Description

This is a request for quote (RFQ) to sources that can provide rock salt for the Edward Hines Jr VAMC, Hines, IL. The Dept of Veterans Affairs has not made a commitment to procure any of the items discussed, and the release of this RFQ should not be construed as such a commitment or as authorization to incur cost for which reimbursement would be required or sought. All submissions become government property and will not be returned. Responders are advised that the US Government will not pay for any information or administrative costs incurred in response to this RFQ. CONTRACT ADMINISTRATION DATA (Continuation from Standard Form 1449, block 18A.) 1. Contract Administration: All contract administration matters will be handled by the following individuals: A. SCHEDULE OF SUPPLIES/SERVICES AND PRICES/COSTS (SF 1449, Continuation of Blocks 19 24) Contractor shall provide the following road salt for the Edward Hines Jr VAMC, Hines, IL. Offeror shall provide the following product: Item No. Description QTY UNIT UNIT PRICE EST TOTAL PRICE 1 Rock Salt 500 Ton TOTAL Salient characteristics: 1. Road salt to be free of sand, stone, fillers, or debris. Upon inspection, if road salt is found to be contaminated with any kind of debris, the shipment/delivery will be halted. 2. Weight tickets must be provided upon delivery. Loads of salt will not be accepted unless a delivery ticket is provided upon arrival. 3. Delivery requires use of a truck capable of loading a spreader truck. 4. The government reserves the right to change the amount of salt delivered. 5. The contractor is responsible for immediately cleaning up any spills, debris, etc., which may occur during delivery.
