- THIS IS NOT A SOLICITATION. This is a Sources Sought Notice only and is issued in accordance with Federal Acquisition Regulation (FAR) 15.201(e) in order to conduct market research. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This notice is issued solely for information and planning purposes and does not constitute a Request for Proposal (RFP) or a guarantee to issue an RFP in the future. This notice does not commit the Government to contract for any supply or service. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. Not responding to this notice does not preclude participation in any future RFP, if issued. This announcement is based on the best information available at this time and is subject to future amendment. The anticipated North American Industry Classification System (NAICS) Code is 624221 Temporary Shelters, Small Business Size Standard $11.0 Million. OVERVIEW: The Department of Veterans Affairs (VA), Network Contracting Office (NCO) 2, and the Canandaigua VA Medical Center (VAMC) anticipate a requirement for low-demand Safe Haven Housing Services as part of its community based Health Care for Homeless Veterans (HCHV) program. The goal of the HCHV program is to remove homeless Veteran families from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient supportive services to meet their needs and ultimately facilitate the improvement of their housing situation. The purpose of this Sources Sought Notice is to obtain interest from Contractors with facilities located within fifteen (15) miles from the Canandaigua VAMC (400 Fort Hill Avenue, Canandaigua, NY 14424) that can provide housing and supportive services to Homeless Veterans in community-based facilities offering a safe and secure environment that supports their goals for recovery from homelessness. It is understood that the Veterans being served through this contract may have been diagnosed with severe mental illness and/or substance use disorders. The Contractor will be required to provide residential and case management services, but shall not provide clinical or detoxification or other hospital level treatment those services will be provided by the VA at VA facilities. The facility must be able to accommodate both male and female Veterans, ensuring adequate privacy and safety for all. The Contractor shall offer: Room and board to include a bed and other furnishings, such as a dresser, storage (to include at least one lockable drawer), and personal linens (towels, bed sheets, blankets, and pillow.) Mattresses must be in good condition and fully encased with protective covering. Residents must be provided with basic personal hygiene products. Meals shall include at least three (3) nutritionally adequate meals a day, 7 days a week and availability of nutritious snacks between meals and bedtime. Laundry facilities and products shall be available for no additional fees for residents to do their own laundry or to have laundry done. Therapeutic and Rehabilitative Services determined to be needed by each individual Veteran referred for treatment as stated in the plan developed by the contractor, with input from the Veteran and the VA HCHV Liaison (or designee). A low-demand environment with a minimum set of rules designed to re-establish trust in the homeless Veteran population and engage the Veteran in needed treatment services. Acceptance of treatment will not be a condition of admission or continued stay; Abstinence from alcohol or drugs will not be a condition of admission or continued stay; and, Infractions of rules should be viewed as opportunities for engagement. Structured group activities such as social recreational/peer support groups, linkage to vocational services/counseling and physical activities as appropriate. Assistance with local transportation to scheduled meetings and appointments related to the recovery plan of the Veteran. The Contractor s facility must: Have a current occupancy certificate issued by the local/state government in the jurisdiction where the facility is located; Be in compliance with all standards of State and local safety, health and sanitation codes. Be equipped with operational heating and ventilation systems. Provide separate and secure sleeping and bathroom space for male and female Veterans. Meet the requirements of the applicable residential occupancy chapters of the current version of NFPA 101, National Fire Protection Association's Life Safety Code. Successfully pass an annual inspection conducted by a multidisciplinary VA team consisting of a social worker, dietitian, registered nurse, a representative of the VA Police, and a Facilities Management Safety Officer, and other subject matter experts as determined necessary A breakdown of historical usage is below: Fiscal Year Average Monthly Bed Days of Care FY 17 410 FY 16 428 FY 15 422 SUBMISSION INSTRUCTIONS: Please submit your response in accordance with the following instructions: Responses shall be no more than 5 pages in length.
