This contract is to provide one (1) Outpatient Physical Therapist (OP PT) at the location below: V.A. Southern Oregon Rehabilitation Center & Clinics 8495 Crater Lake Hwy White City, OR 97503 The Physical Therapy (PT) Practitioners shall see an average of 8-10 patients per day in the PMRS PT Outpatient Clinic located at the VA Southern Oregon Rehabilitation Center & Clinics. The OP Physical Therapist will be assigned to Physical Medicine & Rehabilitation Services from Monday Friday, 8:00 a.m. to 4:30 p.m., shifts are five (5) days per week, with 30 minutes for lunch. There is no on-call duty or holiday duty for this position. The OP PT Practitioner(s) shall have responsibility for examining, evaluating, diagnosing and treating veterans with neuromusculoskeletal conditions, among others, in a fast-paced, direct-access environment. The PT Practitioners shall complete all appropriate documentation of services in the Computerized Patient Record System (CPRS) including all required clinical reminders. This procurement will use cascading procedures the levels are: 1. Service Disabled Veteran Owned Small Business (SDVOSB), 2. Veteran Owned Small Business (VOSB), and Small Business (SB). SDVOSB and VOSB must have a current Veteran Information Pages (VIP) certificate to be considered as a SDVOSB or VOSB. Without the certificate, they will be considered a SB.
