and morale of patients, staff, and guests. Security Requirements: All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA COR, be identified by the contract and employer, and restricted from unauthorized access. Contractor vehicles shall be parked in designated areas of the campus. Request from the VA COR will be needed when picking up and dropping off materials and supplies, if necessary. Performance Monitoring: Performance monitoring shall be conducted by the designated VA COR in accordance to OEM specific guidance; and all salient characteristics as described by this statement of work. VA COR shall inform the CO of any discrepancies. No payments shall be made before acceptance by designated VA COR representative occurs. Place of Performance. Buffalo VA Medical Center 3495 Bailey Avenue Buffalo, NY 14215 Batavia VA Medical Center 222 Richmond Avenue Batavia, NY, 14020 Period of Performance. 03/01/2018 to 09/30/2018 with four (4) option years to be exercised at the discretion of the government.
