- STATEMENT OF WORK Tree & Shrub Removal INTRODUCTION: Environmental Management Service is committed and proudly serves the VA-HVHCS, Montrose & Castle Point Campuses, establishing a clean environment for health care to veterans and the highest standards of service to customers. We do this by keeping a sanitary and safe environment for patients, visitors and staff. ORGANIZATION: Veterans Health Administration provides administrative support of VA Medical Treatment Facilities (MTF's) located worldwide. BACKGROUND: VHA provides facility maintenance support to VA medical treatment facilities (MTF) worldwide. MTFs are responsible for providing health care to working and retired government personnel. Maintenance/repair of equipment at MTFs may have been inconsistent due to non- standardized equipment, service levels, response times, and costs. The absence of standardization may have resulted in inefficiencies in repairs and maintenance schedules that may have resulted in excessive costs and downtimes. PERIOD OF PERFORMANCE: The period of performance for this contract is expected to be from December 1, 2017 to 12/31/2017. Work Hours: Normal Work Hours: (7:00 am to 3:30 pm) National Holidays: The ten holidays observed by the Federal Government are: New Year s Day, Martin Luther King s Birthday, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. Work can be performed on weekends CHANGES TO STATEMENT OF WORK: Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a COR folder along with all other required documents. Costs incurred by the vendor through the actions of parties other than the Contracting Officer shall be borne by the vendor. SCOPE OF WORK: The Contractor shall provide all labor, equipment, supervision, and management to provide on site material Tree & Shrub Care Services for damaged Trees located in High Traffic Areas on the Grounds of the Veteran Administration Hospital in Montrose and Castle Point, New York. A Tree Risk Assessment performed by professional Arborist revealed the physical conditions of damaged Trees and report on other potential Tree-related hazards. The Montrose area has experienced large branch failure along the main exit and potential Tree failures from other damaged Trees. The Contractor shall perform all services On-Site at the Department of Veterans Affairs, Veterans Hospital Administration, Hudson Valley VA, located at 2094 Albany Post Road, Montrose, NY 10548 and Castle Point, 41 Castle Point Rd., Wappinger's Falls, NY 12590. The Contractor shall perform maintenance pruning to properly remove all dead, weak, broken and hanging, excessively overhanging branches; elevated branches from light fixtures/wires, leaning, declining, deadwood, and complete Tree removal to grade. Perform maintenance pruning to cut back, elevated and remove excessive overhang of all branches, for Trees & Shrub Care Service in accordance with the specifications, terms and conditions of this solicitation for a period of All work is to be performed in accordance with (IAW) federal, state and local laws and ordinances of the State of New York. OBJECTIVES: The objective of this contract is to ensure the legal, proper and physical removal and destruction of damaged Trees & Shrubs, to provide the Department of Veterans Affairs, Veterans and employees safety, to include but not limited to: Montrose - Front Entrance Area 27" diameter storm damage White Oak - located in adjacent to the exit lane and security booth -Hazard Tree. Perform complete tree and stump removal, and grind down stump 6" -8" below grade. Adjacent 19" diameter Red Maple - Hazard Tree -has substantial decay at the base. Perform complete tree removal to the grade. 26" diameter dead Hemlock -located in the wood line adjacent to exit lane of facility - Hazard Tree - Perform complete removal to grade. 10" diameter dead Hemlock -covered in bittersweet and poison ivy vines located in wood line adjacent to exit road -Hazard Tree. Perform complete removal to grade. 14" diameter Black Birch Trunk -dead with broken top, located in wood line adjacent to exit road -Hazard Tree. Perform complete removal to grade. 26" diameter Dead Pin Oak - adjacent to exit road and opposite guard booth -Hazard Tree. Perform complete removal; grind down stump 6"-8" below grade. 10" diameter Dead White Oak - in wood line adjacent to exit road opposite guard booth - Hazard Tree. Perform complete removal to grade. Three Dead Hemlock trees -located on hillside adjacent to exit -Hazard Tree. Perform complete removal to grade. 20"diameter uprooting Red Maple -in the lawn area adjacent to entrance -Hazard Tree. Perform complete tree removal; grind down stump 6"- 8" below grade. 24" diameter Dead Black Birch Tree -located adjacent to sidewalk at the entrance - Hazard Tree. Perform complete tree removal; grind down stump 6"- 8" below grade. Three Red Maples 10", 18" and 20" diameter -with severe decline and decay in crowns located adjacent to Hudson Valley Healthcare Systems sign and pathway to building 52 -Hazard Trees. Perform complete tree removal; grind down stumps 6"- 8" below grade. Montrose Campus - Building 52 and 28 18" diameter declining Red Maple - with dead crown -Hazard Tree. Perform complete tree removal; grind down stumps 6" - 8" below grade. Dead Twin Black Birch Trees 16" and 14" diameter respectively - located in wood line adjacent to sidewalk at entrance to Building 52. Perform complete tree removal - Trees; grind down stumps 6"-8"below grade. 38" diameter storm damaged Red Oak -with severe trunk decay located adjacent to the sidewalk at entrance to Build 52 -Hazard Tree. Perform complete tree removal flush to grade. 18" diameter dead Black Birch tree - located in wood line adjacent to sidewalk Hazard Tree. Perform complete tree removal to grade. 24" diameter dead Hickory -located on hillside adjacent to roadway to Building 28 - Hazard Tree. Perform complete removal flush to grade. 38" diameter Red Oak - located at North Side of roadway, at entrance to Parking lot L -Hazard Tree. This tree has multiple vertical cracks at the base and is leaning heavily over roadway. Perform complete tree removal to grade. 18"diameter Dead Black Birch and 12" diameter Hickory - located on hillside above roadway to the entrance of Building 28 -Hazard Trees. Perform complete tree removal to grade. 14" diameter uprooting Red Oak -on hillside opposite entrance to Building 28 - Hazard Tree. Perform complete tree removal flush to grade. 18" diameter Dead Black Birch Tree -located on hillside opposite at entrance to Building 28 -Hazard Tree. Perform complete tree removal to grade. 23" diameter dead Hickory Tree -located in the lawn area adjacent to the bus stop in front of Building 28 -Hazard Tree. Contractor shall execute the terms of this contract or cause to be performed in a manner with regards to safety of veterans and employees. Contractor shall allow only individuals aged 18 or over, who have appropriate contractor identification, to assist with the felling/removal. Contractor's employees must have visible on their uniform, their name and company's logo. Contractor personnel involved in the actual performance of this contract shall wear a contractor provided, clearly readable identification badge that at a minimum states, the name of the employee and the company. Contractor personnel's clothes are to be clean and maintained in good repair. Contractor shall be required to interrupt their work at any time so as not to interfere with the normal functioning at any facility, including utility services, fire protection systems, personnel, equipment and carts.
